Fibrinolysin

Fibrinolysin has an anticoagulant effect on human blood.
The drug prevents blood clotting processes. It has the ability to split fibrin protein threads - it has an effect similar to the action of a proteolytic enzyme. Fibrinolysin is most effective when used at the initial stage - immediately upon the occurrence of fibrin clots and thrombus formation. Then, the therapeutic effectiveness weakens in accordance with the duration of the thrombus.
Indications Fibrinolysin
The product in vials is used for the following disorders:
- thromboembolism affecting the pulmonary or peripheral arteries;
- thrombophlebitis in the active phase;
- thromboembolism affecting cerebral vessels;
- exacerbated thrombophlebitis of a chronic nature;
- myocardial infarction.
Use of drugs in ampoules:
- hemorrhages in the visual organs that are of traumatic origin (at least 4 days after the time of intraocular injury);
- hemorrhages affecting the vitreous body, and along with it the retina or the anterior zone of the eye chamber;
- affecting the central vessels located inside the eye (arteries or veins), along with their branches, thrombosis.

Pharmacodynamics
Fibrinolysin is a plasma element of human blood, produced with the help of trypsin, which through enzymes activates intraplasmic profibrinolysin.
The therapeutic effect of fibrinolysin is determined by a biological method of establishing how it lyses a normal fresh fibrin clot at a temperature of 37°C for half an hour and is designated in U.
Dosing and administration
Using medication from vials.
In case of myocardial infarction or cerebral vascular thrombosis, the drug should be administered within the initial 6 hours. In case of thrombosis affecting peripheral arteries, infusion is required within the initial 12 hours. In case of thrombosis associated with peripheral veins, the injection should be given within 5-7 days from the moment of thrombosis detection.
The drug is used in the form of infusions. Before the infusion procedure, the powder from the vial is dissolved in 9% NaCl in a proportion of 100-160 U/ml. Then heparin is added to the resulting solution in a ratio of heparin 10,000 U to fibrinolysin 20,000 U. At first, the infusion is performed at a rate of 10-12 drops/minute. The rate can also be increased to 15-20 drops/minute (if well tolerated). The total daily dose ranges from 20,000 to 40,000 U, with a minimum procedure duration of 3 hours. It should be taken into account that Fibrinolysin does not contain a stabilizer, and therefore the finished liquid quickly loses its medicinal activity.
After completion of the drug infusion together with heparin, it is necessary to carry out intramuscular or intravenous administration of only heparin in a portion of 40,000-60,000 U/day for 2-3 days. In this case, its dose is gradually reduced, and then a transition to the administration of indirect anticoagulants occurs.
Administration of medication in ampoules.
A portion of the medicine, comprising 300 U, is used only subconjunctivally. Before the procedure, the substance from the 1st ampoule is dissolved in the injection liquid (0.5 ml). The prepared substance is injected under the conjunctiva in the area of the sclera or the transitional fold located below (before this, it is also necessary to inject 0.5% dicaine into the area of the conjunctival sac - as anesthesia).
Repeated injections must be performed after 1-2 days. The total number of injections varies between 3-10, taking into account the existing indications.
Contraindications
Among the contraindications for the product in bottles:
- hemorrhagic diathesis;
- acute phase of hepatitis;
- open wounds;
- fibrinogenopenia;
- increased sensitivity;
- bleeding;
- liver cirrhosis;
- radiation sickness;
- ulcer affecting the gastrointestinal tract;
- nephritis;
- pulmonary tuberculosis in the active phase;
- history of anaphylactic reactions;
- recently undergone (within 10 days) biopsy, surgery, puncture in the area of large vessels or trauma;
- very high blood pressure levels accompanying brain damage.
Contraindications for drugs in ampoules:
- severe intolerance to the drug;
- history of anaphylactic symptoms;
- contraindications regarding dicaine, because it is administered before using Fibrinolysin.
Side effects Fibrinolysin
Side effects of the medicine in vials include:
- allergy symptoms (including fever, facial hyperemia and urticaria);
- decrease in blood pressure (it is necessary to monitor blood pressure when using drugs);
- pain affecting the abdominal area or sternum;
- bleeding;
- pain or other changes in the injection area.
When using ampoules:
- signs of allergy (urticaria, facial hyperemia or local swelling);
- pain or changes at the injection site.
In case of such disorders, it is necessary to reduce the injection rate, and if severe manifestations develop, the administration is stopped. To eliminate negative symptoms, antihistamines or cardiovascular drugs and promedol are used (selected taking into account the direction of the disorders).

The drug is not prescribed to children.
