External manifestations of hemorrhoids are quite typical: itching, discomfort, vascular seals and nodules near the anus, and sometimes severe pain and swelling. How to treat such a delicate disease? Drug therapy usually includes painkillers, anti-inflammatory, thrombolytic, phlebotonic and hemostatic drugs for external hemorrhoids. Most often, oral medications are combined with ointments or gels that are applied directly to problem areas. If such treatment is started on time, then surgery can be avoided.
Troxerutin in the treatment of external hemorrhoids
Troxerutin is considered a complex drug and can be used not only for external hemorrhoids, but also for various venous diseases in general. Among the indications for the use of Troxerutin there are almost twenty vascular pathologies - among them varicose veins, thrombophlebitis, chronic venous insufficiency.
The active component of the drug is a natural flavonoid with P-vitamin properties. It takes part in oxidation and reduction mechanisms, inhibits hyaluronidase, normalizes the structure of cell membranes and reduces the permeability and fragility of capillary vessels. Increases the density of venous walls, stops the development of inflammatory processes.
People suffering from hemorrhoids are advised to undergo treatment with Troxerutin regardless of the stage of the pathology. Since the drug is available both in capsule form and as an external agent, it has a broad therapeutic effect, it can be used as a monodrug. This has a huge "plus": along with a pronounced therapeutic effect, the drug load on the patient's body is reduced.
Troxerutin is not recommended for internal use if the patient suffers from gastric ulcer. External use has no restrictions, except for allergic sensitivity of the body.
Relief Advance for External Hemorrhoids
If external hemorrhoids are accompanied by severe pain, then Relief Advance is the best way to eliminate them. The analgesic effect of this drug is really strong: it contains benzocaine, which is several times more powerful than lidocaine or novocaine, which are usually included in the composition of antihemorrhoidal local agents.
Benzocaine, when applied externally, is not absorbed into the circulatory system and does not have a general effect. The effect is complemented by the presence of cocoa butter in the suppositories, which softens and relieves irritation from the inflamed tissue of the rectum.
External hemorrhoids are not the only indication for the use of Relief Advance. The drug is actively prescribed for rectal fissures, proctitis, etc. The standard scheme of use is the introduction of a suppository in the morning and evening, until a stable relief of the condition.
Side effects during treatment: redness, mild itching near the anus, allergy.
Natalsid
Natalsid is a safe hemostatic agent, therefore it is most recommended for use in hemorrhoids, which are accompanied by bleeding. It can be used by pregnant and lactating women.
The hemostatic effect is complemented by a restorative effect: the suppositories contain sodium alginate, a natural polysaccharide obtained from brown algae (laminaria). The basic component is hyaluronic acid, a substance that stimulates cell division processes in damaged tissues, including vascular walls. After entering the intestinal lumen, hyaluronic acid is transformed and passes into a gel-like state, so it also has an astringent effect.
Natalsid is administered rectally 1-2 times a day, depending on the phase of the inflammatory process. In general, the drug is one of the safest drugs, and only occasionally causes a hypersensitive reaction in the form of itching, burning and redness of the skin.
Proctoglivenol
Proctoglivenol occupies a worthy place among many antihemorrhoidal drugs. This medication has a local analgesic effect, inhibits the formation of nodular dilatations of venous vessels, and creates protection for vascular walls. The composition of the drug is represented by tribenoside, a substance that reduces capillary permeability, stabilizes vascular tone, and reduces the number of inflammation and pain mediators.
An additional pain-relieving effect is provided by lidocaine, another active component of Proctoglivenol. The drug is used in the morning and at night: such a two-time administration allows you to quickly stop the acute course of hemorrhoids, relieve unpleasant painful symptoms. After the acute process subsides, the suppositories can be used once a day, immediately before bedtime.
The duration of the treatment course is determined individually, but usually does not exceed 10 days.
Hepatrombin G
The drug is an antihemorrhoidal agent for local use, so it is the best choice for treating external hemorrhoids. Gepatrombin G is a combination drug, since it contains the anticoagulant heparin, the synthetic glucocorticoid substance prednisolone and the sclerosing component polidocanol. Thanks to this combination of active ingredients, inflammatory signs quickly disappear, pain and itching are relieved, hematomas are absorbed, and thrombophlebitis is prevented.
In addition to external hemorrhoids, the drug is often prescribed for anal thrombophlebitis, internal hemorrhoids, and anal fissures.
In the acute period of the disease, the ointment is applied to the area of inflammation two to four times a day, and after the painful symptoms subside - once a day. The treatment period is usually one week.
Treatment is usually well tolerated, and the likelihood of side effects is minimal when the drug is used appropriately.
Proctosedyl for external hemorrhoids
When it comes to complex treatment of external hemorrhoids, Proctosedyl comes to the rescue - this is a multi-component remedy, which contains a powerful analgesic benzocaine, an anti-inflammatory hormonal substance hydrocortisone, an antibacterial component framycetin, which promotes the healing of infected wounds or anal fissures. Another important component is esculoside - a structural analogue of vitamin P. Its basic property is to prevent vascular fragility, restore their elasticity, and reduce the likelihood of bleeding.
The analgesic effect of Proctosedyl is quite long-lasting, due to the presence of butamben. And heparin, a well-known anticoagulant, provides the antithrombotic activity of the drug.
The composition of the medicine is rich and multifunctional, therefore Proctosedyl is prescribed for any form of hemorrhoids, and the effect becomes noticeable within a quarter of an hour after application. For external hemorrhoids, the product is used both in the form of suppositories and in ointment form. The frequency of application is twice a day, and the duration is no more than a week.
Side effects during treatment: allergic reactions, addiction effect during long-term treatment.
Posterisan
The ointment Posterisan is prescribed for external hemorrhoids and anal fissures. The main action of the ointment is to stimulate local immunity, accelerate wound healing, stop the development of the inflammatory process, and prevent reinfection.
Posterisan is rubbed into the anal area in the morning and at night. The duration of treatment can be 5-7 days.
In patients with hypersensitivity to phenol, Posterisan may provoke the development of contact dermatitis. However, in other cases, side effects are practically non-existent.
Posterisan forte
Posterisan forte is a combination drug. Unlike standard Posterisan, it contains the anti-inflammatory hormonal component hydrocortisone, which enhances non-specific and specific immune reactions and stops the development of an inflammatory reaction.
Due to the presence of a corticosteroid, the drug should not be used for more than 5-7 days in a row, twice a day - in the morning and at night. Treatment is usually stopped as soon as the main symptoms of the disease disappear.
Possible side effects: itching and burning in the anus, pigment formation disorders, dry skin, dermatitis. With prolonged use, adrenocortical suppression, skin atrophy, telangiectasia develop.
Detralex
The oral drug Detralex, as well as its analogues Normoven, Venarus or Avenue, are considered to be among the most effective venotonics and venoprotectors. The composition of these medications is represented by diosmin and hesperidin: these are natural flavonoids that have the ability to establish microcirculation and provide protection for blood vessels. A full course of treatment with Detralex allows you to reduce the degree of extensibility of venous walls, reduce blood stagnation and capillary permeability, and eliminate vascular fragility. It is also important that the effectiveness of the drug has been confirmed experimentally: it is recommended for use in internal and external hemorrhoids, chronic venous insufficiency, as well as constant fatigue and pain in the lower extremities.
For acute hemorrhoids, the daily dosage of the drug is 4 tablets: two tablets should be taken in the morning and the same amount in the evening. After four days, the dose is reduced to two tablets per day. Treatment can be long-term, up to several weeks.
Detralex is a safe drug, treatment with which is practically not accompanied by side effects. Only occasionally are individual hypersensitivity reactions observed.
Levomekol for external hemorrhoids
The drug of levomycetin and methyluracil serves to accelerate the healing of wounds and cracks, including in the anal area. The antibiotic levomycetin is a well-known component with a wide range of antibacterial activity. It potentiates cellular restoration, eliminates signs of inflammation.
Levomekol has a predominantly local effect, and its medicinal activity manifests itself within 24 hours after application.
For external hemorrhoids, Levomekol is prescribed in the presence of cracks, ulcers, inflammatory changes, infected elements in the anus. But this drug is powerless for fungal skin lesions.
The ointment is even prescribed in pediatrics – for the treatment of children from the age of three. However, its use has a number of features:
- the ointment is used from the first day of exacerbation and for four days;
- the ointment is not suitable for longer-term use;
- On the fifth day, Levomekol should be replaced with another alternative remedy.
The preparation can be applied directly to the skin and rubbed into the tissues little by little, or applied as a compress or bandage.
Possible side effects include skin rashes, redness, and dermatitis, as a manifestation of an allergic process.
Troxevasin
Troxevasin is a complete analogue of the drug Troxerutin and is one of the most popular angioprotectors and capillary stabilizing agents.
The product is available as an external gel or capsules and is a mixture of natural flavonoids that prevent damage to cell membranes. Troxevasin improves venous-arterial blood flow, prolongs the period of venous filling, optimizes microcirculation and small-vessel perfusion.
In addition to external hemorrhoids, the drug is actively prescribed for venous insufficiency, pre-varicose and varicose syndrome, thrombophlebitis, etc.
The capsules are taken twice a day, in the morning and at night, after meals, one piece at a time. The gel is applied to the anus area also twice a day, always at night. In chronic hemorrhoids, during an exacerbation, it is preferable to use both medicinal forms of Troxevasin for treatment - both the gel and the oral drug.
Overdose of this medication is considered extremely unlikely. Rarely, allergic reactions occur in the form of skin manifestations, dermatitis, angioedema.
Dimexide
An external solution of dimethyl sulfoxide is Dimexide, which is actively used to achieve a local anti-inflammatory, analgesic, and antiseptic effect.
Dimexide should not be used if external hemorrhoids are accompanied by the appearance of cracks, ulcers, wounds: damaged skin is considered a contraindication to the use of the solution.
In addition, the drug can provoke the development of an allergic reaction, so before the first use, you should always conduct a test - the so-called sample.
As a rule, Dimexide is used in the form of compresses or tampons. In pediatrics - namely, children under 12 years old - the drug is not prescribed.
Possible side effects: dry skin in the area of use, erythema, dyspepsia, sleep disturbances, headaches.
Bezornil
Bezornil ointment for external use is a specific antihemorrhoidal agent of combined action. It has a pronounced astringent, anti-edematous, restorative, analgesic and hemostatic effect, so it can be safely used for external hemorrhoids, occurring with pain, itching, bloody discharge and cracks in the anus.
The ointment is applied in a small amount to the affected area twice a day and additionally after each bowel movement. You can treat the skin directly, or apply the preparation to a cloth napkin and apply it to the anus. The duration of treatment is determined by the doctor.
Among the side effects, only rare allergic reactions are indicated: in general, the drug is safe and quite effective.
Dolobene for external hemorrhoids
A gel preparation based on heparin, dexpanthenol and dimethyl sulfoxide is usually prescribed to relieve muscle and joint pain. However, it is possible to use the preparation for external hemorrhoids, since it relieves pain, eliminates swelling, stops the development of an inflammatory reaction, and at the same time promotes wound healing and rapid tissue epithelialization. But it is better not to use Dolobene for bleeding wounds, since this can only aggravate the problem.
As a rule, for external hemorrhoids, the gel is applied to the inflamed area two to four times a day. At night, you can apply cloth napkins with the preparation.
If after the start of treatment with Dolobene, itching, swelling, skin rashes appear in the area of application, this may indicate the development of an allergic process. In such a situation, it is better to replace the product with another, more suitable one.
Ichthyol ointment for external hemorrhoids
Ichthyol is a well-known analgesic, anti-inflammatory and disinfectant. It is successfully used to treat burn wounds, eczema, neuralgia, arthrosis and arthritis, erysipelas. Important: ichthyol ointment cannot be combined with any other external agent, since the consequences of such a combination are impossible to predict.
For external hemorrhoids, apply a small amount of ointment to the anal area up to three times a day, preferably under a gauze napkin. The duration of therapy is determined individually.
During treatment, sometimes unpleasant symptoms such as allergies and skin irritation occur. In these cases, the product should be replaced with another one with a different composition.
Olestezin
Suppositories based on etazol, concentrated sea buckthorn oil and anesthesin are an excellent local remedy for the treatment of external hemorrhoids. Olestezin is known for its healing, anti-inflammatory, general antihemorrhoidal properties. During treatment with the drug, the tone of the vascular wall increases, cracks and small wounds heal, and dilated venous nodes decrease in volume. Olestezin is especially effective for men who have external hemorrhoids combined with inflammation of the prostate gland.
Suppositories are administered rectally, one piece twice a day (three times a day if necessary). The standard duration of therapy is up to one week. Olestezin is not used for more than ten days in a row.
The drug is well accepted by patients, allergic reactions occur relatively rarely.
Menovazin
The analgesic effect of Menovazin is due to the presence of menthol, procaine and benzocaine. The product eliminates itching, relieves pain and can be used as a symptomatic drug for external hemorrhoids, despite the fact that its usual area of application is neuralgia, arthralgia and myalgia.
Menovazin can be applied to the skin near the anus up to three times a day, and the duration of treatment should be determined by a doctor. Side effects are limited to a possible allergic reaction to the components of the product.
Aurobin
The ointment with a complex composition Aurobin is represented by the corticosteroid component prednisolone, the local analgesic lidocaine and dexpanthenol. The corticosteroid substance has a local anti-inflammatory effect, increases vascular tone and reduces vascular permeability. The analgesic eliminates unpleasant painful and itchy sensations, and dexpanthenol restores damaged mucous and epithelial tissues.
Due to the composition of the drug, it should not be used for a long time. The ointment is applied up to 4 times a day for up to seven days. It is better to reduce the frequency of use and the duration of treatment to the minimum possible values.
Possible side effects: addiction to the drug with prolonged use, delayed wound healing.
Nigepan
Less popular, but quite effective suppositories recommended for patients with external hemorrhoids are Nigepan. The product is a combination of the anticoagulant heparin and the analgesic benzocaine. Thus, Nigepan has a local anesthetic, anti-inflammatory, antithrombotic effect.
Use one suppository in the morning and one at night for 7-14 days.
Side effects when using Nigepan may include: slight bleeding from anal fissures, allergic reactions.
Vinilin for external hemorrhoids
The liquid based on polyvinyl butyl ether Vinilin is better known as Shostakovsky's balm. The product is actively used for external treatment of carbuncles, furuncles, trophic and purulent wounds, burns, dermatitis and external hemorrhoids.
The balm is moistened onto a cloth napkin or applied directly to the inflamed area. The preparation can be added to ointments or other oils, including vegetable ones. It is optimal to treat the affected area 4 times a day, but the duration of treatment is determined individually.
Vinilin rarely causes allergic reactions, which are mainly limited to local manifestations in the form of itching, swelling and small rash. However, in case of individual hypersensitivity of the body, the product should not be used.
Panthenol suppositories for external hemorrhoids
Panthenol, Depantol are effective suppositories of mild action, recommended for the treatment of gynecological diseases. However, the antimicrobial, healing and antiseptic properties allow them to be used in the treatment of external or internal hemorrhoids. The composition of the drug includes dexpanthenol (D-panthenol) - a component that stimulates the restoration of mucous tissues, normalizes cellular metabolism, and stops the inflammatory process.
Suppositories with dexpanthenol are prescribed for rectal administration, one piece twice a day, for 1-1.5 weeks. If necessary, treatment can be continued for up to three weeks.
The suppositories have a mild effect, so side effects in the form of allergic symptoms are extremely rare.
Ointments for the treatment of external hemorrhoids
The most popular, simple and easy-to-use medications for hemorrhoids are ointments and creams - external agents that can be purchased in a wide variety at any pharmacy. Ointments provide a local effect on the problem, which means the delivery of medicinal substances directly to the site of inflammation.
Ointment or gel preparations for hemorrhoids are used equally often, however, experts note that ointments in most cases have a higher concentration of medicinal components, unlike gels, and their absorption is an order of magnitude higher. Therefore, it is still optimal to use ointments, which are different depending on their action:
- anti-inflammatory with non-steroidal component;
- anti-inflammatory with a steroid component;
- antimicrobial;
- hemostatic;
- with anticoagulant properties;
- analgesics;
- venotonic.
Most ointments have a combined composition and a more versatile effect, which is always better. But the choice is still up to the doctor: he will prescribe the drug taking into account all the nuances, such as contraindications, application features, etc.
Vishnevsky ointment
A well-known balsamic liniment, better known as Vishnevsky ointment. The scope of application of the ointment is quite large: abscesses, furuncles, lymphadenitis, burn wounds, frostbite, trophic ulcers and bedsores, anal fissures and external hemorrhoids.
How to use ointment to treat hemorrhoids? Two or three times a day, apply a thin layer of the product to the inflamed area, or apply a bandage of five layers of gauze soaked in ointment. Continue treatment until the symptoms of the disease subside completely: this period is usually 1-3 weeks.
Longer treatment may be accompanied by side effects in the form of allergic reactions (redness, itching, rash).
Treatment of external hemorrhoids with heparin ointment
The product, the effectiveness of which is based on the properties of heparin and benzocaine, is often used to treat patients with varicose veins. This ointment is a direct-acting anticoagulant, has anti-inflammatory and local anesthetic activity, inhibits the formation of new and accelerates the resorption of existing blood clots.
Heparin ointment is successfully used for thrombophlebitis, trophic ulcers, mastitis, lymphangitis, as well as for external hemorrhoids, postpartum inflammation of the hemorrhoidal veins.
The product is used locally. The ointment can be applied directly to the hemorrhoidal area, or a napkin or tampon can be treated with it. The frequency of use of the drug is up to three times a day, for 4-14 days. Overdose is considered impossible, but with prolonged use, the risk of developing hypersensitivity reactions increases.
Treatment of external hemorrhoids with tablets
For external hemorrhoids, the doctor may prescribe tablets with the following range of action:
- phlebotonics, venotonics (Detralex, Rutoside, Aescusan) – prevent overstretching of venous vessels, ensure normal functioning of veins;
- non-steroidal anti-inflammatory drugs (Acetylsalicylic acid, Ibuprofen, Voltaren) – reduce signs of inflammation, relieve pain;
- anticoagulants, antiplatelet agents (Dipyridamole, Pentoxifylline) – reduce blood viscosity, prevent blood elements from sticking together and prevent the formation of blood clots;
- laxatives (Duphalac, Guttalax, Mucofalk) - soften feces, improve bowel function, facilitate the process of defecation, thereby serving as a preventative measure against worsening of the disease and exacerbations caused by constipation.
Suppositories for the treatment of external hemorrhoids
Suppositories are one of the most popular methods of treating hemorrhoids. But with external manifestations of the disease, doctors still recommend using ointments. The fact is that suppositories act more on internal hemorrhoids, and ointments are applied directly to the inflamed area in the anal area.
In addition, suppositories are used only after bowel movement: after defecation, you need to rinse the anus area with running water, pat dry with a napkin and insert the suppository.
The duration of treatment and frequency of use of suppositories is determined by the doctor individually. In most cases, such products should be stored in the refrigerator.
In case of external hemorrhoids, the suppository should not be pushed deeply: since the inflammatory process is external, the medicinal components should act not deep in the rectum, but directly behind the anus. After the procedure, it is advisable to refrain from going to the toilet for 60 minutes if possible.
If there is no relief within a few days from the start of treatment, or even on the contrary, the condition worsens, then you should definitely visit a doctor to adjust the prescribed treatment.
Antibiotics for external hemorrhoids
External hemorrhoids are often accompanied by the formation of blood clots, the appearance of cracks and ulcers in the perianal area. Such pathological changes can give rise to the development of necrosis, as well as lead to infection of the wounds. To avoid dangerous consequences, doctors often resort to antibiotics. The purpose of their use is to prevent the development of purulent complications.
Antibiotics are prescribed at the first signs of an active inflammatory process. This usually corresponds to stage II or III of hemorrhoids. Such signs include:
- severe swelling in the anal area;
- the appearance of purulent discharge;
- severe redness and pain that bothers even when at rest;
- bleeding from the anus;
- presence of blood in the stool.
A purulent process in external hemorrhoids is a strict reason for mandatory antibiotic therapy, which usually lasts about ten days. Drugs for internal use are not always used: antibiotics can be present in ointments, rectal suppositories, and also administered as injections.
The most popular antibiotic ointment that can be used for external hemorrhoids is Levomekol. Of the suppositories, Proctosedyl is most often chosen, which contains an antibacterial component with a wide range of activity. Among the tablets and injection solutions, the doctor may recommend Tetracycline or Levomycetin, Doxycycline, Gentamicin. Injections are used only in the most advanced cases, if there is severe inflammation with a high temperature.
There are a number of medications, particularly local ones, that are prescribed to treat various forms of hemorrhoids, from their initial stage to chronic recurrent disease. Preparations for external hemorrhoids can be different - these are ointments, suppositories, tablets, etc. Among them, the most commonly prescribed are such medications as Troxerutin, Gepatrombin, heparin ointment. No less widely used are preparations with antioxidant properties that improve energy and intracellular metabolic processes. As practice shows, in about 65-75% of cases it is possible to achieve stable remission, especially in the early stages of hemorrhoid development. However, success in treatment is likely if drug therapy is combined with lifestyle changes, elimination of bad habits, and normalization of nutrition.
