Paralen is a drug with antipyretic and analgesic properties.
Indications Paralena
It is used to treat headaches, including migraines, and also pain in the back or muscles, toothaches, rheumatic pains, periodic female pains. Also on the list are pains that occur with arthritis.
In addition, the drug helps reduce the symptoms of fever and pain that occur with the flu or cold.
Release form
The product is released in tablets, 10 or 12 pieces inside a blister pack. The box contains 1-2 such packs.
Paralyzed baby
Paralen baby is produced as an oral suspension in 100 ml glass bottles. The pack contains 1 bottle complete with a dispenser.
Paralyzed hot drink
Paralen hot drink is available in powder form in sachets, 5 g each. The pack contains 5-6 or 10 or 12 such sachets.
Paralene combi
Paralen Combi is produced in the form of drops for oral administration, inside a bottle with a dropper of 25 ml. There is 1 such bottle in the box.
Paralene thyme
Paralen thyme is available in lozenges, 10 pieces inside a blister plate. There are 2 such plates in the package.
Paralen thyme-primrose
Paralen thyme-primrose is a liquid for oral administration, in 100 ml (or 130 g) bottles. There is 1 bottle inside the pack.

Pharmacodynamics
The mechanism of drug action is due to the suppression of PG activity within the CNS. This type of suppression is selective.
The analgesic effect of paracetamol when taken in a single medicinal dose of 500-1000 mg lasts for 3-6 hours. The antipyretic effect lasts for 3-4 hours.
Pharmacokinetics
Paracetamol is absorbed almost completely and at high speed in the gastrointestinal tract. The substance is quickly distributed in fluids and tissues. Peak plasma values are noted after 0.5-1 hour after taking the medication.
The drug passes through the BBB and penetrates into the mother's milk with saliva. Paracetamol undergoes intensive biotransformation processes. Conjugation and oxidation reactions also develop, during which toxic products of drug metabolism are formed. When the drug is administered in medicinal portions, hepatotoxic intermediate products of metabolism are biotransformed at high speed with the participation of glutathione. In the process, mercapturic acid is formed, excreted mainly in the form of conjugates with urine.
Less than 5% of the administered dose of paracetamol is excreted unchanged. The half-life is within 1-3 hours. In people with severe liver failure, this period is extended to 5 hours.
In case of renal failure, this period is not increased, but due to the fact that paracetamol is excreted mainly through the kidneys, its dosage should be reduced.
Paracetamol is able to pass through the placenta and penetrate into breast milk.

Dosing and administration
The medication is taken orally. For adolescents over 12 years of age and adults, the dosage is 1-2 tablets taken 4 times a day.
For children aged 6-12 years, the serving is 0.5-1 tablet, taken 3-4 times a day.
The interval between doses should be at least 4 hours. It is prohibited to take more than 8 tablets (4 g) of the drug in a 24-hour period.
The duration of the course of therapy is determined by the attending physician. Without consulting him, the medication can be used for a maximum of 3 days.
It is prohibited to exceed the dosage indicated in the instructions, as well as to combine Paralen with other drugs that contain paracetamol.

Use Paralena during pregnancy
Prescribing Paralen during pregnancy is permitted only in situations where the possible benefit from taking it for the woman is more expected than the development of complications in the fetus.
Paracetamol is excreted in breast milk, but in quantities insignificant for the medicinal effect. Current information does not prescribe a ban on breastfeeding during treatment.
Contraindications
Main contraindications:
- the presence of high sensitivity to the elements of the drug;
- G6PD deficiency;
- anemia in severe or hemolytic form, blood diseases and non-hemolytic familial jaundice;
- leukopenia;
- severe liver or kidney failure;
- acute form of hepatitis, congenital hyperbilirubinemia;
- combination of drugs with hepatotoxic agents;
- alcoholism.
Side effects Paralena
The use of drugs may lead to the appearance of certain side effects:
- immune disorders: the appearance of anaphylaxis and signs of intolerance, including itching and rashes on the skin and mucous membranes (often erythematous or generalized and urticaria), and in addition, MEE (also Stevens-Johnson syndrome), Quincke's edema and TEN;
- digestive disorders: development of epigastric pain or nausea;
- manifestations affecting the endocrine system: the occurrence of hypoglycemia, which can lead to the development of hypoglycemic coma;
- symptoms from the lymph and hematopoietic processes: development of agranulocytosis, anemia (also its hemolytic form), thrombocytopenia, as well as sulfhemoglobinemia with methemoglobinemia (pain in the heart, cyanosis and dyspnea) and the appearance of bruises or bleeding;
- disorders affecting respiratory processes: bronchospasms in patients with intolerance to aspirin or other NSAIDs;
- problems with digestive activity: impaired liver function, increased activity of liver enzymes (jaundice usually does not develop).
Overdose
Manifestations of intoxication: paleness of the epidermis, loss of appetite and vomiting with nausea. In addition, the development of hepatonecrosis begins (the degree of expression of such necrosis is determined by the degree of poisoning with the drug), the activity of liver transaminases and the values of PTT increase. After 1-6 days, a noticeable clinical picture of liver destruction appears.
To eliminate the disorders, it is necessary to administer SH-category donors to the victim, as well as precursors of glutathione-methionine binding processes (8-9 hours after poisoning), as well as N-acetylcysteine (12 hours later). To prevent the development of late hepatotoxic effects, gastric lavage is performed.

Interactions with other drugs
The drug potentiates the medicinal effect of MAOIs.
Weakens the properties of uricosuric drugs.
Caffeine promotes the acceleration of ergotamine absorption.
Paracetamol increases the excretion time of the element chloramphenicol by 5 times.
When used for a long time, paracetamol can potentiate the therapeutic properties of anticoagulants (dicumarin derivatives).
The combination of the drug with ethyl alcohol increases the likelihood of hepatotoxic effects and the development of acute pancreatitis.
Phenytoin with rifampicin, as well as barbiturates with phenylbutazone, ethyl alcohol, tricyclics and other stimulators of microsomal oxidation increase the production of hydroxylated active products of drug breakdown, as a result of which severe poisoning can develop even with weak overdoses.
Drugs that slow down microsomal oxidation processes (such as cimetidine) reduce the likelihood of hepatotoxic effects.
Domperidone and metoclopramide enhance, while cholestyramine, on the contrary, weakens absorption.
Storage conditions
Paralene must be kept in a place inaccessible to children. Temperature indicators - maximum 25°C.
Shelf life
Paralen can be used for 3 years from the date of manufacture of the therapeutic agent.
Application for children
The medication should not be used by children under 6 years of age or those weighing less than 20 kg.

Analogues
The following drugs are analogs of the drug: Rezistol, Minolexin, Gripp-Heel with Ambroxol, and also Inspiron, Pulmobriz, Ecoclav and Umckalor with Eukabal, Pertussin, Langes and Tussrem Plus. The list also includes licorice root syrup.
