Diclosan contains the element diclofenac, which is included in the subgroup of NSAID substances; it has a strong antirheumatic, anti-inflammatory, antipyretic and analgesic effect. The main principle of its therapeutic effect is the inhibition of the biosynthesis of PG elements.

During inflammation caused by rheumatic diseases or injuries, the medication helps to reduce tissue swelling and pain, as well as reduce the period of renewal of activity of damaged muscles with ligaments, joints and tendons. [ 1 ]

Indications Diclosan

It is used in local therapy of inflammation and pain in the area of muscles, tendons with joints and ligaments, which have a traumatic or rheumatic genesis.

Release form

The drug is released in the form of a gel, inside a tube with a capacity of 40 g. There is 1 such tube inside the pack.

Pharmacodynamics

Clinical data confirm that the drug relieves acute pain within 60 minutes of the first treatment. After 2 days, 94% of those treated showed a response to Diclosan (compared to 8% of those who showed a response to placebo). [ 2 ]

Elimination of functional disorders and pain was observed after 4 days of therapy using the gel. The water-alcohol base of the drug has a cooling and local anesthetic effect. [ 3 ]

Pharmacokinetics

The volumes of diclofenac absorbed through the epidermis are proportional to the size of the treatment area and depend on the total portion of gel used and the intensity of epidermal hydration. With local treatment of an epidermal surface of 500 cm2 using 2.5 g of gel, the absorption rate of the substance was approximately 6%. In the case of applying a sealed dressing for a 10-hour period, the absorption of the drug increased threefold.

When treating the epidermis with gel in the area of knee and wrist joints, diclofenac is noted in the blood plasma (its Cmax value here is approximately 100 times lower than in the case of oral administration), synovium and synovial membrane. Protein synthesis of the drug is 99.7%.

Diclofenac accumulates inside the epidermis, which is a reservoir from which the drug is gradually released into the adjacent tissues. Then the substance mainly passes into the inflamed tissues located deeper (for example, joints), and continues to have an effect there. Here the drug is registered in concentrations that have a level up to 20 times higher than in the blood plasma.

The metabolic processes of diclofenac mainly occur during hydroxylation, when several phenolic derivatives are formed (of which 2 have a medicinal effect, but much weaker than diclofenac).

The active element with its metabolic components is excreted mainly with urine. The systemic intraplasmic clearance of the drug is 263±56 ml per minute, and the final half-life is 1-3 hours (average value).

Dosing and administration

Diclosan should be used 3-4 times a day; the gel is applied by rubbing it lightly into the epidermis. The volume of the substance used is determined by the size of the inflamed area (for example, 2-4 g of gel is enough to treat an area whose size is 400-800 cm2).

After performing the application, you should wash your hands (except in situations where this particular area is being treated).

The duration of the treatment cycle is determined by the therapeutic effectiveness of the gel and the nature of the pathology.

You cannot use the gel for more than 2 weeks in a row.

  • Application for children

The drug should not be prescribed to persons under 14 years of age. In case of using the drug in adolescents over 14 years of age for a period longer than 1 week, or in case of potentiation of the symptoms of the disease, it is necessary to consult a doctor.

Use Diclosan during pregnancy

There is too little clinical data on the use of the drug in pregnant women, which is why the gel is not prescribed during this period, as well as during breastfeeding. An absolute contraindication for use is the 3rd trimester, since Diclosan can provoke weakness of labor activity or premature closure of the arterial tract.

Animal testing has shown no adverse effects on pregnancy, embryonic development, birth process or postnatal development.

In case of strict indications, the medicine can be used during breastfeeding - in situations where the probable benefit is more expected than various risks. The gel cannot be applied to the mammary glands, nor can it be used for a long time.

Contraindications

Main contraindications:

  • severe intolerance associated with diclofenac or other components of the drug;
  • history of cases of asthma, acute rhinitis or urticaria that developed due to the administration of aspirin or other NSAIDs.

Side effects Diclosan

Side effects include:

  • invasive or infectious infections: occasionally a pustular rash appears;
  • immune dysfunction: Quincke's edema or symptoms of intolerance (including urticaria) are occasionally observed;
  • problems with respiratory activity: asthma occasionally occurs;
  • lesions of connective tissues and epidermis: itching, dermatitis (also its contact form), rashes, erythema and eczema are often observed. Burning or photosensitivity occur occasionally. Dermatitis of a bullous nature develops occasionally.

Overdose

The risk of developing intoxication is very low, because diclofenac is extremely poorly absorbed into the systemic circulation in the case of local treatment. When swallowing the gel, general side effects may be noted.

In case of accidental oral administration of the drug, it is necessary to quickly induce vomiting and use an adsorbent. Symptomatic actions are carried out with the implementation of procedures that are prescribed for the treatment of intoxication with NSAID drugs.

Storage conditions

Diclosan must be stored in a place closed to small children. Temperature values should not exceed 25°C.

Shelf life

Diclosan can be used for a period of 24 months from the date of production of the therapeutic substance.

The analogs of the drug are Nimid, Diklobene with Dolgit gel, Butadion and Ketoprofen with F-gel, as well as Diclofenac, Finalgel and Klafen with Revmalin.
