Medogrel is included in the group of antithrombotic and antiplatelet drugs.

Indications Medogrel

It is used to prevent the development of atherothrombosis in people suffering from ACS. Usually, the drug is used to treat such disorders:

  • recent myocardial infarction in acute form;
  • an unstable form of  angina pectoris ;
  • myocardial infarction, having a subendocardial character;
  • the appearance of complications of atherothrombotic nature after the transfer of myocardial infarction or stroke;
  • obliterating atherosclerosis in peripheral arteries.

Release form

The release of the substance is realized in tablets having a volume of 75 mg, 30 pieces per package.

Pharmacodynamics

The main active element of the drug, the substance clopidogrel, effectively inhibits the processes of platelet aggregation, thereby helping to irreversibly modify their ADP-terminations.

The inhibition of platelet aggregation induced by ADP activity occurs after the first use of a daily portion of the drug, which is 75 mg.

With the repeated administration of the same dosages, this effect is potentiated, acquiring stability between the 3-7th days of the treatment cycle. In this phase, the degree of platelet aggregation inhibition can fluctuate, amounting to approximately 40-60%.

After 5 days from the moment of stopping the therapy, the platelet aggregation returns to the initial values.

Pharmacokinetics

Clopidogrel is absorbed at a fairly high rate when consumed both single and reusable daily portions of 75 mg of the substance. Plasma Cmax values are noted on average 45 minutes after the application of the drug.

Inside the liver, extensive metabolic processes are carried out - involving esterases or enzymes of the hemoprotein P450.

Medogrel excretion occurs approximately after 2 hours from the time of drug use - approximately 50% is excreted by the kidneys, and about 46% - with feces.

Dosing and administration

For admission of adults with ischemic stroke, myocardial infarction and diseases in the peripheral arteries, prescribe the use of the 1st pill (75 mg) of drugs per day, without binding to the reception of food. The most suitable duration of the therapeutic cycle is selected individually, and this should only be done by a doctor.

People who previously suffered a myocardial infarction often determine the medication intake during the 25-day period. And after strokes, therapy usually lasts 7 days.

Use Medogrel during pregnancy

Due to the fact that at the moment there is no information on the effect of the main active element of drugs (clopidogrel) on the course of pregnancy and its excretion with mother's milk, it is forbidden to prescribe drugs to lactating or pregnant women.

Contraindications

The main contraindications:

  • presence of severe sensitivity or intolerance relative to the active element of drugs or excipients;
  • Insufficiency of hepatic activity, which has a severe degree of severity;
  • bleeding, which is acute.

Side effects Medogrel

Most often, the medication causes the appearance of such negative symptoms: hemorrhages under the skin, bruises, diarrhea, nasal or gastrointestinal bleeding, pain in the abdominal area and signs of dyspepsia.

More rarely develop such violations: leuko-, thrombocyto- or neutropenia, as well as eosinophilia. There are bleeding, affecting the visual organs, hemorrhages inside the skull, headaches, ulcers in the gastrointestinal tract, dizziness, bloating, gastritis, itching or rashes, and constipation.

Among the single side symptoms : a febrile condition, vasculitis, serum sickness, anemia, thrombotic purpura, urticaria, hepatitis, hallucinations and a sense of confusion, and besides this rash, an increase in blood pressure, a taste disorder and a liver function deficiency to an acute degree.

These negative symptoms may appear due to non-compliance with the regimen of drug use or individual hypersensitivity of the patient.

Overdose

When using medicament in doses that exceed the permissible limits many times, the duration of bleeding significantly increases, which can lead to various complications.

Interactions with other drugs

It is forbidden to combine the medicine with internal anticoagulants, because this increases the probability of intensifying the intensity of developing bleeding several times.

Combined use with medications that slow down the activity of glycoprotein type GPIIb / IIIa should be performed with great care in people with a predisposition to bleeding due to trauma and surgical procedures.

If Medogrel is taken with aspirin, you can not take the latter in a dose exceeding 0.5 g twice a day.

An increase in the likelihood of bleeding can occur with a combination of the drug with heparin.

In the case of drug use together with naproxen and other medicines from the NSAID group, it is possible to increase the incidence and number of latent bleeding inside the gastrointestinal tract.
