Infesol is an infusion fluid that contains amino acids with electrolytes, which are physiological elements or metabolic analogues.

Amino acids are extremely important in the process of the formation of proteins within the body. Xylitol has a very high energy potential, because during the processes of intrahepatic carbohydrate metabolism it splits, forming the metabolic components that are required to perform glycolysis with gluconeogenesis.

Indications Infuse

It is used as a parenteral nutrition substance during the following disorders:

  • gopoproteinemia;
  • loss of fluid having a different etiology (poisoning, intestinal blockage, burn disease, damage to the digestive organs with a disorder of protein absorption, etc.);
  • temporary inability to eat through the mouth (during operations in the stomach or esophagus, etc.);
  • replenishment of the protein deficiency arising in connection with the increased need for protein production, a strong loss of proteins or a breakdown of protein metabolism during digestion, absorption processes or excretion;
  • improvement of exchange and reparative processes after performing heavy operations.

Release form

The release of the drug is in the form of an infusion fluid:

  • as Infezol 40, in bottles with a capacity of 0.1, 0.25 or 0.5 l; in a box - 10 such flakonchik;
  • as Infezol 100, in bottles with a volume of 0.1 or 0.25 l; 10 flakonchikov inside packs.

Pharmacodynamics

Infusion agents, which include amino acids, are used as substances for parenteral nutritional procedures. In combination with liquid, electrolytes and energy carriers, these drugs help to improve the general condition, support metabolism and reduce the risk of weight loss under various critical conditions.

If there is a shortage of exogenous production of amino acids (due to illness), there is a significant deterioration in the amino acid plasma set, which is manifested in both percentages and in the absolute level of individual amino acids.

Pharmacokinetics

The amino acids contained in the composition of drugs after ingestion into the body are fully broken down to the formation of proteins. Excess substances do not accumulate, which distinguishes the substance from fatty acids with glucose.

A small amount of amino acids (less than 5%) in an unchanged state is eliminated from the body.

Amino acid molecules are degraded by deamination of the α-amino group; it is subsequently converted to urea and in the indicated state is excreted through the kidneys.

The carbon skeleton, which remains after deamination, takes part in the citric acid cycle, when it is metabolized and then transformed into pyruvate, an intermediate element or acetyl CoA.

Dosing and administration

The drug is administered through an IV drip. The dosage regimen is determined by the clinical picture, the body's need for obtaining fluid and amino acids with electrolytes, and in addition to the weight of the patient and the level of catabolism.

In connection with the predominance of catabolic processes, the drug should be used according to the scheme of the upper range. The infusion procedure is performed at low speed, while during the first 60 minutes of application the rate of fluid injection is adjusted to the required performance.

Infesol 40

Adults are administered intravenously in a dose of 25 ml / kg per day (contains 0.6-1 g of amino acids) through an IV. In the case of catabolic states, it is required to use up to 50 ml / kg of a substance per day (it contains 1.3-2 g of amino acids).

For 60 minutes you can enter no more than 0.1 g / kg of amino acids - this corresponds to a rate of 0.8 drops / kg per minute (amino acids in the amount of 2.5 ml / kg and xylitol in a portion of 0.125 g / kg). Thus, with a weight of 70 kg and the introduction of 7 g of amino acids in 60 minutes, the dosage is 175 ml; xylitol index equals 8.75 g; you need to enter 60 drops of medicinal fluid.

If the patient needs a little more fluid and calories, the volume of infusion fluid is supplemented by the simultaneous or alternate use of parenteral electrolyte and glucose fluids. If it is possible to partially feed through the mouth, the part of the medication used is reduced in accordance with the volume of the liquid, as well as the calories injected. The drug should be used until there is a need in it - until the patient becomes possible to transfer to enteral nutrition or ingestion. 

Infuzol 100

Adults use a daily dose of 1-2 g / kg of amino acids (corresponding to 10-20 ml / kg); for a weight of 70 kg, this equals 0.7-1.4 liters of liquid or 70-140 g of amino acids.

During the day, you can enter no more than 20 ml / kg (2 g / kg of amino acids). For a person weighing 70 kg a portion will be 1.4 liters of medication (140 g of amino acids).

For 1 hour, you can enter the drug at a speed of no more than 1 ml / kg - this corresponds to 0.1 g / kg of amino acids. People weighing 70 kg - 70 ml of the substance for 60 minutes (7 g of amino acids).

If the above maximum portion is used, it is prohibited to exceed the daily dosage limit of 2 g / kg and the rate of use of the drug equal to 0.1 g / kg. During parenteral nutrition in an adult, the total volume of fluid used per day should be no more than 40 ml / kg.

The use of medication in pediatrics.

The instructions below are approximate; the scheme of therapy and dosage are selected individually, taking into account the intensity of the pathology and the age of the child.

Age subgroup 2-5 years: 15 ml / kg of medication is administered per day.

Age category 5-14 years: 10 ml / kg of the drug is applied per day.

It is required to enter Infezol with a speed no more than 1 ml / kg in 1 hour (corresponds to 0.1 g / kg in an hour).

Infesol 100 can be used exclusively intravenously via an IV drip. In the case of a low speed of the infusion procedure, the elements of the medicinal product are utilized much more actively than in the case of a fast session speed. The drug is used until the moment when it will be possible to make a full transition to enteral nutrition or oral administration.

Use Infuse during pregnancy

Clinical testing of Infezol with the participation of pregnant or lactating women has not been performed.

Contraindications

Among the relative contraindications of the drug:

  • gingivitis; 
  • hyperhidria;

  • gonadotraemia;
  • methanol poisoning;
  • severe intolerance associated with Na disulfide;
  • acidosis.

Among the absolute contraindications:

  • tissue hypoxia - poor oxygen supply of tissues with cells;
  • shock and other situations in which there is unstable blood flow and danger to life.

Side effects Infuse

When using the medication alone, nausea with chills can occur, and with it vomiting. In the case of an excessively high rate of infusion, phlebitis may appear or hyperkalemia or ammonia may develop.

In the composition of Infesol there is Na disulfide, due to which vomiting, hyperergic manifestations, diarrhea, shortness of breath, shock and consciousness disorder (especially in people with asthma) have been observed sporadically. In addition, individuals with asthma may experience bouts of this disease.

The elements produced by the interaction of Na disulfide and tryptophan often lead to an increase in the indices of intrahepatic enzymes and bilirubin, which are determined by a biochemical study of blood serum.

Overdose

Drug poisoning often occurs when the wrong amount of the substance is chosen or the parenteral use rate is too high. In such situations, there is nausea with chills, and in addition to vomiting, intolerance symptoms and a significant loss of amino acids by the kidneys. A significant excess of the optimal portion can lead to amino acid poisoning, hyperhydria and electrolyte imbalance. In individuals with acidosis and kidney failure in case of drug poisoning, the likelihood of hyperkalemia is extremely high. A serum potassium value of 6.5 mmol / l is dangerous to life.

Among the signs of hyperkalemia are sensitivity lesions affecting muscle weakness and problems with the work of the heart (disturbances of conduction processes, sinus bradycardia, blockade of the bundle of His and arrhythmias). There are changes in ECG readings: a large QRS complex, reaching a point of a T-shaped prong and a blockade affecting the bundle of His. It should be borne in mind that in persons who regularly apply SG, changes affecting the T-prong are erased.

To prevent amino acid poisoning, you need to quickly reduce the rate of injection of drug fluids, or even completely cancel the infusion and apply substances that correct the electrolyte balance. In case of very severe overdose, osmotic diuresis should be performed, and at danger to life - hemodialysis sessions.

Interactions with other drugs

The disulfide contained in the medication Na can inactivate the effects of thiamine.

Since the drug is extremely sensitive about bacterial contamination, it cannot be mixed with other parenteral drugs.
