Bisoprol is a highly selective drug that blocks the activity of β1-adrenergic receptors. When used in medicinal dosages, it does not have an internal sympathomimetic effect and does not have a strong membrane stabilizing activity. [1]

The drug has antihypertensive and antianginal effects; reduces myocardial oxygen demand by decreasing heart rate and cardiac output. Due to a decrease in end-diastolic pressure and prolongation of diastole, the volume of oxygen entering the myocardium increases. [2]

Indications Bisoprol

It is used for the treatment of coronary artery disease ( angina pectoris ), elevated blood pressure and heart failure with systolic left ventricular dysfunction (in combination with diuretics and ACE inhibitors, as well as, if necessary, with FH).

Release form

The release of the medication is made in the form of tablets with a volume of 2.5, 5 and 10 mg (20, 30 or 50 pieces per pack).

Pharmacodynamics

The maximum therapeutic effect of bisoprolol develops after 3-4 hours from the moment of using the drug. The maximum hypotensive effect develops after 2 weeks of taking Bisoprol. [3]

Dosing and administration

The medication is prescribed for oral administration, the tablet is not chewed, swallowed whole with water. You need to use the medicine in the morning on an empty stomach (or with breakfast). If required, the tablet can be divided into 2 equal portions.

The size of the standard dosage at elevated blood pressure values is 5 mg once a day; the maximum portion size is 20 mg once a day. With a moderate increase in blood pressure, a dosage of 2.5 mg can be used.

Contraindications

The main contraindications:

  • active form of heart failure or decompensated heart failure requiring inotropic treatment;
  • cardiogenic shock;
  • SSSU;
  • 1st and 3rd degree AV block;
  • a pronounced sinoatrial blockade;
  • symptomatic bradycardia;
  • symptomatic decrease in blood pressure indicators;
  • BA in severe stage;
  • late stages of peripheral blood flow disorder or Raynaud's syndrome;
  • psoriasis;
  • untreated pheochromocytoma;
  • metabolic acidosis;
  • severe intolerance to bisoprolol or other elements of the drug.

Side effects Bisoprol

Among the side effects:

  • sleep disorders, fatigue, depression, cephalalgia and dizziness. Hallucinations appear occasionally;
  • occasionally there is conjunctivitis, visual disturbances and weakening of lacrimation;
  • HF decompensation with the appearance of peripheral edema, bradycardia, decreased peripheral blood flow, AV conduction disorder. Orthostatic collapse rarely develops. Sometimes there are paresthesias and a feeling of coldness in the limbs;
  • occasionally dyspnea occurs in persons with a tendency to develop bronchial spasms (for example, in the case of asthmatic bronchitis);
  • constipation, abdominal pain, diarrhea, nausea and hepatitis;
  • it is possible to develop cramps and muscle weakness, as well as damage to joints (mono- or polyarthritis and arthropathy);
  • sometimes itching appears. Occasionally, hyperhidrosis, rashes and reddening of the epidermis are possible;
  • erectile disfunction.

Overdose

In case of poisoning with Bisoprol, you must immediately get medical attention. In case of overdose, bradycardia, third-degree AV block and dizziness may occur.

Often, with intoxication, the following symptoms occur: active form of HF, hypoglycemia, a decrease in blood pressure values, bronchial spasm and bradycardia.

It is necessary to cancel the medication and carry out symptomatic and supportive procedures.

Interactions with other drugs

The drug potentiates the activity of other antihypertensive drugs.

When used with verapamil, reserpine, amiodarone, SG, diltiazem, clonidine, quinidine substances and α-methyldopa, the risk of cardiac conduction, automatism and contractility disorders increases.

Administration together with dihydropyridine calcium antagonists (especially in people with latent heart failure) increases the likelihood of cardiac decompensation and hypotension, which is why when using Bisoprol, it is prohibited to administer Ca antagonists and antiarrhythmic drugs intravenously.

Combination with sympathomimetics, xanthines and rifampicin leads to a decrease in the half-life.

Ergotamine derivatives increase peripheral blood flow disorders.

The drug is incompatible with MAOI substances.

The effect of insulin and oral antidiabetic drugs can be potentiated when administered together with a medication (the likelihood of hypoglycemia increases).

Storage conditions

Bisoprol must be kept out of the reach of small children. Temperature indicators - no more than 25 ° C.

Shelf life

Bisoprol can be used for a 36-month term from the date of manufacture of the medicinal substance.
