Drug Gasek refers to the gastrointestinal antisecretory drugs of the pharmacological group of proton pump inhibitors that reduce the release of hydrochloric acid in the stomach. 

Indications Gasek

Gasek (Gasek-20 and Gasek-40) is intended for the complex treatment of hyperacid diseases: peptic ulcer of stomach and duodenum, gastroesophageal reflux, ulcerogenic pancreatic adenoma (Zollinger-Ellison syndrome).

Release form

Gelatin capsules with acid-fast microgranules (in polyethylene bottles packed in a cardboard box). Each bottle contains 14 capsules of 20 mg (Gasec-20) or 40 mg (Gasec-40).

The names of synonyms for the preparation are Gasec: Omeprazole, Omega, Omefez, Omekaps, Omipis, Omitox, Omal, Ocid, Ortanol, Gastrozole, Losek, Pleom-20, Promez, Helol, Cisagast, Ultop.

Pharmacodynamics

Due to weakly alkaline properties, the active substance of the preparation Gasek omeprazole covalently binds to the H + / K + -ATPase-hydrolase enzyme potassium-hydrogen adenosine triphosphatase (proton pump), which catalyzes the synthesis of hydrochloric acid. The inhibition of the activity of this enzyme occurs, as a result of parietal cells of the gastric mucosa reduce the production of HCl, reducing the acidity of gastric juice (increasing its pH). This contributes to the cessation of pain and heartburn with hyperacid gastrointestinal diseases.

Pharmacokinetics

After taking Gassek inside, the active substance (omeprazole) is absorbed in the stomach and small intestine; its bioavailability is 40%. Binding to plasma proteins is almost 97%; the highest plasma concentration is achieved 4 hours after the application of the drug.

Biotransformation of the active substance occurs in the liver; metabolites are excreted in urine and feces - with a half-life of omeprazole about one hour.

Dosing and administration

Gasek capsules are taken orally (both before and after meals), with water. The daily dose is one capsule. The standard course of admission is one to two months. However, if the effectiveness of the drug is not evident within the first week, use of Gaskets should be discontinued.

Use Gasek during pregnancy

Although, as a result of clinical studies of the use of omeprazole in pregnant women, no adverse effects on the fetus have been identified, Hasek capsules are not prescribed to reduce heartburn during pregnancy.

Contraindications

Among the contra-indications of the preparation Gasek are noted:

  • individual intolerance to the drug;
  • diabetes mellitus (since there are glucose in the capsules);
  • malignant diseases of the gastrointestinal tract;
  • children under 12 years old.

Side effects Gasek

Possible side effects of Gasek include:

  • nausea, vomiting, dry mouth, disruption of the intestine, increased gassing, abdominal pain, jaundice;
  • increased temperature, bronchospasm, allergic bullous rashes on mucous membranes and skin (Stevens-Johnson syndrome);
  • a decrease in the level of leukocytes, platelets and agranulocytes in the blood;
  • reduction of Na and Mg content in blood plasma;
  • pain in muscles and joints;
  • headaches, dizziness, blurred vision, insomnia, paresthesia, a state of unreasonable anxiety and depression.

Overdose

When the Gasek dosage is exceeded, there is an increase in its side effects, which requires immediate discontinuation of the drug.

Interactions with other drugs

Gasek is not used concomitantly with antiviral drugs for HIV treatment (reytase, viracept, etc.). Do not simultaneously take Gask and some antitumor drugs (tarlenib, etc.), indirect anticoagulants (fenindion, warfarin, syncumar), as well as antifungal drugs group imidazole and triazole.

Because of the risk of exceeding the concentration of alkaloids in digitalis in blood plasma, simultaneous application of Hasek and antiarrhythmic drugs of cardiac glycosides group (digoxin, lanoxin, novigal) is unacceptable.
