Pharmacodynamics
Betamethasone dipropionate is a hydrocortisone fluoride derivative (synthetic form). It quickly passes into the skin, has powerful local antiallergic, antibacterial, anti-inflammatory, and antipruritic properties. It helps to get rid of the cause of inflammation, and besides it prevents the release of histamine and the development of local manifestations of an allergic reaction. As a result of local vasoconstrictive action, exudation reactions are weakened.
Gentamicin is an aminoglycoside antibiotic with certain bactericidal properties. Is antibiotic with a wide range of medicinal effects. Has an antibacterial effect on gram-negative microorganisms (Escherichia coli, and also proteus), as well as individual gram-positive microbes, including staphylococci, which have resistance to penicillin.
It has no effectiveness in the fight against fungi, anaerobic bacteria, and also viruses.
Pharmacokinetics
Betamethasone dipropionate does not undergo biotransformation in the skin. Absorption in the body from the skin is insignificant - no more than 1%. Metabolism is carried out in the liver, and excretion is mainly along with urine, and also, in smaller sizes - along with bile. Absorption of the active ingredient can be increased when used on a gentle skin, on folds, as well as on the skin, the epidermis of which has been damaged, or on which there is inflammation. In addition, absorption is enhanced by frequent use of drugs, and in addition, as a result of the use of the drug in large areas of the skin. More pronounced absorption of the substance is observed in patients of a young age.
Gentamicin sulfate is not absorbed on the intact skin, but if it has burns, lesions or ulcers, it is possible after systemic absorption of the drug. After passing the metabolic process, the substance is excreted from the body in unchanged form together with the urine.
Dosing and administration
Ointment is applied a thin layer on the inflamed areas, gently rubbing the drug substance into the skin. In areas where there is a tightening of the epidermis, the ointment can be applied more than 2 times a day. Therapeutic course lasts no more than 1 month. Within a year after its completion, a second course (appointed only by a doctor) can be prescribed. It is used to treat subacute conditions, chronic form of dermatosis, and also lichenization.
The cream is used to treat acute forms of pathology, as well as the presence of wetness on the skin.

Use Belogent during pregnancy
During pregnancy, the drug can be used only in exceptional situations, and the appropriateness of its use is determined by the treating doctor. It should be borne in mind that in this case it is possible to use the Belogent for a short period of time, and also to process only small areas of the skin.
It is forbidden to treat the skin next to the mammary glands during lactation.

- The presence of wounds in the area to be treated with the drug;
- skin reactions arising after vaccination;
- the presence of acne vulgaris on the skin;
- children's age less than 1 year;
- high sensitivity to gentamicin, as well as betamethasone and auxiliary elements of the drug;
- presence of rosacea;
- trophic ulcers;
- cutaneous tuberculosis;
- syphilitic manifestations on the skin.
Side effects Belogent
As a result of using Belogent, it is possible to develop such side effects:
- the appearance of stretch marks on the skin;
- development of hypopigmentation or urticaria;
- changes in the skin atrophic type;
- local cutaneous hyperemia;
- the appearance of itching or burning;
- increases the growth of hair on the body;
- a rash that looks like acne;
- development of telangiectasia.
Overdose
With prolonged or excessive (in large dosages) use in large areas of the skin, as well as with the use of a hermetically sealed dressing - it is possible the appearance of overdose. In this case, there is an increase in systemic adverse reactions that are characteristic of GCS preparations (such as glycosuria, hyperglycemia, as well as hypercorticism syndrome), or for gentamicin (nephrotoxic as well as ototoxic effect, which can be especially dangerous if the patient has renal failure ).
In the case of a one-time excess of gentamicin dosage, signs of an overdose did not appear. But in case of prolonged use in excessive doses, the growth of antibiotic-resistant bacteria can significantly increase.
To eliminate the violations necessary treatment aimed at getting rid of pathological symptoms. The acute form of hypercorticism, as a rule, is reversible. If necessary, the electrolyte balance is corrected. If the toxic effect has a chronic form, the use of GCS should be gradually phased out. If excessive growth of bacteria resistant to drugs has begun, Belogent should be abolished and a treatment suitable for the patient should be prescribed.
Interactions with other drugs
Due to external use of GCS, drug interactions were not manifested. But it should be noted that during the therapy it is not recommended to vaccinate against smallpox, and besides to perform other types of immunization (in particular, if long-term treatment of extensive areas of the skin is carried out), because the body can be given an inadequate immunological reaction, which manifests itself under the guise of producing corresponding antibodies.
The drug can increase the effectiveness of immunosuppressants, but the effect of immunostimulants, on the contrary, is weakened.
For combined use with other skin remedies, the drug is not recommended, since there is a risk of weakening its effect.
Gentamicin can interact with heparin, β-lactams (eg, cephalosporins), sulfadiazine, and amphotericin B.
