Gepadif is a hepatoprotective drug.

Indications Hepadifa

It is used in combination treatment regimens for the following disorders:

  • hepatitis in acute or chronic stage;
  • alcoholic liver disease;
  • liver cirrhosis;
  • poisoning associated with prolonged use of antitumor or antituberculosis drugs;
  • fatty liver disease;
  • exogenous intoxications.

Release form

The medicinal component is released in the form of a lyophilisate for the preparation of infusion fluid; 5 or 10 vials inside the package.

In addition, the drug is sold in gelatin capsules, in quantities of 10, 30, 50 or 100 pieces per pack.

Pharmacodynamics

Gepadif is a complex medicine that acts due to the activity of its constituent elements. It stimulates biosynthetic and fat metabolic processes, prevents the development of hepatocyte necrosis, stabilizes the proliferation of hepatocytes and restores stable liver function and the production of specific enzymes.

Adenosine is a component of nucleic acids with coenzymes, as well as almost all cells located inside the body; it is an endogenous nucleoside that helps regulate hematopoietic processes. The component reduces the tone of the smooth muscles of cell membranes inside the vascular bed and stabilizes the processes of supplying hepatocytes with oxygen.

Vitamins from the B subcategory (such as pyridoxine with riboflavin and cyanocobalamin) are participants in the redox reaction and the binding of the most important amino acids (tryptophan with methionine, as well as cysteine, etc.), and in addition, they stabilize the metabolism of fats with proteins and carbohydrates. At the same time, vitamins help stimulate erythropoiesis.

The antitoxic fraction, which contains replaceable and, in addition, essential amino acids, provides the medication with antioxidant and detoxifying activity.

Carnitine is a participant in the processes of fatty acid breakdown, helps regulate fat metabolism and improves the processes of food absorption. The substance stimulates bile production, promotes food digestion and facilitates the absorption of carbohydrates required to maintain stable microflora in the small intestine. At the same time, the drug increases cellular sensitivity to insulin and helps excrete it from the body.

Pharmacokinetics

After oral administration of capsules, the complex of active elements of the drug is released inside the duodenum. Affecting the entire length of the small intestine (mostly the jejunum, as well as the ileum), absorption of carnitine with vitamins, as well as the antitoxic fraction and adenine, is carried out.

Medicines are distributed inside the intestines and kidneys with the liver. The highest riboflavin levels are recorded inside the kidneys, myocardial cells, and liver. Metabolic processes involving pyridoxine occur in the liver, and distribution is carried out inside the liver, muscle tissues, and the nervous system.

Excretion of metabolic products occurs mainly through the kidneys.

After intravenous administration of the drug through a dropper, its elements are distributed within the kidneys with the liver, brain, muscle tissues and cardiomyocytes. Metabolic processes also occur in the liver, and the excretion of metabolic products is through the kidneys.

Dosing and administration

The capsules must be taken orally, swallowed whole and washed down with plain water. The duration of the treatment cycle and the dosage regimen are selected by the attending physician.

An adult should take 2 capsules of the drug 2-3 times a day. A child over 7 years old is prescribed 3 capsules per day, in 3 doses.

If you have problems swallowing the capsule, you can dissolve its contents in juice or plain water. Do not take the capsule with milk or dissolve it in milk.

If for some reason the patient cannot take Gepadif capsules, he is prescribed intravenous administration of the medication.

Use of intravenous injections of medication.

Adults are required to administer 8.625 mg/kg per day (usually this corresponds to the volume of 1 bottle). This dose should be administered once per day.

Before performing the infusion, the powder must be dissolved in 5% glucose (0.5 l).

Use Hepadifa during pregnancy

No adverse effects have been reported when used in pregnant or lactating women, but if the drug is prescribed to such patients, the risk and benefit to the woman and the child or fetus must be carefully assessed.

Contraindications

Main contraindications:

  • erythrocytosis, erythema, as well as thromboembolism or nephrolithiasis;
  • presence of allergy to elements of the medication;
  • the presence of an ulcer in the gastrointestinal tract (especially lesions in the acute phase).

Side effects Hepadifa

Hepadif is often tolerated without complications. However, sometimes there may be some negative symptoms: nausea, constipation, headaches, diarrhea, indigestion, vomiting, and in addition, angioedema, urticaria, yellowing of urine and rash.

Overdose

Drug intoxication causes vomiting, polyneuropathy, diarrhea, and potentiation of the strength and frequency of negative symptoms.

To eliminate the disorders, gastric lavage is performed, activated charcoal is administered together with saline laxatives, and symptomatic procedures are also performed.

Interactions with other drugs

Riboflavin is prohibited to be combined with streptomycin, because this vitamin weakens the properties of antibacterial drugs (such as doxycycline with tetracycline, lincomycin and oxytetracycline).

The metabolic processes of Gepadif, especially its effect on the heart, are weakened under the influence of amitriptyline, tricyclics and imipramine.

Pyridoxine reduces the effectiveness of levodopa and the incidence of adverse effects from the use of isoniazid or other anti-tuberculosis drugs.

The degree of absorption of cyanocobalamin is affected by cimetidine, calcium supplements, alcoholic beverages and PAS.

Storage conditions

Gepadif should be stored in a place closed to children. Temperature indicators are less than 25°C.

Shelf life

Gepadif can be used within 5 years from the date of manufacture of the pharmaceutical product.

Application for children

Should not be prescribed in pediatrics (under 7 years of age).

Analogues of the drug are the drugs Octamin Plus, Heptral, Tavamin, Carnitine with Carnitene, and in addition to this Heptor, Stimol, Alimba and Agvantar with Elkar. The list also includes Carnivit, Steatel, Le Carnita and Carnigel.

Reviews

Gepadif has a wide therapeutic range, it is often used in combination with other drugs to support liver function. Reviews regarding its effectiveness are quite contradictory, because taking into account the type of disease, the degree of severity of the existing lesion, as well as joint treatment with other drugs, the drug can lead to the development of completely different effects.
