Pharmacodynamics

The medicinal effect of the drug during the treatment of acne develops through the provision of antimicrobial activity, as well as direct influence on follicular hyperkeratosis.

There is a clinically significant decrease in the density of the colony of propionibacterium acne, as well as a marked reduction in the fraction of fatty acids of free type inside lipids of the epidermis.

Nonanedic acid in vitro, as well as in vivo, slows down the multiplication of keratinocytes, and also stabilizes the destruction of the terminal processes of epidermal differentiation during the formation of acne.

Experimental data demonstrate that nonanedioic acid has a retarding effect on the viability and growth of pathogenic melanocytes (this effect depends on the size of the portion and the duration of therapy). The molecular mechanisms responsible for this process could not be completely determined. The existing information makes it possible to determine that the main effect of nonanedioic acid during the therapy of melasma is due to the slowing down of DNA binding or the slowing down of the cellular respiration of pathogenic melanocytes.

Tests of total tolerability of drugs with repeated external and internal use of nonanedioic acid did not reveal any symptoms of possible development of negative signs (even in the presence of extreme factors - for example, in case of occlusion or treatment of a large area of the body).

Pharmacokinetics

After treatment with cream, nonanedioic acid passes into all skin layers. A higher speed of its passage is noted when it hits the affected epidermis (in comparison with intact areas). With a one-time treatment of the epidermis 1 g nonanedioic acid (corresponding to 5 grams of cream), 3.6% of the used portion is absorbed under the skin.

The part of nonanedioic acid that is absorbed through the epidermis is excreted when urinating in the unmodified state. The rest of the substance decomposes by β-oxidation into dicarboxylic acids having a short chain size (C7, C5); their presence is also recorded inside the urine.

Dosing and administration

Before starting the use of the cream, it is required to wash the epidermis with water or use a cleansing cosmetic preparation. After that, the skin is dried, because the treatment must be carried out on dry epidermis.

To apply a medicine it is necessary 2 times a day (in the morning, and then in the evening), on the affected areas, gently rubbing the cream. About 1 g (corresponds to 4-cm cream), the substance is sufficient to treat all the skin on the face.

It is extremely important to apply the drug regularly throughout the treatment cycle.

The duration of the therapeutic course can vary with regard to the individual course of the pathology and the degree of its severity. People with acne have a noticeable improvement usually after the first month of use. But to get the best effect you need to regularly use Acne-Derm for several months. There are data on the continuous use of nonanedioic acid for a period of up to 12 months.

In case of melasma treatment, the cream should be used for at least 3 months. To obtain a more pronounced effect, the drug should be applied continuously. During therapy with melasma, it is necessary to use sunscreens with a high degree of protection (UFA, UFI) to prevent the development of exacerbations provoked by irradiation with sunlight or repigmentation of the epidermal areas irradiated by the sun.

With too much irritation of the epidermis, it is required to reduce the frequency of using the cream (up to 1 application per day) for a while, until the irritation disappears, or stop the treatment for several days.

Use Acne-Derma during pregnancy

Adequate and appropriate tests regarding the use of nonanadic acid medications in pregnant women for external treatment have not been performed. Therefore, in this period, Acne-Derma should be administered with great caution.

There is no information as to whether the medication is excreted with human milk. One of the experiments in vitro showed that the active element of the drug can penetrate into the mother's milk. The volume of nonanedioic acid, potentially capable of penetrating the baby through the mother's milk, is very small. But nevertheless it is recommended to use very cautiously a cream at thoracal feeding.

Contraindications

It is contraindicated to prescribe the drug in the presence of intolerance relative to its active element or any of the auxiliary components.
