Tenochek is a combination medicine with antihypertensive properties.
Indications Tenochek
It is used to treat people with stable angina or elevated blood pressure.
Release form
The release of the substance is implemented in tablets, packed in blister plates of 14 pieces. The box contains 2 such records.
Pharmacodynamics
Active elements of the drug are atenolol with amlodipine. They mutually potentiate the medicinal effect and have complementarity.
Amlodipine has a potent hypotensive and antianginal effect, weakening the tone of the smooth muscles of the vascular layer, and thereby reducing the total peripheral vascular resistance, and in addition reduces the values of blood pressure. In addition, amlodipine lowers the need, experienced by the heart muscle, in obtaining oxygen and stabilizes the energy balance within myocardial cells. At the same time, the substance helps to slightly expand the coronary vessels and improve blood circulation inside the ischemic and healthy areas.
Atenolol is a β1-adrenoreceptor blocker; at the same time, it does not have a membrane stabilizing, as well as an internal sympathomimetic effect. The substance has a pronounced hypotensive effect, and at the same time it has antianginal and antiarrhythmic properties, which develop as a result of a decrease in heart rate parameters. Atenolol weakens the need of the heart muscle in obtaining oxygen, reduces the number of angina attacks and helps to improve the state of the myocardium during hypoxia.
Dosing and administration
Tenochek is used for ingestion. It is forbidden to grind tablets, but if there is a need, they are allowed to be divided along the line. In order to get the maximum medical effect, the medicine should be taken before meals, at the same time of the day. The abolition of the drug is carried out gradually, under the supervision of the attending doctor. With the abrupt end of the use of drugs may develop withdrawal.
Persons with elevated blood pressure or angina are often prescribed 1-well pill per day.
People with kidney problems and CC values above 10 ml / minute are often prescribed at 50% of the usual dose of drugs.
Those patients who have a QC level of less than 10 ml / minute for disorders of renal function are recommended to take 25% of the usual dosage of the drug.
If there is a need, a portion of Tenochek can be increased by the attending doctor. During the first few weeks of therapy after increasing the dosage, it is necessary to monitor the condition of the patient.
Use Tenochek during pregnancy
You can not use Tenochek for lactation or pregnancy.
Contraindications
Main contraindications:
- use in people with intolerance in relation to the constituent elements of the drug;

- heart failure in acute form;
- sinoatrial blockade;
- bradycardia;

- diabetes;
- metabolic form of acidosis;
- chronic obstructive pulmonary pathology or BA;
- psoriasis;
- disorders in the kidneys or liver, with severe severity;
- cardiomegaly without heart failure symptoms;
- spontaneous angina.
Caution is required when used in such cases:
- AV blockade of the I-th degree;
- CHF at the stage of compensation;
- aortic stenosis;
- pheochromocytoma;
- hyperthyroidism;
- state of depression (presence in history or at the moment);
- myasthenia;
- patients from 65 years.
People who use contact lenses need to take into account that when using Tenochek, the production of tear fluid can be reduced.
Side effects Tenochek
The drug is fairly well tolerated. During treatment in individual patients, the following side effects were noted:
- disorders in the cardiovascular system: problems with the rhythm of the heartbeat, a decrease in blood pressure values, a disorder of AV conduction processes, the appearance of signs of heart failure. It is rare for people with angina pectoris to worsen;
- problems in the PNS or CNS: feeling very tired, feeling cold with tremor in the limbs, depression, dizziness, strange dreams, daytime disorder, cramps, and in addition, ear noise, hallucinations, headaches, polyneuropathy and a feeling of weakness;
- disorders of the digestive tract: vomiting, dryness of the oral mucosa, taste receptor function disorder, nausea and stool disorder. In addition, pancreatitis or hepatitis is also noted, or the bilirubin and liver enzymes increase;
- lesions affecting the respiratory system: dyspnea or bronchospasm;
- endocrine function disorders: gynecomastia, erection disorders and impaired glucose tolerance;
- signs of allergy: urticaria, erythema multiforme, hyperemia, photosensitivity, pruritus and angioedema;
- others: thrombocytopenia, weakening of visual acuity, purpura, alopecia, antinuclear antibody formation, myalgia, hyperhidrosis, conjunctivitis, and dry keratoconjunctivitis. In addition, there can be marked myasthenia gravis, gingival hyperplasia, vasculitis, pain in the joints and lipid metabolism.
Overdose
When using excessively large portions of drugs, there are rhythm disorders of the heartbeat or consciousness, bronchospasm, nausea, and in addition to this, cyanosis of the fingers, dizziness and convulsions that have a generalized nature. At the same time, one can expect the appearance of a tachycardia of a reflex nature or a significant decrease in the level of blood pressure that reaches shock.
In case of poisoning or the risk of shock, you must stop using the medication. If there is a strong drop in blood pressure values, it is necessary to carry out supporting procedures aimed at the operation of the CCC.
An overdose of amlodipine and atenolol requires monitoring of the respiratory function, the work of the cardiovascular system, diuresis and the volume of circulating blood.
The attending doctor may decide on the appointment of parenteral administration of calcium gluconate, atropine (bolus v / injection per dose of 0.5-2 ml), as well as glucagon (jet intravenous administration of 1-10 mg of the substance, and then - through an IV drip speed of 2-2.5 mg / hour), and in addition, sympathomimetics.
If there is insensitivity to medication and severe bradycardia, then EX is prescribed for a while.
In the event of generalized seizures at low speed, diazepam is injected intravenously.
Interactions with other drugs
You can not combine the drug with drugs that slow down the activity of ACE.
Parenterally-administered blockers of the Ca channels, as well as other antiarrhythmic drugs, can be administered to people using Tenochek exclusively on intensive care.
Anesthetics and antiarrhythmic agents in combination with the drug can potentiate the cardiodepressive effect of atenolol.
The combination with antidiabetic drugs may require changes in the dosage of insulin, as well as used inside hypoglycemic drugs.
Simultaneous use with clonidine, reserpine, and besides guanfacine, SG and α-methyldopa leads to the potentiation of the dromotropic, bathmotropic, and also chronotropic effect of the drug.
The antihypertensive effect of atenolol is weakened when combined with sympathomimetics, xanthines, estrogens and non-narcotic analgesics.
Apresin, antacids, and also nitroglycerin are capable of weakening the absorption of atenolol when combined with the drug, while cimetidine inhibits the metabolic processes of atenolol.
Sympatholytics increase the drug activity of atenolol.
There is a potentiation of the effect of atenolol on the central nervous system when Tenochek is combined with sedative drugs, neuroleptics, ethanol, hypnotics and tricyclics.
Quinidine, substances for inhalation anesthesia, calcium antagonists and amiodarone can enhance the effect of amlodipine.

Storage conditions
Tenochek is required to be maintained at temperature indices constituting within the range of 25 ° C.
Shelf life
Tenochek can be used within 36 months from the moment of manufacturing the pharmaceutical preparation.
Application for children
The medicine is not used in pediatrics.
