Cardiket affects the body by relaxing the smooth muscles in the area of the vascular walls, which leads to the expansion of peripheral arteries and veins - causing the development of a vasodilating effect.
As a result, there is a decrease in diastolic pressure values, and in addition, a reduction in the processes of backflow of venous blood into the cardiac ventricle, and at the same time a decrease in the volume of its filling. [ 1 ]
Indications Cardiquette
It is used in the treatment and prevention of angina attacks, coronary heart disease, and myocardial infarction.
In addition, it can be prescribed in case of CHF – in combination with other medications.
Release form
The drug is released in the form of tablets with a prolonged type of activity - 10 pieces in cell packs, 1 or 5 such packs inside a pack.
Pharmacodynamics
Cardiket demonstrates antianginal activity. Contains isosorbide dinitrate (the substance is an organic nitrate). The drug has a vasodilating effect on veins and arteries. At the same time, it is deposited inside the peripheral system of blood vessels, which leads to a decrease in venous cardiac return, a decrease in the final level of diastolic blood pressure, and in addition to a decrease in the volume of left ventricular filling. [ 2 ]
After oral administration, the medication dissolves in the stomach within a few minutes. The active ingredient is released from the tablet over a long period of time. The development of the therapeutic effect occurs quickly, with the effect being maintained over a long period of time. [ 3 ]
Pharmacokinetics
The development of medicinal effect is noted after 15 minutes from the moment of taking the tablet. Bioavailability values are 22%, and protein binding indices are 30%.
The main metabolic elements formed inside the liver are isosorbide-2-mononitrate and isosorbite-5-mononitrate, which are excreted by the kidneys. Complete excretion occurs within 24 hours.
Dosing and administration
The treatment regimen and portion sizes should be selected by the attending physician. The tablet is swallowed whole, without chewing, and washed down with plain water.
The usual daily dose is 60-80 mg of the drug (the dose should be divided into 1-3 doses). In the case of a 20 mg dose, the drug should be taken 1 tablet 2 times a day. Therapy usually begins with a small dose and gradually increases it. It is prohibited to abruptly stop taking the drug.
- Application for children
It is prohibited to use Cardiket in pediatrics.
Use Cardiquette during pregnancy
The drug is used during breastfeeding and pregnancy only in situations where it is considered that the expected benefit is more likely than the risks of negative consequences for the fetus/child.
Contraindications
Among the contraindications:
- drug allergy;
- shock or vascular collapse;
- stenosis;
- marked decrease in blood pressure;
- diseases and conditions in which there is an increase in the level of intracranial pressure or intraocular pressure (glaucoma);
- hemorrhagic stroke and active phase of myocardial infarction;
- head injuries;
- decrease in circulating blood volume;
- complex administration with substances that slow down the action of PDE-5 (including sildenafil with tadalafil and vardenafil).
Caution in use and medical supervision are necessary in the following disorders:
- cardiac tamponade;
- severe anemia;
- toxic pulmonary edema;
- obstructive type of cardiomyopathy (hypertrophic nature);
- pericarditis, which has a constrictive form;
- low blood pressure values of a chronic nature;
- thyrotoxicosis;
- severe stages of liver or kidney pathologies.
Side effects Cardiquette
Main side effects:
- decreased blood pressure, bradycardia, weakness, headaches and angina;
- drowsiness, slow reactions and ischemia;
- belching, stomach pain, xerostomia and nausea;
- redness of the skin on the face, epidermal rash of allergic origin and tolerance to other nitrate substances.
Overdose
Intoxication is characterized by collapse, respiration, reddening of the epidermis, loss of consciousness, nausea, anoxia, diarrhea and coma.
It is necessary to administer 1% methylene blue intravenously at a dose of 2 mg/kg.
Interactions with other drugs
It is necessary to be extremely careful when combining Cardiket with tricyclics, neuroleptics, antihypertensive agents, agents that block the influence of Ca channels, vasodilators, adrenergic blockers, as well as with novocainamide, alcohol, tadalafil and quinidine, vardenafil, sildenafil and dihydroergotamine.
When combined with α-blockers or sympathomimetics, the antianginal effect is weakened.
The therapeutic efficacy of drugs is directly affected by agents that have an enveloping effect on the stomach, since they reduce the rate at which the active component passes from the stomach into the circulatory system.
Atropine, together with other m-anticholinergics, can potentiate the likelihood of an increase in IOP.
The drug can be used together with amiodarone, nifedipine, propranolol, as well as riboxin and panangin.
Storage conditions
The cardiket must be kept in a place out of reach of children and sunlight.
Shelf life
Cardiket can be used for a period of 5 years from the date of manufacture of the drug.

Cardiket receives mostly good reviews – it is often used for disorders of cardiac function or its pathologies.
