Macmiror in giardiasis: treatment in adults and children

Among a number of microorganisms, the growth of which is effectively suppressed by the active substance of this antibacterial drug (nifuratel), protozoa are also named. Among them are giardia. Infection with these tiny parasites is quite common. Diagnostics are not always informative, and there is no uniform tactic for treating giardiasis. Drugs that can destroy parasites are quite toxic and have a number of side effects, one of which is hepatotoxicity. Persistent immunity to infection does not develop after recovery. In addition, giardia have developed resistance to drugs often used for their eradication, such as metronidazole or furazolidone.
Often the presence of giardia is combined with fungal infections (candidiasis) and the detection of Helicobacter pylori.
Therefore, the emergence of a new drug suitable for eradication of giardia has attracted the attention of doctors. Macmiror (nifuratel) is currently considered the drug of choice for giardiasis. Laboratory and clinical studies confirm its high efficiency in performing this task (exceeds 90%). In addition, in the presence of combined intestinal infections, which often accompany giardiasis, there is no need to additionally prescribe other drugs, since this antibiotic has a wide range of antibacterial activity. Eliminating from the body through the urinary tract, it sanitizes them along the way.
And, perhaps, the main advantage of nifuratel is its low toxicity (all previously known antiparasitic drugs are very toxic), which is manifested by a minimal number of side effects.
Over three decades of its use, not a single case of development of resistance in pathogenic microorganisms has been identified.

Inflammatory diseases of the genitourinary system and digestive tract, the pathogens of which are sensitive to the active substance of the drug:
- high sensitivity is demonstrated by Giardia ( lamblia ), Trichomonas vaginalis, Entamoeba histolytica, Helicobacter and Papiliobacter pylori, Candida fungi, Salmonella (typhi, typhimurium, enteridis), Shigella (flexneri 2a, flexneri 6, sonnei), enterococci, Staphylococcus aureus, Escherichia coli and other enterobacteria;
- Proteus (mirabilis and vulgaris), Prseudomonas aeruginosa are moderately sensitive.
Release form
The medicine is available in solid tablet form, coated with a gastro-solubilizing coated coating. Contains 0.2 g of the active substance - nifuratel.
Pharmacodynamics
Suppression of the development and growth of microorganisms sensitive to it by the active ingredient of the drug occurs through several routes of action.
Firstly, Nifuratel inhibits the process of their cellular respiration: the molecules of the active substance attach to themselves oxygen molecules, creating its deficiency for microorganisms. In addition, Nifuratel inhibits the enzymatic activity of some cellular respiratory enzymes.
Secondly, when it enters the cells of a microorganism, the nitro group (NO2) is restored, and the resulting component turns out to be toxic to the cells of the pathogenic organism.
The combination of these actions leads to the death of aerobic microorganisms, as well as candidal microflora.
Nifuratel, as a representative of nitrofurans, is capable of blocking the process of synthesis of the daughter DNA molecule of pathogenic organisms, and also, to a lesser extent, suppressing the replication of their RNA.
Pharmacokinetics
The drug, getting into the gastrointestinal tract, is distributed in the body tissues with good speed, easily overcomes the blood-brain and placental barriers, is found in breast milk. Splitting into metabolites occurs mainly in the liver and muscle tissues. It is eliminated from the body in full (about half of the drug is excreted unchanged) through the urinary tract, providing a disinfecting effect.
Dosing and administration
Macmiror for giardiasis in adults is dosed at 0.4 g (two tablets) twice or three times a day. The recommended duration of treatment is a week. The medicine should be taken after meals.
Macmiror for giardiasis in children is used from the age of six and is dosed at the rate of 15 mg per kilogram of the child's weight twice a day or 10 mg per kilogram of weight three times a day. The duration and method of administration are similar.
Use macmiror for giardiasis. during pregnancy
The active substance of the drug easily overcomes the hematoplacental barrier and is found in breast milk. Therefore, although its teratogenic properties have not been detected, the prescription of Macmiror to pregnant and lactating women is possible only in case of vital necessity and subject to the transfer of the baby to artificial feeding.
Contraindications
Known intolerance of the patient to any of the ingredients of the drug.
Renal failure, degenerative-dystrophic changes in nerves, G6PD enzyme deficiency.
Children up to six years of age, although some sources do not specify age restrictions.
Side effects macmiror for giardiasis.
The most probable are bitterness in the mouth, diarrhea, nausea, very rarely - a more detailed picture of dyspeptic phenomena. Quite rarely - allergies in the form of rashes and skin itching, neuropathy of the peripheral nerves.

Overdose
There are no known cases of exceeding the recommended dose; hypothetically, there may be an increase in side effects.
Storage conditions
Store for no more than five years in the original packaging, observing temperature conditions up to 25℃. The storage place of the drug should be out of reach of children.
Analogues
Other medications can also be used to eradicate Giardia. Nitroimidazole derivatives, in particular Metronidazole, have long occupied a leading position in the treatment of this disease. Currently, this drug and its synonyms (Trichopolum, Klion, etc.) are not recommended for giardiasis, since the parasites have already lost sensitivity to this drug. Other drugs from this group, for example, Ornidazole, are effective in about 90% of cases. However, the incidence of side effects for Ornidazole is estimated at 15% versus 2% for Macmiror.
The modern antihelminthic drug Nemozol (albendazole) is universal. It acts on all known types of worms at any stage of development (from eggs to mature individuals). The drug is also active against giardia. Nemozol is broken down into metabolites in the liver. Consequently, the frequency and number of side effects of this drug is higher. The answer to the question of which drug to choose - Nemozol or Macmiror for giardiasis depends on many factors. Firstly, on the patient's age. Nemozol is used from the age of six, the use of Macmiror is allowed by some authors from two months. Secondly, possible concomitant infections should be taken into account. If giardia is detected in combination with bacterial infections, then the choice is for Macmiror. If, in addition to infection with giardia, the patient is found to have pinworms or roundworms, then the drug of choice in this case will be Nemozol. The choice of treatment regimen and suitable drugs should be left to a specialist who will take into account the individual specifics of the disease.
If we are talking only about infection with giardia, then Macmiror is currently the most suitable medicine for their destruction both in terms of efficiency and safety for adults and children. Although usually in the schemes of eradication of giardia two courses of different drugs are used, each of which is active against these parasites.
Reviews
Doctors unanimously claim that Macmiror is the most suitable modern effective and safe drug for the treatment of giardiasis (according to studies in Switzerland - 97% of patients were cured). However, it did not help three percent of study participants.
Patients' opinions are, as always, polar, many are very happy, their children got rid of giardia, while other drugs did not help. But many adults complain of side effects not listed in the instructions: severe nausea, vomiting, headache, constipation (not diarrhea), strong heartbeat, vision problems. Although it is unknown whether they took into account the interaction of the drug with alcohol during the treatment.
Macmiror is certainly a good drug for giardiasis, but it may not suit someone. Older drugs have many side effects, they are toxic and change the blood formula, although it is restored after stopping the intake.
