Indications Fexofen
It is used to eliminate allergic rhinitis, which has a seasonal form (tablets with a volume of 0.12 g), and besides this idiopathic form of urticaria, which has a chronic character (0.18 g tablets).
[3]
Pharmacodynamics
Fexofenadine is the blocker of histamine (H1) endings, which has a specific nature of action. Fexofenadine is a pharmacoactive product of the degradation of terfenadine.
When used in therapeutic portions, fexofenadine does not have antidophaminergic, antiadrenergic or anticholinergic effects. Even when used in high dosages, the element does not have a blocking effect on the potassium channels inside the myocardiocytes, and besides it does not have cardiotoxic properties (arrhythmia or prolongation of the QT-interval values).
Fexofenadine does not penetrate the BBB and therefore can not interact with H1-termini within the CNS. The drug has no sedative effect.
Clinical tests performed in adults with an allergic rhinitis of seasonal nature demonstrated that 1 hour after the use of drug dosages of 0.06, 0.12 and 0.18 g, a rapid improvement of the condition was noted, and a therapeutic the impact persisted for the next 24 hours.
Pharmacokinetics
When administered orally, fexofenadine is absorbed at high speed from the digestive tract, reaching plasma Cmax values after 1-3 hours. The average level of Cmax, when consumed 120 mg once a day, was approximately 427 ng / ml. A similar figure for a single dose of 180 mg per day was approximately 494 ng / ml.
The dosage of the drug, which is 0.12 g, has a half-life (equilibrium) of about 14.4 hours. In people aged more than 65 years, these values are similar to the younger group. The half-life of children is 18 hours.
Synthesis of a substance with a plasma protein is about 60-70%. Metabolism is approximately 5% of the portion consumed.
With bile excreted 80% of the dosage of medicament, and in urine - 11%.
Dosing and administration
Tablets are taken without reference to eating, while washing them with plain water. The size of the standard dosage (for adults and adolescents from 12 years) is 0.12 g for the elimination of seasonal allergic rhinitis, as well as for 0.18 g for the removal of symptoms of idiopathic urticaria (chronic). Use the drug once a day (it is recommended that you do this in the morning).
The duration of such therapy is selected for each individual by the attending physician, taking into account the course of the pathology. With a constant intake of drugs during the 28th day of the emergence of tolerance for fexofenadine did not develop.
Use Fexofen during pregnancy
There is too little information on the use of Fexofen in pregnancy. A few tests conducted on animals do not directly or indirectly affect the course of pregnancy, fetal, as well as fetal development of the fetus, the process of childbirth or the postnatal development of the child.
However, it is still forbidden to administer fexofenadine hydrochloride to pregnant women, except for situations where a possible benefit for a woman is more expected than the appearance of complications in the fetus (if there are life indications).
Since the element fexofenadine is excreted with human milk, it is forbidden to use the drug during lactation.

Overdose
There are data on the appearance of a feeling of drowsiness, dizziness, and also dryness of the oral mucosa during poisoning with fexophen. In studies involving volunteers, the use of a single dose to 0.8 g and a double dose of 0.69 g per day for the first month or a dosage of 0.24 g once a day for 12 months did not cause clinically significant negative symptoms in comparison with placebo. It was not possible to establish the maximum allowable size of the transferred portion of fexofenadine.
In intoxication, symptomatic and supportive activities are necessary. The procedure for hemodialysis will not be effective.
Interactions with other drugs
Fexofenadine is not exposed to hepatic metabolism, which is why it does not interact with other drugs using this mechanism.
Combination with ketoconazole or erythromycin results in a 2-3-fold increase in the plasma level of fexofenadine. The effect on the length of the QT-interval with this change is not related. The frequency of development of negative symptoms did not increase in comparison with the use of each of these medications separately.
No interaction with omeprazole is observed.
Use of magnesium and aluminum-containing antacids 15 minutes before the use of Fexofen in a 0.18 g dose weakens the properties of fexofenadine, as well as the degree of its synthesis inside the gastrointestinal tract. It is necessary to adhere to the 2-hour interval between the medications of these medications.
