Enadipine is a complex drug with hypotensive properties.
Indications Enadipine
It is used to reduce high blood pressure, as well as to treat angina pectoris and variant angina.
Release form
The therapeutic agent is released in tablet form.
Pharmacodynamics
Enadipine is a combination drug with a hypotensive effect, containing the substances enalapril with amlodipine. The medicinal effect is provided by the properties of its active elements.
Amlodipine inhibits the activity of slow Ca channels and also blocks the penetration of calcium ions through the walls of slow Ca channels into vascular cells and smooth muscle cardiomyocytes. As a result, the overall resistance of peripheral vessels decreases, which leads to a decrease in blood pressure.
This does not lead to a change in heart rate indicators, and as a result of reducing the load on the heart muscle, the heart's need for oxygen decreases.
The therapeutic effect allows to expand the main arterioles with coronary arteries - inside the ischemic and healthy myocardial zones. Due to this, the passage of oxygen into the myocardium is enhanced, and in people with variant angina, it prevents the occurrence of coronary spasms. In people with increased blood pressure values, a single daily dose of the drug leads to a decrease in these indicators for 24 hours. Amlodipine has a gradual onset of action, due to which it does not cause signs of acute hypotension.
In people with angina, taking a single dose of Enadipine per day increases exercise tolerance, reduces the frequency of angina attacks and the number of nitroglycerin tablets taken.
Enalapril maleate is an ACE inhibitor. It inhibits the formation of angiotensin 1 and 2. During its action, the overall resistance of peripheral vessels, blood pressure, and pre- and post-load in relation to the myocardium decrease. In addition, the substance reduces the rate of bradykinin degradation and increases the binding of PG.
Long-term use leads to weakening of myocardial hypertrophy, and in addition to this, myocytes of arterial membranes of a resistive nature. The component helps to improve the blood supply to ischemic areas of the myocardium. At the same time, it slows down the progression of left ventricular dysfunction in people who have previously suffered a myocardial infarction.
The peak antihypertensive effect is observed after 4-6 hours and lasts for 12-24 hours.
Pharmacokinetics
Pharmacokinetic characteristics of the drug are determined by the interaction of its elements. Peak values of amlodipine in blood plasma are noted after 3-4 hours after taking the drug and remain at this level for 24 hours. Food intake does not affect the degree of absorption of the substance in the stomach. Stable values of enalapril in plasma are recorded after 7-8 days.
Enadipine is metabolized within the liver, and excretion of the drug occurs primarily through the kidneys.
In elderly people, a decrease in clearance is observed, due to which the plasma half-life of the drug may increase.
Dosing and administration
The drug can be used without regard to food intake, while washing down the tablets with 0.5 glass of plain water.
The dosage of the medicine should be selected individually for each patient, taking into account their condition and the nature of the disease. Adults are usually prescribed 1 tablet of the medicine once a day. Taking into account the individual sensitivity of the patient, a maximum of 2 tablets of the medicine per day is allowed.

Use Enadipine during pregnancy
There is no data regarding the safety of using the drug in pregnant or breastfeeding mothers.
Contraindications
Main contraindications:
- the presence of intolerance to the components of the medication;
- stenosis affecting the aortic orifice;
- hypertrophic form of cardiomyopathy, accompanied by obstruction of the left ventricular outflow tract;
- decreased blood pressure;
- cardiogenic shock;
- Bilateral or unilateral stenosis affecting the arteries inside the kidneys;
- hyperkalemia;
- patients who have recently received a kidney transplant.
Side effects Enadipine
Taking the tablets may cause the following side effects: feeling of heat, hot flashes, headaches, orthostatic hypotension, decreased blood pressure, dizziness and tachycardia. In addition, swelling of the legs, allergy symptoms (itching, rashes, skin hyperemia, urticaria and Quincke's edema), vomiting, dry cough, myositis, arthritis, as well as nausea, arthralgia, Stevens-Johnson syndrome and liver dysfunction may occur. Long-term use leads to the occurrence of paresthesia, a feeling of severe fatigue or weakness, as well as pain in the limbs.
Rarely, an increase in creatinine with urea in the blood plasma is observed. After the end of the drug use, these indicators return to normal values. Such negative symptoms are usually noted in people with kidney problems or when combined with diuretic drugs by people suffering from heart failure.

Overdose
Most often, the following symptoms are observed during intoxication: a sharp decrease in blood pressure (approximately 6 hours after taking the drug), a state of stupor and tachycardia.
To treat disorders, gastric lavage, maintenance of cardiovascular function, control of pulmonary and cardiac function, and monitoring of BCC and diuresis values are prescribed. It is also necessary to keep the patient's limbs elevated and administer dopamine with calcium gluconate intravenously.
Interactions with other drugs
Combination of the drug with potassium diuretics (such as spironolactone and triamterene with amiloride) is prohibited because it may increase plasma potassium levels. With such treatment, plasma potassium levels must be monitored.
The use of the drug together with drugs that block Ca channels, diuretics, hydralazine, and also with β-blockers and prazosin potentiates the antihypertensive effect.
The combined use of Enadipine with NSAIDs may reduce the therapeutic activity of the drug.
Combination with lithium drugs may cause a delay in lithium excretion, resulting in an increased risk of toxic effects.
Co-administration of the drug with warfarin results in a small change in the effect of warfarin on new PTT values.
Storage conditions
Enadipine should be kept in a dry place, out of the reach of small children. Temperature indicators should be within the range of 18-25°C.
Shelf life
Enadipine is allowed to be used within 24 months from the date of release of the therapeutic drug.
Application for children
There is no information regarding the use of Enadipine in pediatrics.

The analogs of the drug are Amapin, Gipril, Ekvator and Bi-Prestarium with Eneas, as well as Bi-Ramag, Rami-azomex and Enap-combi.
