Indications of the betaderma
It is used during therapy for dermatoses that can be cured with SCS, if complications develop or if there is a suspicion of the presence of a complication with a secondary infection provoked by the activity of bacteria sensitive to gentamicin:
- eczema (coin-like or atopic nature, as well as children's);
- neurodermatitis;
- itching of senile or anogenital origin;
- seborrheic or contact form of dermatitis;
- intertrigo, photodermatitis or exfoliative form of dermatitis;
- dermatitis of a stagnant nature or psoriasis.
Release form
The release is made in the form of a cream, in tubes with a capacity of 15 g.

Pharmacodynamics
Betamethasone dipropionate is an artificial fluoride derivative of hydrocortisone. This element easily seeps through the surface of the skin, possessing a powerful local anti-allergic and anti-inflammatory effect. The drug acts on the causes of inflammation, eliminating them, and in addition prevents the release of histamine and the emergence of local signs of allergy. Local vasoconstrictive properties allow the drug to reduce exudative manifestations.
Gentamicin sulfate is an antibiotic with a wide range of drug activity. It has antibacterial properties in relation to gram-negative microbes (such as E. Coli or Proteus, etc.), and in addition to individual Gram-positive microorganisms (this includes also staphylococci with resistance to penicillin).
Pharmacokinetics
Betamethasone dipropionate is not exposed to skin metabolism. After passing through the skin (maximum 1% absorption), the substance passes through the metabolic processes inside the liver, and then excreted more in the urine, and also in small amounts - with bile.
Increase absorption of this element through the skin surface can be used on overly delicate skin, in areas of the folds, and also on the skin, whose epidermis has been damaged or affected by inflammation. At the same time, absorption is potentiated when the frequency of drug use is too high and when applied to large areas of the skin.
Absorption of betamethasone through the skin in young patients is more pronounced than in adults.
Gentamicin sulfate when applied topically to the intact skin surface is not absorbed. But when used on sites with injuries, sores or burns, it can be systemically absorbed. The element undergoes metabolism, and then excreted unchanged together with the urine.
Dosing and administration
For adolescents from 12 years and adults - you need to treat a thin layer of cream with an inflamed area, twice a day - in the morning, and also in the evening.
The number of procedures for drug application is selected for each person separately, taking into account the severity of the pathology. In mild forms of the disease, the cream can be used once a day, but with more severe forms, it may be necessary to apply more often.
The length of the course is determined by the location and size of the painful lesion, and, in addition, by the person's reaction to therapy. But in the absence of symptoms of improvement after 3-4 weeks of using the cream, it is necessary that the treating doctor review the diagnosis.
Use of the betaderma during pregnancy
Use of betamethasone during pregnancy is prohibited.
There is no information as to whether the active element of Betaderm passes into the mother's milk after the local application of the cream, so it is not recommended to use it during lactation.
Contraindications
The main contraindications:
- the presence of intolerance with respect to gentamicin, GCS or other elements of the drug;
- skin tuberculosis, skin symptoms of syphilis, skin symptoms that develop as a result of vaccination;
- a common form of psoriasis plaque type;
- varicosity;
- perioral dermatitis;
- tumor or viral lesions in the area of the skin;
- simple acne and rosacea;
- chickenpox;
- other skin infections of fungal or bacterial origin that are not treated with adequate antibacterial and antimycotic therapy;
- use in large areas of the skin, in particular, if its integrity has been violated - for example, in the presence of burns.

Side effects of the betaderma
The use of the cream can lead to the appearance of certain side-effects:
- disorders affecting the subcutaneous layer and skin: acne, atrophy of subcutaneous tissues, steroid purpura, erythema, folliculitis, and secondary infection, hypertrichosis and suppression of epidermal growth processes are noted. In addition, there are rashes of a spotty-papular nature, hives, burning, sweating, irritation and itching or exacerbation of already existing symptoms. There is also skin hyper-or depigmentation, its dryness, atrophy or discoloration, and besides this, skin stretch marks (if the drug is used longer than the prescribed term). Treatment of the face can lead to the appearance of dermatitis of a perioral nature;
- disorders of vascular activity: single note (with too long use of the cream) development of telangiectasia;
- impairment of auditory function: alone, when used in large areas of skin (especially damaged) or mucous membranes, as well as a hermetically sealed bandage, it is possible to develop an ototoxic effect under the influence of gentamycin sulfate;
- problems with the endocrine system: single signs of hypercorticism. With long-term use of the substance in large areas of the body, under the hermetically sealed bandage, or children, due to enhanced absorption of betamethasone, a powerful systemic exposure, which is often noted with the use of GCS, is possible. This disorder develops in the form of swelling, weakening of immunity, increase in blood pressure, glucosuria or hyperglycemia, and in addition in the form of suppression of development and growth in children, as well as hypercorticism;
- disorders of visual function: with local use in the skin area on the eyelids, glaucoma development or acceleration of the development processes of an already existing cataract is observed;
- Immune lesions: occasionally intolerance develops with regard to GCS. With such a violation, you should immediately stop using Betaderm;
- symptoms in the area of the urination system and kidneys: occasionally after treatment with cream large areas of the body (especially damaged skin) or applying under the hermetically sealed bandage, the nephrotoxic properties that gentamicin sulfate may have.
The medication contains cetyl stearyl alcohol, which can cause local skin symptoms - such as a contact form of dermatitis.
The propylene glycol contained in the drug can lead to skin irritation.
The substance chlorcresol can lead to the development of signs of allergy.
Overdose
Continuous use of the cream on large areas of the body and in large portions, as well as use together with a hermetically sealed bandage, can contribute to the development of signs of intoxication. It manifests itself in the form of potentiating systemic side-effects, which are often found in the use of GCS (such as glucosuria, hyperglycemia, and in addition hypercorticism) or gentamicin sulfate (nephrotoxic or ototoxic effect).
One-time overdose of gentamicin does not cause development of signs of poisoning. However, prolonged treatment with gentamicin in large portions may cause an increase in the growth of bacteria resistant to antibiotics.
To remove the disorder, adequate symptomatic measures are prescribed. Symptoms of acute form of hypercortisy are often curable. If there is a need, stabilization of the electrolyte balance can be performed. Chronic poisoning effect requires the gradual elimination of drug use. If excessive growth of resistant bacteria is noted, it is recommended that treatment with Betaderm be stopped and the necessary therapy is carried out.
Interactions with other drugs
With external use of GCS, there was no pharmacological interaction with other drugs. But it should be noted that during therapy, smallpox should not be vaccinated, and at the same time perform any other immunization procedures (especially if prolonged use is expected on extensive areas of the skin), because it is likely that the required immunological response in the form of the necessary antibodies will not arise.
Betaderm is able to potentiate the effectiveness of immunosuppressors and weaken the properties of immunostimulants.

Storage conditions
Betaderm is required to be kept in a place that is closed from children's access. Temperature values are a maximum of 25 ° C. Do not freeze the cream.
Shelf life
Betaderm is allowed to be used for 3 years from the date of release of the medicinal cream.
