Pharmacodynamics

The effect of the drug is due to its constituent components-chlorhexidine and metronidazole (they have antibacterial properties).

Metronidazole is a derivative of nitroimidazole (it has antiprotozoal and antibacterial action). It exhibits activity in relatively simple anaerobes, and in addition to periodontal pathogens: Fusobacterium fusiformis, porphyromonas gingivalis, Prevotella intermedia, denticola prevotella, Borrelia vincentii, rectal volinella, treponem, Eikenella corrodens, bacteroides melaninogenicus, and with it Selenomonas sp. The minimum concentration of the substance, which allows to suppress about 50% of the strains, is less than 1 μg / ml for such anaerobic agents of periodontitis: Porphyromonas gingivalis, Prevotella intermedia, vonnelella recta, and Fusobacterium nucleatum.

Chlorhexidine is an antiseptic with bactericidal properties. Affects most gram-positive and gram-negative microbes (vegetative forms), and at the same time on lipophilic viruses, dermatophytes and yeast.

Pharmacokinetics

After local use, the metronidazole concentration index on the gum site exceeds similar indicators after internal drug intake. The indicator of systemic absorption of the substance when using the gel form of the drug will be much lower than the analogous figures after oral administration. Metronidazole and its decay products are mainly excreted through the kidneys (reduced renal function does not affect the pharmacokinetics of a single dose of the substance).

As a result of swallowing excess chlorhexidine left after its local application, about 1% of the dosage of drugs inside the stomach is absorbed from the gastrointestinal tract. Chlorhexidine accumulates in the body, the metabolism of the substance is minimal.

Dosing and administration

Used topically for dental procedures.

For children from 6 years old, as well as adults, when gingivitis is eliminated, it is required to apply a thin layer of drugs on the gums twice a day (for this, you can use a cotton swab or finger). The medicine does not need to be washed off. The duration of the therapeutic course on average is 7-10 days. After completing the procedure for applying the gel for about half an hour it is forbidden to drink and eat.

In the treatment of periodontitis should be treated with the substance periodontal pockets, as well as perform its application on the gums (do this after the procedure to remove deposits on the teeth). The duration of the exposure is 30 minutes. The degree of severity of pathology depends on the number of procedures performed. The subsequent gel applications can be made by the patient himself: it is necessary to apply the medicine to the gums twice a day for 7-10 days.

When treating the aphthous form of stomatitis, treating the inflamed part of the oral mucosa is also required twice a day for the next 7-10 days.

The same number of procedures and duration of the course is required in the prevention against possible exacerbations of chronic forms of periodontitis or gingivitis. For a year, 2-3 such prevention courses are allowed.

In the prevention of postextraction lunar aches, the gel is treated with a well that was formed after the tooth was removed. Then there is an outpatient drug application 2-3 times a day for 7-10 days.

If you want to prescribe Denta gel during breastfeeding, you must stop breastfeeding for the duration of treatment.

Overdose

Intentional or occasional oral gels in high dosage can cause an increase in adverse reactions. As a rule, they are caused by metronidazole, since chlorhexidine is almost not absorbed from the digestive tract. Among the symptoms - nausea along with vomiting, as well as dizziness. In severe overdose - the appearance of seizures and paresthesias.

To eliminate the violations, gastric lavage is required, and in addition, if necessary, therapy aimed at getting rid of the symptoms of an overdose.
