Tenacsum is the selective agonist of the endings of imidazole. Included in the group of antihypertensive drugs.

Release form

The release takes place in a tablet form, 15 pieces each inside a blister plate. The packet contains 2 such plates.

Pharmacodynamics

Tenaxum is an antihypertensive drug from the oxazoline category. It is selectively synthesized with I1-termini of imidazole inside the central, as well as peripheral (mainly renal) nerve centers of the vasomotor type. Synthesis of rilmenidine with imidazole endings slows the sympathomimetic activity of nerve centers that are peripheral and cortical, and also causes a decrease in blood pressure.

The medication leads to a decrease in the values of the blood pressure of the diastolic as well as the systolic type (the degree of diminution of the parameters depends on the size of the portion of the drug), having a therapeutic effect both in the patient's horizontal position and in the vertical position.

When administered in medicinal portions, the duration of exposure is 24 hours.

Pharmacokinetics

Suction.

The level of plasma Cmax (3.5 ng / ml) is observed 1.5-2 hours after a single dose of 1 mg of LS. Bioavailability indicators are 100%. The medicine is not subjected to the first hepatic pass.

The concentration of the drug is stable and does not have individual deviations. Admission together with food does not affect the bioavailability. The degree of absorption when used in medicinal portions does not change.

Distribution processes inside tissues.

Synthesis with a protein of blood plasma is less than 10%. Indicators of the distribution volume - 5 l / kg.

Metabolic processes.

The drug is extremely poorly metabolized. Only a small part of its decay products are noted inside the urine - they are a consequence of the oxidation or hydrolysis of the oxazoline ring. These metabolic products do not affect the α2-terminal.

Excretion.

The medication is mainly excreted through the kidneys: 65% of the consumed portion is excreted unchanged in urine. The index of renal clearance is 2/3 of the level of total clearance.

The half-life of drugs is 8 hours. This indicator is not tied to the size of the consumed portion and its repeated receptions. The drug effect is more prolonged, a significant hypotensive effect persists for 24 hours from the moment of drug use in people with elevated blood pressure values, which take it in a daily dose of 1 mg.

The phase of stable equilibrium indices inside the blood plasma is observed after 3 days and remains at the same level for 10 days.

Dosing and administration

It is required to take orally 1 mg of the drug per day. If the required result is absent after the 1st month of drug administration, it is allowed to increase the daily dose to 2 mg, which is used in 2 doses. Therapy is carried out by a long cycle.

Use Tenaxum during pregnancy

It is forbidden to use Tenaxum during pregnancy.

If you need to take drugs during the lactation period, you must stop breastfeeding, because Rilmenidine is excreted with human milk.

During the experimental tests, it was found that the Rilmenidine component does not have embryotoxic or teratogenic properties.

Contraindications

The main contraindications:

  • a state of severe depression;
  • renal insufficiency, which has a chronic form (QC values are <15 ml / min);
  • presence of hypersensitivity against rilmenidine.

Side effects Tenaxum

Taking the drug can lead to the emergence of certain side effects:

  • CAS function disorders: development of the heartbeat. At the same time, orthostatic collapse, cooling of extremities or hot flushes are noted singly;
  • disorders in the work of the central nervous system: a feeling of weakness due to physical activity, a feeling of drowsiness or insomnia. In addition, spasms, anxiety, or depression appear sporadically;
  • problems affecting the digestive activity: dry mouth mucosa, pain in the epigastric region, as well as diarrhea. Constipation or nausea is observed;
  • dermatological symptoms: puffiness or rash. Itching develops;
  • others: single violations of sexual function are noted.

Overdose

When the drug is poisoned, such manifestations as attention disturbance or marked decrease in blood pressure indicators are noted.

To eliminate the disturbances, it is necessary to rinse the stomach of the patient, and also to prescribe the reception of sympathomimetics (if necessary). Excretion of the substance with the aid of dialysis procedures is very insignificant.

Interactions with other drugs

After combined use with diuretics, vasodilators, and antihistamines, the antihypertensive effect of rilmenidine is increased.

The combination of Tenacum with tricyclics leads to a weakening of the hypotensive properties of the element rilmenidine.
