Zimaks is a systemic antimicrobial drug from the macrolide category. The active component of drugs is azithromycin.

Indications of the zimaxa

It is used for infectious diseases in the following areas of the body:

  • upper respiratory system, as well as ENT organs: pharyngitis with sinusitis, as well as tonsillitis and tonsillitis with inflammation of the middle ear;
  • the lower part of the respiratory system: acute bronchitis (or exacerbated chronic form of the disease), alveolitis, and interstitial pneumonia;
  • subcutaneous layer with skin: tick borreliosis (at the initial stage of development), impetigo, erysipelas, and in addition dermatitis having a secondary form of infection;
  • STDs: cervicitis, and in addition uncomplicated form of urethritis;
  • 12-intestine and stomach: diseases caused by the bacterium Helicobacter pylori (as one of the elements of combined treatment).

Pharmacodynamics

Component azithromycin is in the category of drug-azalides, which are macrolide antibiotics. The substance undergoes synthesis with the ribosomal (70S) subunit of 50S that have the sensitivity of individual bacteria, suppressing the protein binding process caused by RNA. Together with this, it inhibits microbial growth with reproduction, and at high concentrations can cause bactericidal effects.

Azithromycin has a large range of antimicrobial activity. Among bacteria susceptible to its effects:

  • cocci of Gram-positive type: streptococcus pyogenic, as well as pneumococcus along with streptococcus agalactia and Streptococcus viridans. These include golden streptococci with streptococci of types C and F, and also G;
  • germs of Gram-negative type: Pfeiffer's wand, Haemophillus parainfluenzae, moraxella cataralis, Campylobacter effini, bacterium Borde-Gangu, Legionella pneumophilus, Bordetella parapertissis, Ducrea's stick, Gonococcus along with Gardnerella Vaginalis;
  • separate anaerobes: this includes clostridium perfringens with Bacteroides bivius and peptostreptococci, and in addition mycoplasma pneumonia with ureaplasma urealism, Chlamydia trachomatis and spirochetes such as pale treponema and along with it Borrelia Burgdorfer.

The substance does not have the ability to affect Gram-positive bacteria that have resistance to erythromycin.

Pharmacokinetics

Azithromycin undergoes rapid absorption from the gastrointestinal tract - this property is due to its lipophilicity, as well as its resistance to being in an acidic environment. The bioavailability index is approximately 37% (it is affected by the "1st hepatic transit"). Peak serum level, which is estimated at 0.4 mg / l, the substance reaches 2.5-3 hours after oral intake of 0.5 g LS.

The medicine passes inside the organs and tissue structures of the genitourinary organs (among them also the prostate), respiratory ducts, soft tissues, and in addition to this skin. Inside cells with tissues, its values are higher than inside the blood serum (10-100 times). High values of drugs inside the tissues, as well as a long half-life of it, are caused by a low level of synthesis of azithromycin with proteins inside the plasma, and in addition its ability to pass inside the eukaryote cells and concentrate inside the surrounding lysosome of a medium having a low pH. This factor causes a high level of the conditional distribution volume (31.1 l / kg), and in addition a high level of clearance within the plasma.

It was proved that the phagocytes transport the medicine to the sites of the location of the infectious process, in which it is released. Full and rapid passage of the active component into the cells, as well as its cumulation inside the phagocytes, by means of which it moves to the area of inflammatory foci, promotes high antimicrobial effects of drugs. Although the drug has a high concentration inside the phagocyte, it does not significantly affect their functional abilities. Bactericidal values within the inflammatory foci Zimaks retains in the period of 5-7 days after the use of the last portion. This factor allowed to develop short-term therapeutic courses - 3-5 days.

Excretion of the drug from the serum is carried out in 2 phases: the half-life to which the component is exposed is 14-20 hours in the period of 8-24 hours after oral administration of the capsule, and 41 hours in the period of 24-72 hours. This factor allows you to use drugs once a day.

Dosing and administration

Zimaks should be taken once a day before meals (for 1 hour) or after 2 hours after it. The course lasts for 3-5 days.

During the treatment of diseases in the lower and upper part of the respiratory system, and in addition soft tissues with skin take 0.5 g of medicine on the first day, and then 250 mg in the period of 2-5 days (or daily at 0, 5 g in one dose per day for 3 days). The total dose taken for the course is 1.5 g.

To eliminate infections in the genitourinary system (acute type), one-time drug use is required in the amount of 1 g.

If the initial stage of Lim's disease is eliminated, 1 gram of Zymax should be taken on the 1st day, and then in the 2-5th days, 0.5 g (the total dosage per course is 3 g).

If pathologies in the area of the duodenum intestines or stomach (caused Helicobacter pylori), you need to drink 1 grams of medication per day in the period of 3 days (as a means of complex treatment).

If the dose was missed, it is required to use this capsule as quickly as possible, and the subsequent capsules should be used at intervals of 24 hours.

Use of the zimaxa during pregnancy

Pregnant is prohibited from taking Zimax capsules. If you need to take during lactation, you must cancel breastfeeding for the period of treatment.

Contraindications

The main contraindications:

  • the presence of intolerance relative to the active component and other components of the drug, as well as macrolides;
  • disorders in the liver function in severe degree;
  • children age not exceeding 12 years.

Side effects of the zimaxa

The use of the drug can cause the development of various side effects:

  • reactions of the digestive tract: nausea, abdominal pain, the appearance of flatulence or vomiting, as well as diarrhea;
  • manifestations from the hepatobiliary system: melena, transient increase in hepatic enzyme activity and intrahepatic cholestasis;
  • faster heart rate, pain in the sternum, a feeling of drowsiness or weakness, and in addition vaginitis with nephritis and pseudomembranous form of colitis;
  • photosensitivity and candidiasis;
  • a feeling of excitement or a state of insomnia;
  • neutrophilia or neutropenia, as well as eosinophilia.

Such skin reactions, as a rash, appear sporadically.

Overdose

Due to an overdose, the victim develops vomiting, severe nausea, as well as diarrhea and hearing loss.

To remove signs of disorders, you need to rinse the stomach and give the patient enterosorbents, and then perform symptomatic treatment. The drug has no antidote.

Interactions with other drugs

Antacids inhibit the rate of absorption of azithromycin, which makes it possible to take these medications solely with a gap of at least 2 hours.

Zimaks inhibits excretion from the body of substances such as terfenadine, digoxin and theophylline with carbamazepine, as well as oral anticoagulants, phenytoin, cyclosporine with warfarin and ergotamine.

The drug potentiates the properties of dihydroergotamine, and with it the alkaloids of ergot.

The efficacy of Zymax is reduced by the action of lincosamides.
