Pharmacodynamics

The action of the drug is due to its components - chlorhexidine and metronidazole (they have antibacterial properties).

Metronidazole is a nitroimidazole derivative (it has antiprotozoal and antibacterial action). It is active against protozoan anaerobes, as well as periodontitis bacteria: Fusobacterium fusiformis, Porphyromonas gingivalis, Prevotella intermedia, Prevotella denticola, Borrelia vincentii, Volinella recta, Treponema, Eikenella corrodens, Bacteroides melaninogenicus, and Selenomonas sp. The minimum concentration of the substance that allows suppressing about 50% of strains is less than 1 μg/ml for such anaerobes that cause periodontitis: Porphyromonas gingivalis, Prevotella intermedia, Volinella recta and Fusobacterium nucleatum.

Chlorhexidine is an antiseptic substance with bactericidal properties. It affects most gram-positive and gram-negative microbes (vegetative forms), as well as lipophilic viruses, dermatophytes and yeasts.

Pharmacokinetics

After local use, the concentration of metronidazole in the gum area exceeds similar values after internal administration of the drug. The systemic absorption of the substance when using the gel form of the drug will be much lower than similar figures after oral administration. Metronidazole and its decay products are mainly excreted through the kidneys (reduced renal function does not affect the pharmacokinetics of a single dose of the substance).

Due to the ingestion of excess chlorhexidine remaining after its local application, approximately 1% of the drug dosage that entered the stomach is absorbed from the gastrointestinal tract. Chlorhexidine accumulates in the body, and the metabolism of the substance is minimal.

Dosing and administration

Used locally in dental procedures.

For children aged 6 years and over, as well as adults, when eliminating gingivitis, it is necessary to apply a thin layer of the drug to the gums twice a day (for this, you can use a cotton swab or finger). The medicine does not need to be washed off. The duration of the therapeutic course is on average 7-10 days. After the procedure of applying the gel, it is forbidden to drink and eat for about half an hour.

When treating periodontitis, the periodontal pockets should be treated with the substance, and it should also be applied to the gums (this should be done after the procedure for removing plaque on the teeth). The exposure time is 30 minutes. The number of procedures depends on the severity of the pathology. The patient can do subsequent gel applications himself: the medicine should be applied to the gums twice a day for 7-10 days.

When treating the aphthous form of stomatitis, the inflamed part of the oral mucosa also needs to be treated with the medicine twice a day for the next 7-10 days.

The same number of procedures and course duration are required for prevention against possible exacerbations of chronic forms of periodontitis or gingivitis. It is allowed to conduct 2-3 such preventive courses per year.

In the prevention of post-extraction alveolar pain, the gel is applied to the alveolus formed after the tooth was extracted. Then, the medicine is administered on an outpatient basis 2-3 times a day for 7-10 days.

Use Dentagel during pregnancy

It is prohibited to use the medicine in the 1st trimester.

If it is necessary to prescribe Denta gel during breastfeeding, it is necessary to stop breastfeeding for the duration of treatment.

Contraindications

Among the contraindications:

  • children under 6 years of age;
  • hypersensitivity to chlorhexidine and metronidazole, as well as derivatives of nitroimidazole and other components of the drug.

Side effects Dentagel

As a result of using the drug, allergic reactions (hives, and along with this itching and rashes on the skin) and headaches may occur.

Overdose

Intentional or accidental oral administration of the gel in high dosages can cause increased side effects. As a rule, they are caused by metronidazole, since chlorhexidine is almost not absorbed from the gastrointestinal tract. Symptoms include nausea with vomiting, as well as dizziness. In case of severe overdose, convulsions and paresthesias may occur.

To eliminate the disorders, gastric lavage is required, and in addition, if necessary, therapy aimed at eliminating the symptoms of overdose.

Storage conditions

The medicine should be kept out of the reach of small children. The temperature should not exceed 25°C.

Shelf life

Denta gel is permitted to be used for 3 years from the date of release of the drug.
