Cybor has anticoagulant therapeutic activity. The drug is a low molecular weight heparin, which is part of the group of anticoagulants with a direct form of drug effect. When using a medication, there is a decrease in the severity of blood clotting.
The principle of the drug effect of the active element bemiparin Na is associated with the potentiation of the suppressive effect of antithrombin 3, which is in relation to the existing factors of blood clotting. [1]
Indications Cybor
It is used for the following disorders:
- prevention of the development of thromboembolism in the case of general or orthopedic surgery;
- prevention of thromboembolism in persons with an increased risk of thrombus formation (without performing an operation);
- during hemodialysis to prevent blood clotting;
- prevention of the development of relapses of venous thromboembolism in cases where the patient has DVT.
Release form
The release of the drug element is made in the form of a liquid for subcutaneous injections, inside syringes with a volume of 0.2 ml. 10 or 30 syringes inside the box.
Pharmacokinetics
After performing a s / c injection, the drug is well absorbed. Bioavailability values are 96%. The term for achieving the plasma Cmax of anti-Xa-factor effect is determined by the size of the drug dosage. With a portion in the range of 2500-3500 IU, this indicator is noted after 3 hours after using the drug; It should be noted that when using the indicated portions, the anti-2a-factor effect does not develop. For it to appear at around 0.01 IU / ml, higher dosages (7500-12 500 IU) are required.
The half-life of bemiparin is approximately 6 hours (at dosages of 2500-12 500 IU), which is why the drug is administered once a day. There is no information on metabolic processes, protein synthesis and excretion of the drug.[2]
Dosing and administration
The medication is used for orthopedic and surgical procedures, as well as for hemodialysis and for prophylaxis, taking into account the degree of risk intensity.
In the case of a low likelihood of venous thromboembolism, on the day of the orthopedic procedure or surgery, the patient is injected with an SC injection in portions of 2500 IU, 2 hours before the procedure or 6 hours after its completion. Further, during the period of high probability of thromboembolism (7-10 days), 2500 IU is used at 24-hour intervals.
If the likelihood of developing the disorder is high, it is required to increase the dosage to 3500 IU.
To prevent blood clotting during dialysis, when the probability of bleeding is low or absent, the medication is injected into the arterial bed through a bolus injection - once, before the start of hemodialysis. The portion size is determined taking into account the patient's weight: if the weight is below 60 kg, 2500 IU of antifactor-Xa is used; if the weight is above 60 mg, 3500 IU is required.
Cybor should be injected into the posterolateral lumbar zone or the anterolateral abdominal area, on the left and right in turn. It is necessary to insert the needle into the skin fold exclusively perpendicularly. Rubbing the injection area is prohibited.
- Application for children
Forbidden to use in pediatrics.
Use Cybor during pregnancy
It is possible to prescribe Cybor to breastfeeding or pregnant women only in extreme cases, taking into account the possible risks.
Contraindications
The main contraindications:
- increased intolerance to medication, heparin and products obtained from pork organs;
- severe blood clotting disorder and frequent active bleeding;
- a history of suspected or diagnosed thrombocytopenia, immunologically associated with heparin;
- severe hepatic dysfunction;
- trauma or surgery in the area of auditory and visual organs, as well as the brain;
- active form of bacterial endocarditis;
- diseases in which the likelihood of bleeding is increased (peptic ulcer, hemorrhagic stroke and cerebral aneurysm).
Caution is required when using in persons with insufficient kidney function, high blood pressure, urolithiasis, diseases of the retina and iris of the eye, and in addition, if there is a history of gastric ulcer and lumbar puncture is performed.
Side effects Cybor
Most often, the use of drugs causes ecchymosis or pain and hematoma in the area of the injection.
Sometimes bleeding develops in the mucous membranes of the gastrointestinal tract and the urogenital tract, as well as signs of allergies.
Occasionally, anaphylactic symptoms appear (itching, dyspnea, bronchial spasm, fever, swelling of the larynx and urticaria, as well as vomiting and nausea).
Overdose
The main symptom of an overdose is bleeding. At the same time, taking into account the likelihood of thrombosis and the severity of hemorrhage, a decision is made regarding drug withdrawal. If the hemorrhage is weak, specific therapy is not required, and if severe bleeding occurs, it is necessary to introduce protamine sulfate in a portion of 1.4 mg / 100 IU of antifactor-Xa.
Interactions with other drugs
It is forbidden to combine bemiparin with other drugs that have an anticoagulant effect and can reduce the rate of platelet aggregation.
You can not use the medicine together with dextran and systemic GCS, because with such a combination, there is an increase in the likelihood of bleeding.
Cybor should be used with caution in conjunction with drugs that contribute to the onset of hyperkalemia.
The introduction of the drug in combination with intravenous injections of nitroglycerin leads to a weakening of the therapeutic activity of bemiparin Na.
You can not mix the drug with other drugs that are administered parenterally.
Storage conditions
Cybor must be kept at temperatures not exceeding 30 ° C.
Shelf life
Cybor can be used for a 24-month term from the date of sale of the pharmaceutical product.
Analogs
The analogs of the medicine are the medicines Novoparin, Ecosanparin, Kleksan with Flenox, and in addition, Axparin, Eclectic and Heparin with Fraxiparin. Also on the list are Fragmin, Giesend and Enoxarin.
Reviews
Cybor generally receives positive feedback from patients who have used it.
