Samesil is an effective sulfanilamide agent that has a direct effect on digestive and metabolic processes in the body. The active component of Samesel is mesalazine (mesalazine), which is actively used in inflammatory bowel diseases.

Depending on the form of the drug, Samesel is prescribed for various pathologies of the intestine:

  • candles for intrarectal administration Samezil is prescribed for non-specific ulcerative colitis with involvement in the rectal process (ulcerative inflammation);

Release form

Soluble inside the intestine tablets (10 pieces in a blister pack) based on 400 or 800 mg of mesalazine, containing a number of auxiliary substances that form the coating of the drug (starch, maltodextrin, macrogol, talc, etc.). The shell is characterized by a brownish-red hue, sometimes with additional inclusions. The inside of the tablet is gray.

Rectal suppositories (5 pieces per pack) contain solid fats and 500 mg of active ingredient of mesalazine. The form of the suppository is conical, the color is pastel.

Suspension for intrarectal use consists of 4 g of mesalazine (per 100 g of preparation), tragacanth, sodium acetate, xanthan gum and other additional components. The suspension is a homogeneous mass of a cream shade. Produced in plastic capsules of container type, with a capacity of 50 or 100 g. The package contains an applicator and an annotation on the use of the drug.

Pharmacodynamics

The medical agent Samesil has a local anti-inflammatory effect, which is explained by oppression of the function of neutrophilic lipoxygenase and a decrease in the production of prostaglandins and lipid highly active substances of leukotrienes.

Samesil inhibits migratory, phagocytic and degranulation properties of neutrophils, blocks the lymphocyte secretion of immunoglobulins.

Samesil differs antimicrobial action in relation to the coccal flora and Escherichia coli. Antimicrobial effect is fully manifested in the large intestine.

Samedil can act as an antioxidant, since it has the property of forming bonds and destroying free radicals of oxygen.

The release of the active ingredient occurs in the terminal part of the small intestine, as well as in the large intestine. Forms such as suppository and suspension, have the maximum effect in the distally located areas of the large intestine, as well as directly in the rectum.

Pharmacokinetics

Almost half of the ingested amount of the drug Samesil is absorbed mainly in the cavity of the small intestine. The process of acetylation (replacement of hydrogen atoms) in the active ingredient of mesalazine proceeds in the mucosa of the intestinal tissue and in the liver, with the formation of N-acetyl-5-aminosalicylic acid.

The binding with plasma proteins is more than 40%.

Dosing and administration

Method of use for tablets Samedil:

  • The tablet is drunk between meals, without chewing or grinding, with water;
  • in the acute stage of the disease, adult patients are shown to use 800 mg of the drug three times a day, maximum - 4 g / day;
  • in the acute stage of the disease, children from 40 kg of weight are prescribed maximum 50 mg / kg of drug per day;
  • as a supportive treatment for adults shows the intake of 400 mg of the drug 4 r./day or 800 mg of the drug 2 r./day, and a dose of 30 mg / kg / day is prescribed for children from 40 kg of weight.

Method of application of rectal suppositories Samesil at 500 mg:

  • in the acute stage, adults and children weighing 40 kg - 1 suppository three times a day;
  • as a supportive treatment for adults and children weighing 40 kg - 1 suppository once a day.

Before you enter the suppository into the rectum, you should take measures to empty the intestine. The suppository is injected deeply and held for at least 60 minutes. For convenience of administration, it is recommended to hold the suppository for 5 minutes in cold water.

Method of application of rectal suspension Samesil:

  • in the acute stage of 50-100 ml of the drug in the morning and at night;
  • as a maintenance treatment - 50 ml of the drug before going to bed.

For children, the dosage and frequency of application of the suspension is selected by the doctor individually. The limiting amount of Samesil for children is 50 mg / kg in the acute stage, or 30 mg / kg as a maintenance treatment.

The suspension is administered by first cleaning the intestine: the capsule with the preparation is shaken, the protective cap is removed, the tip of the applicator is inserted deep into the anal opening. The procedure is more convenient if the patient lies on his left side, bending his right leg and pulling out his left.

The drug is administered gradually, without hurrying. At the end of the introduction, the patient should continue to lie for at least 30 minutes.

Usually the course of therapy with Samesyl suspension lasts up to 3 months, until a stable period of remission is reached.

Use Mezavant during pregnancy

It has been proven by specialists that the active ingredient of Samesil overcomes the placenta, but the degree of risk of fetal damage remains uncertain, since studies of this kind have not been conducted. Due to a lack of information, it is not recommended to use Samesil to pregnant women, unless in the case when the use of the drug is vital.

The active ingredient and the products of its metabolism are found in sufficient quantities in the milk of the nursing mother. The question of using Samesil in this period should be decided by the doctor.

Self-treatment of Samesel is unacceptable, especially in pregnant and lactating women.

Contraindications

There are situations when the use of Samesil is extremely undesirable or unacceptable:

  • with a high probability of an allergic response of the body to the components of the drug;
  • with blood diseases and hematopoiesis;
  • in the acute stage of stomach and duodenum ulcers;
  • with a deficiency of glucose-6-phosphate dehydrogenase;
  • with increased bleeding, hemorrhagic diathesis;
  • with severe liver and kidney damage;
  • in the period of gestation (especially in the third trimester of pregnancy);
  • in the period of breastfeeding;
  • in early childhood (up to 2 years).

Side effects Mezavant

Side effects during treatment of Samesel can affect various systems and organs of the patient:

  • attacks of nausea and vomiting, stool disorders, appetite disorders, thirst, inflammation of the oral mucosa, inflammation of the pancreas and liver;
  • increased heart rate, blood pressure changes, discomfort in the heart, shortness of breath, inflammation of the heart muscle or pericardium;
  • pain in the head, ringing in the ears, trembling in the limbs, development of depressive state, dizziness;

Overdose

Cases with an overdose of Samesel occur very rarely, but can manifest themselves with such signs:

  • attacks of nausea, vomiting;
  • pain in the area of the projection of the stomach;
  • increased fatigue, apathy;
  • drowsy condition.

An overdose of Samesil is treated by washing the stomach, taking a laxative. According to the indications, it is possible to prescribe symptomatic therapy.

Sometimes the manifestation of side effects may increase. In this case, the decision to cancel Samesil is taken by the attending doctor.

Interactions with other drugs

Samesil has the ability to enhance the effect of drugs based on sulfonylureas, increases the property of ulcer formation of glucocorticosteroid agents and the toxic effect of methotrexate.

Samesil impairs the effect of Furosemide, sulfonamide drugs, Rifampicin, Spironolactone, and also improves the action of drugs that prevent blood clotting.
