Pharmacodynamics

The drug contains the amlodipine (S) isomer of the levorotatory type, which refers to the dihydropyridine derivatives and includes 2 stereomers (but only the amlodipine of the levorotatory character has a medicinal effect). The therapeutic effect of the drug is due to its ability to block Ca channels, which prevents the penetration of calcium ions into the cells of the vessels and muscles of the heart. Due to this inside the smooth muscle cells there is a decrease in the tone of the vascular walls, as a result of which the level of blood pressure decreases (hypotensive effect appears).

In addition, Azomex exerts a strong antianginal effect due to the effect of amlodipine on CCC (peripheral vessels are dilated, thus reducing their resistance). Reduction of peripheral vascular resistance allows to reduce afterload, and in addition load on the musculature of the heart together with its need for oxygen. Exposure to smooth muscle vascular walls leads to a decrease in spasm of the cardiac vessels and stabilization of the coronary blood flow.

The drug does not have a pronounced effect on fat and carbohydrate metabolism, which allows it to be prescribed to people with gout, bronchial asthma, and diabetes mellitus.

Pharmacokinetics

Azomex has a good absorption inside the gastrointestinal tract, and eating does not affect its absorption. The level of bioavailability of the drug is approximately 70-80%. Significantly significant values reach after 10-12 hours. Since the drug has a delayed onset of exposure, it does not lead to a sharp decrease in blood pressure. Stable values of drugs inside the blood are observed after a lapse of 7-8 days.

At biotransformation, inactive products of metabolism are formed.

Excretion occurs with urine, and half-life is about 35-50 hours.

Reviews

Azomex generally receives positive reviews about its medicinal effects. Calcium antagonists are highly effective and have been used for treatment for more than 30 years. This drug has many advantages, because it contains amlodipine S-isomer, which has a higher drug efficiency, which allows to reduce its dosage and reduce the likelihood of developing side-effects.

The active drug S-form has a longer half-life and has a prolonged antihypertensive effect, so one-time use is sufficient to control blood pressure. It is not necessary to adjust the size of medicinal portions when prescribing to people with disorders of renal activity. Long-term use of drugs does not cause tolerance. The reviews show that the drug can be used in monotherapy or in combination with other antianginal drugs (nitrates or beta-blockers) - and in all cases has a positive effect. It is also noted that the drug almost does not cause side effects.
