Zimaks is a systemic antimicrobial drug from the macrolide category. The active component of the drug is azithromycin.
Indications Zimaxa
It is used for infectious diseases in the following areas of the body:
- upper respiratory tract, as well as ENT organs: pharyngitis with sinusitis, as well as tonsillitis and tonsillitis with inflammation of the middle ear;
- lower respiratory tract: acute bronchitis (or exacerbated chronic form of the disease), alveolitis, and interstitial pneumonia;
- subcutaneous layer with the skin: tick-borne borreliosis (at the initial stage of development), impetigo, erysipelas, and also dermatitis, which has a secondary form of infection;
- STDs: cervicitis, as well as uncomplicated urethritis;
- Duodenum and stomach: diseases caused by the bacterium Helicobacter pylori (as one of the elements of combination treatment).

Pharmacodynamics
The component azithromycin is in the category of azalide drugs, which are macrolide antibiotics. The substance undergoes synthesis with the ribosomal (70S) subunit 50S of individual bacteria with sensitivity, suppressing the process of protein binding caused by RNA. At the same time, it inhibits microbial growth with reproduction, and at high concentrations can cause a bactericidal effect.
Azithromycin has a wide range of antimicrobial activity. Among the bacteria susceptible to its action are:
- Gram-positive cocci: Streptococcus pyogenes, as well as pneumococcus together with Streptococcus agalactiae and Streptococcus viridans. This also includes Streptococcus aureus with Streptococcus types C and F, as well as G;
- Gram-negative microbes: Pfeiffer's bacillus, Haemophillus parainfluenzae, Moraxella catarrhalis, Campylobacter jeuni, Bordetella-Gengou bacteria, Legionella pneumophila, Bordetella parapertissis, Ducrey's bacillus, gonococcus together with Gardnerella vaginalis;
- individual anaerobes: these include Clostridium perfringens with Bacteroides bivius and peptostreptococci, as well as Mycoplasma pneumoniae with Ureaplasma urealyticum, Chlamydia trachomatis and spirochetes such as Treponema pallidum and Borrelia burgdorferi.
The substance does not have the ability to affect gram-positive bacteria that are resistant to erythromycin.
Pharmacokinetics
Azithromycin is rapidly absorbed from the gastrointestinal tract – this property is due to its lipophilicity, as well as stability when in an acidic environment. The bioavailability index is approximately 37% (affected by the “first liver passage”). The substance reaches its peak serum level, which is estimated at 0.4 mg/l, 2.5-3 hours after oral administration of 0.5 g of the drug.
The drug penetrates into organs and tissue structures of the genitourinary organs (including the prostate), respiratory tract, soft tissues, and in addition to this, the skin. Inside cells with tissues, its values are higher than inside blood serum (10-100 times). High values of the drug inside tissues, as well as a long time of its half-life are due to a low rate of synthesis of azithromycin with proteins inside the plasma, and in addition to this, its ability to penetrate into eukaryotic cells and concentrate inside the environment surrounding the lysosome, which has a low pH level. This factor determines a high level of conditional distribution volume (31.1 l / kg), and in addition to this, a high clearance rate inside the plasma.
It has been proven that phagocytes transport the drug to the sites of the infectious process, where they release it. Full and rapid passage of the active component into the cells, as well as its accumulation inside the phagocytes, with the help of which it moves to the area of inflammatory foci, contributes to the high antimicrobial effect of the drug. Although the drug has a high concentration inside the phagocytes, it does not have a significant effect on their functional capabilities. Zimax maintains bactericidal values in inflammatory foci for 5-7 days after using the last portion. This factor made it possible to develop short-term therapeutic courses - 3-5 days.
The excretion of the drug from the serum is carried out in 2 phases: the half-life of the component is 14-20 hours in the period of 8-24 hours after oral administration of the capsule, and 41 hours in the period of 24-72 hours. This factor allows the drug to be used once a day.
Dosing and administration
Zimaks should be taken once a day before a meal (1 hour) or 2 hours after it. The course lasts within 3-5 days.
During the treatment of diseases in the lower and upper respiratory system, as well as soft tissues with skin, take 0.5 g of the drug on the 1st day, and then 250 mg in the period of 2-5 days (or daily 0.5 g in one dose per day for 3 days). The total dose taken per course is 1.5 g.
To eliminate infections in the genitourinary system (acute type), a single dose of 1 g of the drug is required.
When eliminating the initial stage of Lyme disease, it is necessary to take 1 g of Zimax on the 1st day, and then in the period of 2-5 days - 0.5 g (the total dosage per course is 3 g).
For pathologies in the duodenum or stomach (caused by Helicobacter pylori), you need to take 1 g of the medicine per day for 3 days (as part of a complex treatment).
If a dose of the medicine is missed, it is necessary to take this capsule as soon as possible, and the following ones after it - at an interval of 24 hours.
Use Zimaxa during pregnancy
Pregnant women are prohibited from taking Zimax capsules. If it is necessary to take it during lactation, breastfeeding should be discontinued for the duration of treatment.
Contraindications
Main contraindications:
- the presence of intolerance to the active ingredient and other components of the drug, as well as macrolides;
- severe liver dysfunction;
- children under 12 years of age.
Side effects Zimaxa
The use of the drug can cause the development of various side effects:
- reactions of the gastrointestinal tract: nausea, abdominal pain, flatulence or vomiting, as well as diarrhea;
- manifestations from the hepatobiliary system: melena, transient increase in the activity of liver enzymes and intrahepatic cholestasis;
- increased heart rate, pain in the sternum, a feeling of drowsiness or weakness, as well as vaginitis with nephritis and pseudomembranous colitis;
- photosensitivity and candidiasis;
- a feeling of excitement or a state of insomnia;
- neutrophilia or neutropenia, as well as eosinophilia.
Skin reactions such as rashes appear sporadically.
Overdose
An overdose causes the victim to experience vomiting, severe nausea, diarrhea and hearing loss.
To remove the signs of disorders, it is necessary to wash the stomach and give the patient enterosorbents, and then perform symptomatic treatment. The drug has no antidote.
Interactions with other drugs
Antacids slow down the rate of absorption of azithromycin, which is why these medications can only be taken with an interval of at least 2 hours.
Zimax inhibits the excretion of substances such as terfenadine, digoxin and theophylline with carbamazepine, as well as oral anticoagulants, phenytoin, cyclosporine with warfarin and ergotamine from the body.
The drug potentiates the properties of dihydroergotamine, and along with it, ergot alkaloids.
The effectiveness of Zimax is reduced by the action of lincosamides.
Storage conditions
Zimaks should be kept in a place inaccessible to small children. The room temperature should be a maximum of 25°C.

Zimaks is a very effective remedy for various infectious diseases. Reviews indicate that it is often used for colds, in the treatment of which the drug demonstrates high efficiency and speed of action. Another advantage of the drug is the need to take it once a day, which reduces the likelihood of missing a dose. In addition, the short duration of the drug course is also positively noted.
The disadvantage of the drug was its rather high price, although it is believed that the high efficiency of Zimax justifies its cost. Some patients also noted the occurrence of side effects among the negative aspects, but such cases were quite rare.
Shelf life
