Intralipid 20% is a parenterally administered nutrient. Increases the level of essential and non-essential fatty acids (the constituent components of cell walls required for energy metabolism) and energy reserves.
When used in standard portions does not lead to hemodynamic changes. In the case of the use of drugs in accordance with the prescriptions, clinically noticeable changes in pulmonary activity are noted. In individuals, a transient increase in the activity of intrahepatic enzymes occurs, but this effect is reversible and goes away after the end of parenteral therapy.

It is used as a parenteral diet, with a lack of essential fatty acids.
Release form
The release of the therapeutic substance is realized in the form of an infusion 20% emulsion, inside the vials with a volume of 0.1 and 0.5 l; in a pack - 12 flakonchik.
Pharmacokinetics
The biological parameters of Intralipid 20% are similar to the properties of internal chylomicrons, but unlike them, the drug does not contain cholesterol esters or apolipoproteins, and the indicators of phospholipids inside it are significantly higher.
During the early phase of catabolism, the drug is excreted in the same way as internal chylomicrons. Components are hydrolyzed and covered by peripheral and hepatic endings. The rate of excretion depends on the composition of the fat particles, the intensity of the pathology, the state of the nutritional regime and the speed of the infusion. In volunteers, the indicators of the maximum clearance of drugs when used on an empty stomach were equal to 3.8 ± 1.5 g of triglycerides / kg per day.
Lipid oxidation and excretion are determined by the clinical picture of the disease. Increased metabolism observed after injury or surgery. However, in people with hypertriglyceridemia and renal dysfunction, the metabolic processes of external fat emulsions are rather weak.
Dosing and administration
The drug is administered to an adult through an IV drip in a way at a speed of maximum 0.5 l / h. You can use no more than 3 g / kg of triglycerides per day.
Infants and newborns are administered intravenously via an IV at a maximum speed of 0.17 g / kg per hour. Premature babies should be infused continuously for 24 hours.
The initial dose is 0.5-1 g / kg per day; it can also be increased to 2 g / kg per day. A subsequent increase in dosage to maximum values (4 g / kg per day) is allowed only with regular monitoring of serum indicators of triglycerides and liver samples, as well as with oxygen saturation of hemoglobin.
Use Intralipid 20% during pregnancy
There is no information about the development of negative manifestations when using Intralipid 20% during breastfeeding or pregnancy.
Contraindications
The main contraindications:
- shock in the acute phase;
- gingival lipidemia;
- pancreatitis in the active phase;
- nephrosis of lipoid character;
- hemophagocytic syndrome;
- insufficiency of the liver or kidneys in severe;
- diabetes mellitus having decompensated form;
- hypothyroidism (with hypertriglyceridemia occurring on the background);
- intolerance to soy, egg whites, peanuts or other elements of the drug.
Side effects Intralipid 20%
Adverse events include vomiting, fever, nausea, and chills.
Overdose
In the case of medication elimination disorder, an excessive fat load syndrome may appear, which may cause infection or a disorder of renal activity. With such a syndrome fever, hyperlipidemia, disorders in the work of various organs, fatty infiltration and coma are noted. In severe poisoning with an emulsion, which contains triglycerides, together with the use of carbohydrates, the development of acidosis is possible.
With the abolition of the introduction of Intralipid 20% of all the above symptoms usually disappear.
Interactions with other drugs
The introduction of heparin leads to a temporary increase in plasma lipolysis values, which is why the clearance of triglycerides decreases for some time (due to the depletion of lipoprotein lipase).
The action of lipase may also change under the influence of insulin, although the clinical significance of this effect is rather limited.
K1-vitamin, which is present in the composition of soybean oil, is an antagonist of coumarin derivatives, which is why people who use such drugs need to carefully monitor their blood coagulability.
Storage conditions
Intralipid 20% must be kept in a closed place from small children and sunlight. The temperature level is not more than 25 ° С. Do not freeze the medicine.
Shelf life
Intralipid 20% can be used within a 2-year term since the production of the therapeutic element.
Application for children
Lipid elimination in newborns and infants up to 12 months of age should be monitored regularly. Lipid excretion is assessed by detecting serum triglycerides.
