Zulbex is an antisecretory drug that does not have significant antihistamine and cholinolytic effects; it has antiulcer activity. Contains the component rabeprazole.
The drug is capable of inhibiting secretion processes inside the stomach, slowing down the activity of the enzyme proton pump H + /K + -ATPase. As a result of the processes occurring, inhibition of stimulated and basal intragastric secretion develops. The intensity of the drug's effect depends on the size of the portion taken.
Indications Zulbeksa
It is used for the following disorders:
- an ulcer that is in the acute stage;
- GERD;
- destruction of Helicobacter pylori;
- esophagitis having an erosive form;
- prevention of the development of seasonally aggravated ulcers;
- gastrinoma.
Release form
The drug is released in enteric-coated tablets, the volume of which is 10 or 20 mg.
Pharmacodynamics
The component rabeprazole penetrates into the parietal cells, accumulates inside them, and then enters the excretory canals, where it is protonated with the formation of a sulfenamide form. It is in this form that the drug is combined with the proton pump cysteine, causing an irreversible slowdown lasting 40 hours.
Antisecretory activity develops after 1 hour, reaching peak values after 3-4 hours.
The inhibitory effect after repeated use of the drug increases slightly, and after 3 days it reaches an equilibrium value. After stopping taking Zulbex, gastric secretion is restored after 3 days.
The bactericidal effect against Helicobacter pylori was revealed in vitro tests. Combination with antibiotics allows to obtain high eradication rates for H.pylori (80%) and increase the rate of mucosal regeneration.
Pharmacokinetics
The tablet coating is resistant to the acidic gastric environment, dissolving in the intestine, where absorption occurs. Cmax values are recorded after 4 hours, and their value is determined by the size of the dose taken. Bioavailability values are 52%, protein synthesis is 97%.
The half-life is 1.5 hours.
Dosing and administration
The tablets should be taken orally, whole, without breaking or chewing.
In case of development of the active phase of the ulcer, it is necessary to use 20 mg 1-time, in the morning before breakfast. The whole cycle lasts approximately 1 month. If necessary, it can be extended.
For GERD accompanied by erosive reflux esophagitis – 1-time administration of 20 mg of the drug for 1-2 months.
In case of GERD, against which erosions do not occur, it is necessary to take 10 mg of the substance once a day during the first month. Therapy can be extended if necessary.
During gastrinoma, 60 mg of Zulbex should be used first once a day, then increasing the dosage to 0.12 g. Portions less than 0.1 g are consumed in 1 dose, and a dosage of 0.12 g should be divided into 2 doses. Therapy is carried out until the clinical signs of pathology disappear.
To destroy Helicobacter pylori, various combinations of medications are used.

Use Zulbeksa during pregnancy
It is prohibited to use Zulbex during pregnancy.
Contraindications
Main contraindications:
- severe intolerance associated with the drug;
- lactation.
Side effects Zulbeksa
Side effects include:
- insomnia, headaches, severe nervousness, depression, dizziness and confusion (single cases);
- leuko-, thrombocyto- or neutropenia, as well as leukocytosis;
- weight gain or anorexia;
- visual impairment;
- peripheral edema;
- runny nose or cough;
- nausea, pain in the abdominal area, dry mouth, diarrhea, belching and vomiting;
- itching, erythema multiforme or rash;
- myalgia, severe back pain, cramps and arthralgia;
- gynecomastia (single).
Overdose
After a single daily dose of 0.16 g of the drug, symptoms listed among the side effects may develop. They disappear on their own after stopping the drug.
There is no antidote. Symptomatic measures are taken if necessary.
Interactions with other drugs
The drug causes prolonged inhibition of gastric secretion, which may lead to interactions with substances whose absorption depends on pH values.
The levels of rabeprazole and its metabolic component clarithromycin are increased after combination with Zulbex.
All substances that inhibit proton pump activity are prohibited to be combined with atazanavir.
The drug does not interact with drugs whose metabolic processes involve the hemoprotein CYP450 (including warfarin, diazepam with phenytoin, theophylline and amoxicillin).
The combination of rabeprazole with itraconazole, ketoconazole and digoxin causes a decrease in their blood counts, which requires a correction of their dosage.
Storage conditions
Zulbex must be stored in a place protected from moisture penetration; temperature level – no more than 30°C.
Shelf life
Zulbex can be used within a 2-year period from the date of sale of the drug.
Application for children
The safety of the drug in pediatrics has not been studied, which is why it is not prescribed to this category of people.
Analogues
Analogues of the drug are Zolispan, Vero-Rabeprazole, Noflux, Rabeprazole with Pariet, Ontime, Rabeprazole-OBL and Hayrabezol.
Reviews
Zulbex receives good reviews from most patients. The drug is part of the rabeprazole group, the latest generation of inhibitors, which act significantly more effectively than lansoprazoles and omeprazoles. Rabeprazole is enough to take 1 time per day, which is very convenient for patients, which they note in their comments, and which is emphasized by gastroenterologists.
Despite the fact that many people used the medicine for more than 1 month, the development of strong side effects was not observed. Among the most common manifestations are constipation, flatulence and headaches.
