Telfast is a medicinal product against allergic reactions, a derivative of Terfenadine, which is devoid of cardiotoxic action.
Indications Telfast
Pathological processes in the body for the use of Telfast:
1. Allergic rhinitis. It can be seasonal (at a specific time period) or year-round (all year round). It is characterized by congestion and swelling of the nasal passages, and the release of a considerable amount of colorless mucus.
2. Seasonal fever. It is formed by contact with pollen of any plant to which there is increased sensitivity. It appears seasonally, during the flowering of the plant and stops on its own, but nevertheless causes unpleasant sensations to the patient.
3. Hives. There are many allergens that cause this allergic reaction, from medications to food products. Edema develops around the blood vessels, which expand. Because of this, the patient develops red spots on various areas of the skin. They quickly turn into blisters, and the patient begins to experience itching.
Release form
The medication is available in the form of film-coated tablets in three different doses:
- 30 mg, ten tablets;
- 120 mg, ten or twenty tablets;
- 180 mg, ten or twenty tablets.
Pharmacodynamics
The active substance of the drug, Fexofeadin, is a derivative of Terfenadine, exhibits antihistamine, selective antagonist activity to H1 receptors, without affecting H2 and H3 receptors.
In addition, unlike most allergy medications, the drug does not have a sedative or any other effect on the central nervous system.
During the tests to evaluate allergic reactions on the skin, it was found that the effect of taking Telfast 1 or 2 times a day begins to show its effectiveness after an hour. And it reaches its maximum, maintaining its effect throughout the day, in six hours.
Sensitivity to the drug does not develop even after twenty-eight days of use. With an increase in dosage from 10 mg to 130 mg, a dose-dependent increase in the antiallergic effect is observed if Telfas is taken once. For stable effectiveness throughout the day, a dose of at least 130 mg was needed, if using the same model of antiallergic action. The skin reaction was suppressed by more than eighty percent.
Patients with seasonal allergic rhinitis who received up to 240 mg Telfast (as twice daily dose) for fourteen days, when compared with the placebo group, showed the same duration of the QTc interval.
The same data are provided after taking the drug by healthy people twice a day at 60 mg for six months, 400 mg for 6.5 days, 240 (per day) for a year.
If the plasma concentration of the drug is exceeded by 32 times, Telfast will not affect the delayed-rectifier potassium pathways in the heart muscle.

Pharmacokinetics
Telfast is quickly absorbed and reaches maximum density values (in one to three hours). At the same time, the concentration value at different doses will differ. For example, when used in knocks 120 mg - 289 ng / ml, 180 mg - 494 ng / ml.
The active substance binds to serum proteins by sixty to seventy percent. Metabolism occurs in the liver or outside of it, as evidenced by the presence of Fexofenadia in urine and feces.
The final half-life is about eleven to fifteen hours if the medication is taken in a course.
Based on the information that is currently available, Telfast is mainly excreted in bile, and a small amount (approximately ten percent) is excreted in urine.
Dosing and administration
The medication is contraindicated for children under six years of age.
For children aged six to eleven years, it is recommended to take Telfast at 30 mg twice a day.
Adults and children over twelve years of age can take Telfast at a dose of 120 or 180 mg once a day.
The pill should be washed down with a large volume of water. Its intake does not depend on food intake. If prolonged use of the medicine is required, it is advisable to adhere to a period of time of knocks.
For patients who belong to the elderly population or with liver or kidney failure, no change in the recommendations for administration is required.
Use Telfast during pregnancy
Telfast cannot be used during the gestation period. This is explained by the lack of reliable information on the safety of using this medication during this period. It is worth noting that animal studies have not revealed any negative impact on the development of the child in utero and postpartum, as well as on the course of pregnancy and labor.
Lactation
It is not recommended to use Telfast while breastfeeding, as there is no information about its presence in mother's milk. But it is worth keeping in mind that Terfenadine can penetrate into breast milk. This is important because Fexofenadine is a metabolite of Terfenadine.
Contraindications
In some pathologies, Telfast should be used with caution:
- Chronic renal failure. The process of excretion of the drug may be disrupted, thereby increasing its duration of action.
- Chronic liver failure. Due to the decrease in hepatocyte function, the usual dose of the drug will have a more pronounced and prolonged effect.
- Elderly patients. The dose of the drug needed to achieve the desired effect is lower than usual, since with age all body functions weaken.
- Patients with heart muscle pathology. Telfast should not be used by people who suffer from ischemic heart disease or have recently had a heart attack. This is because the medication can significantly increase the load on the heart muscle.
Telfast should not be used in the treatment of:
- In case of individual sensitivity to any component of the medicine. In this case, you should contact your doctor so that he can recommend a medicine from another antiallergic group.
- Breastfeeding period. Since there is a possibility that Telfast can get into the mother's milk and cause the child to develop hypersensitivity to this medication. This is due to the unstable immune system of infants.

Side effects Telfast
No severe adverse effects were observed. Dyspeptic disorders, headache, weakness and increased sleepiness did not differ in frequency of occurrence from those in the control group.
Overdose
Data from studies in healthy people show that taking 800 mg of Telfast once (or 690 mg twice daily for two months, or 290 mg twice daily for a year) does not cause significant adverse effects on the body.
But if you take six to ten tablets at a time, you may experience signs of an overdose. For example, such as: drowsiness, decreased reaction, inhibition. This happens because Telfast, due to its very high density in the blood, probably begins to affect the H3-histamine receptors in the brain.
As soon as the patient is diagnosed with an overdose, his stomach should be immediately washed and activated carbon should be given to prevent further absorption of Telfast into the systemic circulation. If these actions do not help the patient and his condition worsens, an ambulance should be called immediately.
Interactions with other drugs
Telfast may interact with some medications, such as:
1. Erythromycin or Ketoconazole - there is an increase in the density of Telfast in the blood serum by two to three times, but without significantly increasing the QTc interval. At the same time, there is no particular difference between taking these drugs together or as motor therapy.
2. Omeprazole – no interactions observed when used together.
3. Medicines that are metabolized in the liver do not react.
4. Medicines (antacids) that contain aluminum or magnesium - if taken fifteen minutes before taking Telfast, this will lead to a decrease in the bioavailability of Fexofenadine due to its binding in the gastrointestinal tract.
Storage conditions
Telfast should be stored in a dry place, out of reach of children, at a temperature of 25°C.

- Patients who took Telfast speak positively about its high efficiency and speed of action. They note that thanks to the use of this medication for allergic rhinitis, nasal breathing is quickly restored. But due to the high price and long-term use (for some pathologies, taking the drug can last up to several weeks), the medication also has negative reviews.
- Medical workers also dismiss Telfast as an effective, reliable remedy. A positive advantage is also the selective blocking of H-histamine receptors (Telfast blocks only H1, without affecting H2 and H3). Due to this, the likelihood of side effects is significantly reduced even with prolonged use.
