Gemsineral-td is an antianemic drug containing various medicinal elements.
Indications Gemsinerala td
It is used to prevent the development or to treat anemia of folate or iron deficiency nature. Such anemia occurs with the following disorders and conditions:
- alimentary iron deficiency;
- lactation or pregnancy;
- ancylostomiasis;
- bleeding that is chronic (menorrhagia or hemorrhoidal bleeding).
The medication is also used in cases of increased need for medicinal components, arising from burn diseases, hypochlorhydria, malabsorption syndrome, gluten enteropathy, hemodialysis, and also after surgical procedures.
Gemsineral-TD is prescribed to people who are rapidly losing weight, as well as to prevent the development of anemia or its treatment in elderly people who are on special diets.
Release form
The substance is released in capsules, packed in blister plates of 10 pieces. Inside the box there are 3 such plates.
Pharmacodynamics
Gemsineral-td is a complex remedy that helps treat or prevent the development of anemia of folate or iron deficiency. It contains cyanocobalamin with iron fumarate, and in addition, vitamin B9.
Ferrous fumarate is an organic component containing a large amount of elemental iron (the indicator reaches 66.6 mg). It promotes rapid development of medicinal action in case of iron deficiency anemia.
Cyanocobalamin is required for the formation of red blood cells, and also for DNA binding processes.
Vitamin B9 is transformed inside the body, acquiring the form of folinic acid, and becomes a participant in the processes of nucleotide binding, together with cyanocobalamin helping the maturation of red blood cells. This acid is needed to strengthen the immunity of cells.
The specific technology of moving the active elements of the drug into the duodenum prevents the development of an irritating effect on the gastric mucosa. Iron fumarate is released inside those parts of the intestine where its absorption is highest (in the upper part of the small intestine and in the duodenum). This occurs due to the slow dissolution of the pellets.
Pharmacokinetics
Elements of Gemsineral-td are quickly absorbed inside the intestine. Increased iron levels in the blood serum are registered already after 1 hour after the use of the medication.
The technology of transportation and subsequent slow release of active elements allows to avoid sharp fluctuations in blood iron levels, and its medicinal effect is maintained for the next 10-12 hours.
Dosing and administration
During treatment of anemia, a person needs to take 1 capsule 2-3 times a day for at least 3-4 weeks (the exact duration depends on the severity of the disease).
For prevention, the medication is taken 1 capsule per day.
Use Gemsinerala td during pregnancy
During lactation or pregnancy, the medication can be prescribed only if iron deficiency has been diagnosed, after consultation with a doctor and under his supervision.
Contraindications
Main contraindications:
- the presence of high sensitivity to medicinal substances;
- excess iron (the presence of hemosiderosis or chromatosis) or a predisposition to its development;
- other types of anemia that do not arise as a result of iron deficiency (hemolytic, sideroachrestic, aplastic, as well as iron-refractory and caused by lead intoxication; in addition, hemoglobinopathies, thalassemia, etc.);
- pernicious form of anemia;
- frequent blood transfusion procedures;
- erythrocytosis or erythremia, acute forms of thromboembolism;
- tumors, except in cases associated with megaloblastic anemia;
- liver cirrhosis;
- late porphyria cutanea;
- acute inflammatory intestinal diseases;
- aggravated ulcers in the gastrointestinal tract;
- stenosis affecting the esophagus or other obstructive diseases developing in the digestive tract;
- intestinal diverticulum or intestinal obstruction;
- combined use with parenteral types of iron.
Side effects Gemsinerala td
Taking capsules may cause some side effects:
- digestive system disorders: gastralgia, a feeling of fullness or pain in the epigastric region or in the abdomen, vomiting, bloating, loss of appetite, constipation or diarrhea, nausea, black stool, and along with this, darkening of tooth enamel;
- lesions affecting the subcutaneous tissue and epidermis: acne, itching, rash on the epidermis, redness, urticaria and bullous rashes;
- immune dysfunction: bronchospasm or anaphylaxis;
- problems with the functioning of the nervous system: dizziness, feeling of nervous excitement and headaches;
- disorders in the heart area: pain or tachycardia;
- Others: feeling of general weakness, hyperhidrosis, hot flashes and hyperthermia.
With prolonged unjustified use of drugs, hemosiderosis may develop.
Overdose
Intoxication may provoke an increase in the symptoms of side effects. With uncontrolled long-term use of Gemsineral-td, hemochromatosis may develop.
Exceeding the required doses of the drug for treatment may cause overdose symptoms.
In acute iron poisoning, vomiting with nausea occurs, and in severe forms of intoxication, collapse with a fatal outcome. The lethal dose of elemental iron is 180-300 mg/kg of the substance. It should be understood that for some patients, a dose of iron equal to 30 mg/kg may be toxic.
In acute iron intoxication, signs of the disorder may develop within 10-60 minutes or after several hours. Abdominal or epigastric pain, nausea, diarrhea with green stools, which later acquire a tarry hue, occur. Vomiting (sometimes with blood), melena, a feeling of weakness or drowsiness, paleness of the epidermis, cyanosis with acrocyanosis and sticky cold sweat also appear. In addition, lethargy, weak pulse, decreased blood pressure, a feeling of confusion or fatigue, palpitations, comatose or shock state, hyperthermia and signs of hyperventilation are observed, as well as convulsions, paresthesia and necrosis in the area of the gastrointestinal mucosa.
Remission is often observed after approximately 4-6 hours. Then, after 12-48 hours, severe shock may occur, with the development of periodic breathing, coagulopathy, oliguria, and toxic liver failure.
Due to the possible occurrence of toxic symptoms of poisoning, the patient should be given the required assistance immediately. By slowing down the process of iron release, strong absorption can be prevented, which will increase the time period for countermeasures. In case of accidental consumption of a large number of capsules, the victim should drink milk and eat raw eggs - this will form insoluble iron compounds in the gastrointestinal tract and facilitate the excretion of iron from the body. The capsules can also be removed from the stomach by vomiting.
Specific medical procedures.
The patient's vomit should be checked for capsules. If the number of removed capsules is small, gastric lavage is required, using a 0.9% NaCl solution or a 1% aqueous solution of soda ash, as well as prescribing a laxative to the patient. In addition, an X-ray of the peritoneum is performed to determine the number of capsules remaining in the stomach. If the desired result is not achieved after using all of the above methods, a surgical procedure may be required.
The most appropriate way to assess the severity of the condition is to examine serum iron levels and the FSBS level. If the serum iron levels exceed the FSBS level, general intoxication can be suspected.
Deferoxamine may be needed for treatment. Chelation therapy with deferoxamine is used in the following situations:
- when consuming a potentially lethal dose (180-300+ mg/kg);
- serum iron values are greater than 400-500 mg/dL;
- the serum iron level is higher than the ISR level, or the patient shows signs of severe iron poisoning such as shock or coma.
In case of acute intoxication, to synthesize iron not absorbed from the gastrointestinal tract, 5-10 g of deferoxamine is taken orally (10-20 ampoules should be dissolved in ordinary water). To eliminate iron that has already been absorbed, the substance is administered intramuscularly (12 g) at intervals of 3-12 hours. In severe disorders, against the background of which a state of shock is observed, the victims are administered 1 g of the drug intravenously through a dropper and symptomatic measures are carried out.
In young children, the risk of acute iron intoxication is extremely high. Consumption of 1 g of the drug is considered life-threatening.
If necessary, procedures aimed at treating acidosis and shock are performed.
People with anuria or oliguria undergo hemodialysis or peritoneal dialysis sessions.
Interactions with other drugs
The absorption of iron salts is mutually impaired and may be weakened when used in combination with tetracyclines.
The intensity of iron absorption in the gastrointestinal tract may decrease when Gemsineral-TD is combined with Almagel, calcium, cimetidine, phosphates, and in addition with aluminum, magnesium salts, organic acids and drugs containing pancreatic enzymes.
Combination of the substance with carbamazepine, hormonal contraception, phenobarbital, as well as sodium valproate, pyrimethamine, trimethoprim and sulfasalazine, as well as with folic acid antagonists and triamterene leads to a decrease in the biological availability of folic acid.
Storage conditions
Gemsineral-td should be kept in a place closed to small children. Temperature values are within 25°C.
Shelf life
Gemsineral-TD is allowed to be used for 24 months from the date of manufacture of the therapeutic agent.
Application for children
Gemsineral-TD is prohibited for use in pediatrics – children under 12 years of age.

Analogues of the drug are the medications Ranferon-12 and Ferro-Folgamma.
