N-desmopressin spray is used in the complex therapy of non-specific diabetes insipidus, a disease in which there is a violation of water reabsorption in the renal tubules, and pathologically large volumes of fluid are excreted from the body through the bladder.

It can also be used as an additional drug for urinary incontinence in children and adults, for residual polyuria due to surgical treatment of the pituitary gland, for isolated pituitary gland syndrome. H-desmopressin spray is used in the diagnosis of functional disorders of the kidneys.

An indication for the use of H-desmopressin spray may be to stop nosebleeds in patients with uncomplicated hemophilia A. This effect is achieved due to the fibrinolytic properties of the drug.

Release form

Composition: desmopressin acetate 0.1 mg/ml, isotonic aqueous sodium chloride solution, benzalkonium chloride (preservative), citric acid, sodium phosphate and purified water.

N-desmopressin spray is an individual dosed 0.01% nasal spray. It is available in two volumes: 2.5 ml and 5 ml (respectively, 25 and 50 doses, 10 mcg of desmopressin acetate per dose). A bottle with a dosed dropper cap and an adapter.

Pharmacodynamics

H-desmopressin spray (I-diamine-8D-arginine vasopressin), a synthetic derivative of antidiuretic hormone (ADH).

Enhances the hydropermeability of renal tubule tissues and restores the function of water reabsorption. Has the property of activating the process of blood clotting in patients with hereditary hemophilia. Has a slight effect on the smooth muscles of the vascular wall and abdominal organs. Increases the osmotic pressure of blood plasma, which leads to a reduction in the number of urges to urinate and relief of the problem of fluid deficiency in the body.

Pharmacokinetics

When using H-desmopressin spray, about 25% of desmopressin acetate is adsorbed in the nasal cavity, the antidiuretic effect appears and increases within 1 hour and lasts up to 20 hours, the maximum concentration of the drug in the blood is observed within the first five hours after use.

Taking the tablet form of H-desmopressin allows you to achieve the effect just as quickly, but the duration of the drug's action is reduced to 8 hours. When taken orally, only 5% of the drug is absorbed, since H-desmopressin is decomposed by gastric enzymes. Therefore, sublingual use of the tablet form of the drug is recommended, or, more appropriately, the use of H-desmopressin spray.

Dosing and administration

There are two main methods of using desmopressin: intranasal and tablet.

N-desmopressin spray is used in strictly individual dosages as prescribed by a doctor. One drop of the drug contains 5 mcg of the active substance. The usual dosage is 1-4 drops of 0.1% desmopressin acetate solution intranasally every 12 hours. In severe forms of the disease, it can be used every 8 hours.

For the treatment of primary enuresis, use 1 drop immediately before bedtime.

We also use H-desmopressin spray when examining the concentration function of the kidneys: children are prescribed 1 drop, and adults – 2 drops once.

Use N-desmopressin spray 10. during pregnancy

There is no scientifically proven information on the use of H-desmopressin spray during pregnancy and lactation.

Specialized clinical studies on the use of this drug have not been conducted, and the presence of a teratogenic effect has not been proven.

Currently, there are more than 50 known cases of the use of H-desmopressin spray in pregnant women: no harm to the fetus or the expectant mother has been found.

Normal doses of the drug do not cross the placental barrier. Examinations of reproductive capacity in rabbits and rats did not reveal any pathological abnormalities in the fetuses during the use of H-desmopressin spray.

In any case, the drug should be prescribed and monitored by a doctor, determining the possible degree of risk for the woman and her future child.

Contraindications

Obvious contraindications to the use of H-desmopressin spray are heart failure (especially in the decompensation stage), ischemic heart disease, severe hypertension, cardiac arrhythmia, epilepsy, and complete anuria.

It is recommended to use with caution in cases of edema of various etiologies, hypersensitivity to drugs analogous to ADH, as well as in the treatment of the elderly and children.

Overdose

When using the drug in quantities exceeding 800 mcg, it is necessary to monitor osmotic pressure and water balance, especially in childhood and adolescence, in order to prevent hyperhydration of the body, as well as monitoring diuresis, blood pressure and heart rate. Interactions with other drugs

The vasoconstriction effect and stimulating effect on uterine tone are enhanced when used in combination with methylergometrine and oxytocin. The simultaneous use of H-desmopressin spray and cardiac drugs that reduce the heart rate may cause severe bradycardia.

The interaction of H-desmopressin spray with indomethacin, chlorpramide, clofibrate enhances the antidiuretic effect of desmopressin, while carbamazepine and glybutide, on the contrary, inhibit it.

H-desmopressin spray accelerates the effect of other drugs that increase blood pressure.

Storage conditions

The most acceptable storage conditions for N-desmopressin spray are in a dry, cool place protected from light, such as a refrigerator. Storage temperature from +2 to +10 C. It is strictly not subject to freezing!
