Derinat belongs to the category of immunomodulators and cytokines.
Indications Derinat
It is used in the following cases:
- radiation damage;
- disorder of hematopoiesis processes;
- the presence of myelodepression and resistance to cytostatics (developed as a result of radiation or cytostatic treatment procedures) in people with oncological pathology. Helps to stabilize hematopoiesis, as well as reduce the myelo- and cardiotoxicity of chemotherapy drugs;
- for stomatitis caused by the use of cytostatic treatment;
- erosive gastroduodenitis, as well as ulcers in the duodenum or stomach;
- IHD;
- vascular pathologies of the obliterating type in the legs, as well as chronic ischemic disease of the legs (stages 2-3);
- long-term non-healing trophic ulcers;
- complications of a purulent-septic nature and odontogenic form of sepsis;
- rheumatoid arthritis;
- burn disease;
- in surgery – the periods before or after an operation;
- endometritis or prostatitis;
- mycoplasmosis or ureaplasmosis, as well as chlamydia;
- chronic obstructive pulmonary pathologies;
- pulmonary tuberculosis.
Release form
Released as a solution for injections, in 5 ml vials. There are 5 such vials inside the package.
Pharmacodynamics
The medicine promotes activation of the immune response (humoral and cellular). Immunomodulatory properties are provided by stimulation of the activity of B-lymphocytes, as well as T-helpers.
Derinat promotes the development of non-specific resistance of the body, optimizing the processes of the inflammatory response, and along with it immune manifestations in relation to antigens of viral, bacterial, and also fungal types. Activates the processes of healing and reparation, increases the body's resistance to infections, and in addition regulates the process of hematopoiesis (helps to normalize the number of phagocytes with leukocytes, and along with this platelets with granulocytes).
The pronounced lymphotropicity of the drug helps it to activate the drainage and detoxifying action of the lymph. Derinat significantly reduces cellular sensitivity to the negative impact of radiation therapy procedures, as well as chemotherapeutic drugs. The drug has no carcinogenic or teratogenic properties.
Pharmacokinetics
The active component of Derinat is quickly absorbed and then distributed inside tissues and organs (with the participation of the internal lymphatic transport route). The drug has a strong tropism with respect to the organs of the bloodstream, it actively participates in cellular metabolic processes, integrating into the structures of individual cells. At the stage of intensive entry of the substance into the blood, redistribution between formed blood components and plasma occurs, and metabolic processes and excretion occur in parallel.
The introduction of a single injection affects the indices of all pharmacokinetic parameters that describe the change in the level of the drug inside the studied tissues with organs, as follows - a rapid increase stage, and then a rapid decrease stage of the substance indices in the period of 5-24 hours. The half-life after injection is 72.3 hours.
The drug is quickly distributed within the body, and with daily course use accumulates within organs and tissues. Maximum values are observed within the thymus with the spleen, as well as lymph nodes and bone marrow. Lower values are noted in the brain, liver, large and small intestine, and also the stomach. The peak level of the drug in the bone marrow is observed 5 hours after injection. The active component passes through the BBB. The drug reaches peak values in the brain after half an hour.
The substance undergoes metabolism. Excretion (in the form of decay products) occurs partly with feces, and also, in larger quantities, with urine.
Dosing and administration
The adult dosage is an intramuscular injection of 5 ml (75 mg) of a 1.5% solution over 1-2 minutes. The procedures are carried out at intervals of 24-72 hours.
For the treatment of coronary heart disease, perform 10 injections with intervals of 48-72 hours between them.
To eliminate ulcerative pathologies in the intestines or stomach – 5 injections with an interval of 48 hours.
When eliminating oncological pathologies – 3-10 injection procedures with an interval of 24-72 hours between them.
For the treatment of gynecological (such as endometriosis, myoma, adnexitis) or andrological (such as prostatitis) diseases – perform 10 injection procedures at intervals of 24-48 hours.
Therapy for pulmonary tuberculosis involves 10-15 injections with intervals of 24-48 hours between them.
For chronic inflammation, give 5 intramuscular injections with an interval of 24 hours, and then another 5 injections, but with an interval of 72 hours.
The treatment regimen for children is similar to the adult regimen. Children under 2 years of age should be administered a single dose of 7.5 mg (or 0.5 ml of a 1.5% solution).
Single doses for children aged 2-10 years are calculated in proportion of 0.5 ml for each year of their life. For children over 10 years old, the size of a single dose is 5 ml of a 1.5% solution, and the entire course consists of 5 injections of the drug.

Use Derinat during pregnancy
Derinat should not be prescribed to pregnant or lactating women.
Contraindications
Contraindication is individual hypersensitivity to the components of the drug.
Side effects Derinat
Due to rapid intramuscular injection, moderate pain occurs at the injection site, which does not require special treatment.
There is also evidence that sometimes after intramuscular injections (after 1.5-3 hours) a short-term increase in temperature develops (on average up to 38°C). This disorder can be eliminated by using symptomatic drugs (diphenhydramine or analgin).
People with diabetes mellitus experience a hypoglycemic effect, which requires constant monitoring of glucose levels.
Interactions with other drugs
Combined use with Derinat helps to increase the effectiveness and reduce the duration of therapy in combination with a significant reduction in the doses of antiviral drugs, as well as antibiotics.
The drug potentiates the effect of antitumor antibiotics (from the anthracycline group), as well as cytostatics.
The drug enhances the medicinal effectiveness of basic therapy used to eliminate ulcers in the duodenum and stomach. The drug reduces the iatrogenicity of basic drugs in the process of eliminating the rheumatoid form of arthritis with subsequent improvement (indicators of 50% and 70%) of many complex values of pathological activity.
During surgical sepsis, injection of the solution (as a complex substance) helps to reduce the level of poisoning, activate the immune system, improve the functioning of organs (which help the detoxification processes within the body), and also normalize the hematopoietic process.
Shelf life
Derinat is permitted to be used for a period of 5 years from the date of manufacture of the medicinal solution.
