Dibenzamil is a peripheral vasodilator drug.
Indications Dbezimila
It is used for pheochromocytoma : to control increasing blood pressure indicators during the preoperative period, during surgery, and in addition, for persons with inoperable lesions.
Release form
The release of a therapeutic agent is in the form of an infusion fluid, in 1 ml ampoules. Inside a pack of 5 such ampoules.
Pharmacodynamics
The drug acts as an indiscriminate adrenergic blocker, has an effect through α-adrenoreceptors, with a prolonged peripheral vasodilating effect. After in / in the injection develops "chemical sympathectomy."
Has a vasodilating effect, due to which there is an increase in blood circulation inside the epidermal vessels, mucous membranes and organs located inside the peritoneum, which causes a significant decrease in blood pressure values. Does not affect the activity of parasympathetic NA.
Pharmacokinetics
After the use of one intravenous dose (1 mg / kg), the achievement of the Cmax indicator is noted after 60 minutes; it persists over a 2-3 day period. After i / v injection, the half-life of the drug is 24 hours.
Dibenzimil exchange processes are carried out inside the liver. The substance accumulates inside the body. In the case of daily use of the drug, its therapeutic effect lasts for another 7 days after the last dose has been administered.
The duration of the effect of phenoxybenzamine is determined by the rate of binding inside the body of new α-adrenoreceptors after their irreversible blockade occurs, caused by the use of phenoxybenzamine.
Dosing and administration
At the preoperative stage, as well as during operations in people with pheochromocytoma, the drug is administered via an IV drip in portions of 1 mg / kg per day for at least 2 hours. The drug must be pre-diluted in 0.9% NaCl (0.2 L).
Therapy with the use of dissolved phenoxybenzamine should be continued until an adequate blood pressure index is reached.
Subsequent treatment requires the administration of Dibenzimil tablet form, in 10 mg portions, 2 times a day, with a gradual increase in dosage until the desired result is obtained. The maximum allowable size of a daily serving is 1-2 mg / kg (this dosage should be divided into 2-3 uses).
If the drug must be used for a long time to eliminate negative symptoms (tachycardia), β-blockers should be used.

Use Dbezimila during pregnancy
Appropriate tests involving animals were not performed, which is why Dibenzimil is prescribed to pregnant women only in situations where the likely benefit to the woman is more expected than the possibility of complications in the fetus.
If the medication needs to be used during lactation, breastfeeding needs to be canceled - because there is no information as to whether phenxybenzamine is excreted with mother's milk.
Contraindications
It is contraindicated for use in cases of severe intolerance with respect to therapeutic elements, and in addition, if there is a likelihood of a decrease in blood pressure values.
Side effects Dbezimila
There is evidence that animals after prolonged use of phenoxybenzamine (longer than 52 weeks) increase the likelihood of malignant tumors.
Nausea, miosis, dizziness, tachycardia, orthostatic collapse, swelling of the nasal mucosa and suppression of ejaculation are noted among the lesions of the NS. More rarely observed dryness of mucous membranes, erosion in the gastrointestinal tract, a feeling of increased fatigue or sleepiness.

Overdose
Poisoning can trigger the development of tachycardia, allergic symptoms, dizziness, orthostatic collapse, vomiting and feelings of weakness.
If signs of intoxication develop, the use of drugs should be discontinued. Standard procedures will be ineffective. You should not use adrenaline, because the blocking of α-adrenoreceptors occurs for a long time, which can cause manifestations of "canceled adrenalin influence" in the form of a subsequent decrease in blood pressure.
Also, the patient should be laid horizontally, raising his legs. With light overdoses, such a measure will be sufficient to normalize hemodynamics. Since the effect of the drug is prolonged, the victim should stay in the indicated position for about 24 hours. The use of bandages in the area of the peritoneum and extremities leads to a decrease in the length of indisposition.
In severe stages of the severity of violations, it is necessary to introduce bitartate levarterenol in / in the way, in portions, which will be enough to stabilize blood pressure values.
Interactions with other drugs
Dibenzimil interacts with drugs that stimulate α-, as well as β-adrenergic receptors (this includes adrenaline), reducing tachycardia and antihypertensive effects.
The drug blocks the development of hypothermia, which occurs due to the use of reserpine.
The drug enhances the effect of alcoholic beverages, therefore it is forbidden to take them during treatment with Dibenzimil.
Combination with antihypertensive drugs can potentiate antihypertensive activity.

Storage conditions
Dibenzimil needs to be kept in the place closed from small children. Temperature marks - no more than 25 ° С.
Shelf life
Dibenzimil can be used within a 24-month period from the time of release of the pharmaceutical agent.
Application for children
You can not prescribe medication in pediatrics.
Analogs
Analogues of the drug are medications Papazol, Halidor with Dibazol, as well as Vineball and Dusofarm.
