Indications Seventa

It is used for long-term and continuous treatment of people with a curable obstruction of the respiratory ducts associated with asthma (also in situations where nighttime seizures are present), and in addition with bronchoconstriction caused by physical exertion.

It can also be used in people with obstructive and chronic lung diseases.

The drug is prescribed for people with chronic bronchitis, if there is obstruction in marked form.

Release form

The release of the drug is in the form of a metered aerosol, inside the cans with a special adapter-sprayer, the volume of which is 60 doses. Inside the box - 1 such a spray.

Pharmacodynamics

The drug contains the component salmeterol, a selective β2-adrenoreceptor agonist, which has a prolonged effect and does not affect (when used in therapeutic portions) the work of the CCC.

The drug leads to the development of prolonged bronchodilation in people with treatable obstruction of the respiratory ducts. Its therapeutic effect lasts a maximum of 12 hours. Regular use of salmeterol helps to achieve a steady improvement in pulmonary activity and reduce the severity of developing obstructive syndrome, as well as the nocturnal narrowing of the bronchi in people with asthma.

With constant use of the aerosol in a large number of people with asthma, there is no need to additionally use bronchodilators. But at the same time, it should be borne in mind that Serevent is not suitable for relieving acute bronchial spasms - such people need to constantly keep with them short-lasting bronchodilators.

In vitro tests revealed that salmeterol, in addition to the direct bronchodilator effect, reduced the amount of histamine, leukotrienes and PG from the labrocytes released, which suppressed the early and late stages of the allergic response.

With 1-fold use of aerosol, a decrease in bronchial hyperreactivity was observed, which demonstrates the anti-inflammatory effect of salmeterol. The mechanism of mediated and direct bronchodilatory effects, as well as the suppression of the allergic response and the anti-inflammatory effects of drugs, is different from the effect of corticosteroids. Because of this, against the background of the use of Serevent, it is forbidden to cancel the already used corticosteroids (administered through inhalation or inside).

Pharmacokinetics

After inhalation of 50 μg of the substance 2 times a day, the achievement of serum Cmax values of the active element is observed (up to a maximum of 200 pg / ml). The concentration of the drug inside the serum is quite low, so with the introduction of therapeutic portions should not expect the development of systemic effects. The therapeutic efficacy of drugs is not determined by its performance inside the serum, because the effect of salmeterol is implemented inside the lung tissue.

Dosing and administration

Use of aerosol is permitted only for inhalation procedures. It must be used constantly, because the drug is ineffective in relieving acute spasms of the bronchi.

The physician selects the dosage of medication, treatment regimen, as well as additional therapy. When treating patients with asthma, it is necessary to constantly monitor the general condition of patients, and in addition to control lung activity.

Adults for the treatment of asthma or pulmonary pathologies of a chronic nature, against which obstructive syndrome is noted, often require 2 inhalations of drugs 2 times a day. Persons with obstruction of the respiratory ducts, which have a severe degree of severity, and in addition, if there is no result after using the above portions, the dosage is increased to 4 inhalations 2 times a day. Independently changing the dosage of Serevent, discontinuing therapy or changing portions of concomitant treatment is prohibited.

Children from 4 years old are often prescribed 2 inhalations 2 times a day.

Use Seventa during pregnancy

In clinical tests, the teratogenic effect of the drug was not found, but given the limited amount of existing information regarding testing for this group of patients, it is allowed to prescribe Serevent to pregnant women only with the permission of the doctor, who previously carefully analyzed the existing risks.

Since the inside serum indicators salmeterol extremely insignificant, its level inside the mother's milk is also low. Therefore, the probability of exposure to drugs on infants is also quite small. The decision of whether to use the medicine for breastfeeding should be made by the treating doctor.

Contraindications

It is contraindicated to use the drug in people with hypersensitivity to salmeterol or auxiliary components contained in its composition.

It is required with great care to prescribe an aerosol to persons with hyperthyroidism.

Side effects Seventa

The use of an aerosol can lead to the appearance of separate side-signs typical of most β2-adrenoreceptor agonists - headaches, tremors, dizziness and palpitations. Often these manifestations are treatable, and their severity decreases (or they disappear completely) with continued therapy or reduction of the dosage of the drug.

Individual patients after the introduction of drugs may develop heart rhythm disorders (such as atrial fibrillation, supraventricular tachycardia, and extrasystole). Similar symptoms occur occasionally, the likelihood of their occurrence is higher in people with CVD diseases, and besides, inhalations of too large portions of the aerosol.

As in the case of the use of other inhaled drugs, during therapy with the use of Serevent, the appearance of paradoxical bronchospasms is observed in certain people. In such reactions, it is required to abolish the use of drugs and prescribe the use of bronchodilator drugs with rapid exposure, and in addition, with a doctor's prescription, choose an alternative treatment option.

Occasionally, when using salmeterol, patients experienced pain in the joints, muscle cramps, and signs of intolerance (angioedema, rash, and anaphylaxis).

The use of an aerosol can cause the occurrence of local negative symptoms, including irritation of the mucous membranes in the mouth and pharynx.

Overdose

After inhalation of large portions of salmeterol, patients may experience muscle spasms, headaches, dizziness, tremor, and tachycardia, and in addition, the risk of other negative symptoms characteristic of the active drug element may increase.

In case of poisoning with Serevent, the appointment of cardio-selective β-blockers is required, and in addition, if there is a need, other symptomatic procedures are carried out. It is necessary to take into account that persons with a history of indicating the appearance of bronchospasms should be prescribed β-blockers with extreme caution.

Interactions with other drugs

It is forbidden to combine selective and indiscriminate β-blockers with Serevent.

There may be an increase in the level of salmeterol inside the serum when using drugs along with ketoconazole and the other potent inhibitors of the CYP3A4 component. In addition, the combination of these drugs may increase the risk of the appearance of negative symptoms from the cardiovascular system (among them, an increase in systolic blood pressure, tachycardia and prolongation of the QT interval).

Storage conditions

Serevent must be kept out of reach of children, and also far from sunlight, sources of open fire and heating apparatus. Temperature readings are standard for pharmaceuticals.

Do not pierce, disassemble, heat or deform the medicine cans, even after the drug has run out.

Shelf life

Serevent can be used within 24 months from the moment of release of medicinal substance.

Use in children

Children under 4 years of age are not allowed to prescribe medication (because there have been very few clinical tests for this age group).
