Elivel belongs to a group of drugs called antidepressants.

Indications Elivela

It is used in the following cases:

  • manic-depressive psychosis (in its depressive phase);
  • depression (also childhood), as well as behavioral disorders and emotional disorders of mixed type;
  • various phobias;
  • anorexia of psychogenic type;
  • to control abnormal digestive behavior in the development of bulimia.

Release form

Released in tablets, 10 pieces inside a blister pack. The pack contains 10 such blisters.

Pharmacodynamics

Elivel is a drug that belongs to the tricyclics category. It has antiserotonin, antihistamine and cholinolytic properties. The mechanism of its action is due to the slowing down of the processes of reuptake of such elements as serotonin and dopamine with norepinephrine by neurons.

The drug helps improve mood during depressions of various etiologies, but is most effective in depressions of the endogenous type. The thymolytic effect of the drug is combined with a strong sedative effect, as a result of which it has a particularly powerful effect in states of depression and anxiety, against the background of which sleep disorders and agitation are observed.

Pharmacokinetics

After taking the tablet, the substance is quickly absorbed in the gastrointestinal tract. The synthesis of the drug with plasma protein is 94%.

The half-life of the substance is approximately 20 hours. Metabolism occurs inside the liver, resulting in the formation of an active primary decay product - the substance nortriptyline. The components amitriptyline with nortriptyline pass through the BBB and penetrate into the mother's milk.

Excretion of the substance occurs mainly in the form of decay products – along with urine.

Dosing and administration

The initial dosage of the drug is 50-75 mg per day (this dose is taken in several doses). If necessary, it is allowed to increase the dosage over a period of 5-6 days (gradually), up to 150 mg per day (for outpatients) and 200 mg per day (for inpatients). Most of the dose should be taken before bedtime.

After obtaining a stable medicinal effect, the dosage is reduced to 50-100 mg per day, and then maintenance treatment procedures are continued for at least another 3 months. The medicinal effect begins to appear after 7-10 days of the therapy course.

If there is no improvement in the patient's condition after 3 weeks of treatment, the drug should be discontinued. In this case, the discontinuation should be gradual - the dosage is reduced over a period of 2 weeks.

The duration of the treatment course should not exceed 6-8 months.

Use Elivela during pregnancy

Elivel may only be used during pregnancy in situations where the benefit to the woman outweighs the risk of adverse symptoms to the fetus.

If you need to take the medicine during lactation, you must stop breastfeeding during this period.

Contraindications

Main contraindications:

  • intolerance to the components of the drug;
  • presence of glaucoma;
  • paralytic intestinal obstruction;
  • epileptic seizures;
  • pyloric stenosis;
  • myocardial infarction in the acute phase;
  • combination with MAOI drugs.

Side effects Elivela

Taking the pills may cause the following side effects:

  • mental reactions and manifestations from the nervous system: a feeling of confusion, agitation, disorientation or drowsiness, development of ataxia, dizziness, dysarthria, insomnia, headaches, polyneuropathy. Hallucinations and convulsions also occur, as well as numbness of the limbs and changes in EEG indicators. The process of ADH secretion and the ability to concentrate may be disrupted;
  • dysfunction of the cardiovascular system: development of tachycardia, myocardial infarction, postural hypotension and stroke, increased blood pressure, as well as disturbances in the heart rhythm or cardiac conduction;
  • reactions due to anticholinergic properties of drugs: accommodation disorder, development of mydriasis, blurred vision, increased intraocular pressure. Urinary retention and dilation of the urinary tract also occur. Constipation, dry mouth, hyperrexia and paralytic intestinal obstruction may occur;
  • gastrointestinal disorders: development of stomatitis or anorexia, the appearance of vomiting, diarrhea or nausea, as well as discomfort in the epigastrium and taste bud disorders;
  • others: development of alopecia, galactorrhea or gynecomastia, appearance of skin rash, weakening of potency and libido, weight gain/loss, swelling in the parotid glands and disorders of hematopoietic processes. Functional liver disorder is observed sporadically.

Overdose

As a result of drug intoxication, the following symptoms occur: bradycardia, arrhythmia, decreased blood pressure, AV conduction disorder, and liver function. The development of severe side effects is also a symptom of overdose. Often, to eliminate them, it is enough to reduce the dosage or temporarily cancel the drug.

The treatment method is gastric lavage and taking saline laxatives with activated carbon. Elivel has no antidote. Symptomatic treatment may also be performed. The victim must be kept under regular observation (ECG and blood pressure monitoring). To eliminate bradycardia, it is permissible to use β1-adrenergic agonists or atropine, and also to temporarily install a pacemaker in the patient.

Interactions with other drugs

The suppressive effect of Elivel on the central nervous system is potentiated when combined with the following drugs: anticonvulsants, opiates, central analgesics, hypnotics, neuroleptics, anesthetic drugs and alcoholic beverages.

The combination of anticholinergic agents and Elivel increases the likelihood of developing paralytic intestinal obstruction.

Combined use with cimetidine may increase plasma levels of Elivel.

The drug is capable of blocking the hypotensive effect of guanitidine.

Storage conditions

Elivel should be kept in a place protected from moisture and sunlight, and out of reach of children. Temperature indicators – no more than 25°C.

Shelf life

Elivel is approved for use for a period of 4 years from the date of release of the drug.
