Cefamezin is an antibiotic from the cephalosporin group (1st generation). It has a wide range of medicinal activity and bactericidal properties.

Indications Cefamesin

It is used to eliminate pathologies of infectious and inflammatory origin, provoked by the activity of microbes sensitive to cefazolin:

  • diseases affecting the lower and upper respiratory system;
  • pathologies inside the biliary tract and urinary system, as well as in the area of soft tissues, joints with bones, organs inside the small pelvis, and also on the skin;
  • sepsis with endocarditis, mastitis with osteomyelitis, and in addition otitis media and peritonitis;
  • infections that occur after burns, surgical procedures and wounds;
  • gonorrhea or syphilis.

Release form

The product is produced in the form of a lyophilisate used in the manufacture of an injection medical solution, in vials with a capacity of 0.5, 1 or 2 g. There are 10 such vials inside the box.

Pharmacodynamics

The drug is active against:

  • gram-positive microbes: staphylococci (strains that produce and do not produce penicillinase), streptococci (this also includes pneumococci), diphtheria corynebacteria and anthrax bacilli;
  • gram-negative microbes: gonococci, meningococci, salmonella, shigella, klebsiella and E. coli.

In addition, it affects Leptospira spp, as well as Spirochaetoceae.

It does not affect the activity of Pseudomonas aeruginosa, anaerobes, indole-positive strains of Proteus, and Mycobacterium tuberculosis.

Pharmacokinetics

After intravenous injection, stable plasma values of the drug are observed after 3 hours. The active element quickly passes through the placenta, synovium and histohematic barriers. A small part of the substance is excreted with breast milk. The level of the drug in bone tissue is similar to its plasma indicators.

Excretion of the unchanged element occurs mainly by glomerular filtration. With intramuscular administration of 0.5 g, excretion of 56-89% of the drug occurs in 6 hours, and 80-100% - in 24 hours. Peak values of the drug in urine are 1+ g / l, and with intramuscular administration of 1 g of solution, the maximum indicator in urine is 4 g / l.

Dosing and administration

The dosage regimen is determined individually for each person. The severity of the disease, the location of the infectious process, and the sensitivity of the causative bacteria are taken into account.

The medication is administered intramuscularly or intravenously (by jet or drip method). On average, adults require a dose of 1 g per day, with a frequency of administration of 2-4 times a day.

To prevent infection after surgery, it is necessary to administer the drug in a dose of 1 g half an hour before the operation, then another 0.5-1 g during the procedure, and then another 0.5-1 g throughout the next day after the operation (at intervals of 6-8 hours).

No more than 6 g of the drug may be administered per day.

Children are required to be administered 20-40 mg/kg of the substance per day. If a severe degree of infection is observed, the dose may be increased to 100 mg/kg/day. The duration of therapy is 7-10 days.

Use Cefamesin during pregnancy

The use of Cefamezin in a nursing or pregnant woman is permitted only in situations where the possible benefit to her is more likely than the occurrence of complications in the fetus or infant.

Contraindications

Main contraindications: infants under 1 month, as well as hypersensitivity to cephalosporins.

Caution is required when used in individuals with kidney problems.

Side effects Cefamesin

The use of drugs sometimes causes the development of the following side effects:

  • lesions affecting the digestive system: vomiting, nausea or diarrhea may occur. Occasionally, a transient increase in liver transaminase activity is observed;
  • manifestations of allergy: itching, urticaria, fever or eosinophilia may develop. Arthralgia, angioedema or anaphylaxis are observed sporadically;
  • signs caused by chemotherapeutic influence: development of pseudomembranous colitis, as well as candidiasis;
  • disorders of hematopoietic function: occasionally curable leukopenia is observed, as well as thrombocyto- or neutropenia;
  • urinary disorders: functional renal impairment occasionally develops;
  • local symptoms: pain may occur at the site of the intramuscular injection.

Overdose

With parenteral injection in large doses, headaches, paresthesia and dizziness may occur. In case of intoxication with the drug or its accumulation in a person with chronic renal failure, neurotoxic symptoms may develop. In such situations, increased convulsive readiness, vomiting, clonicotonic seizures of a generalized nature, as well as tachycardia are observed.

If the victim shows toxic symptoms, or if there are signs of drug overdose, its excretion from the body can be accelerated using hemodialysis. However, peritoneal dialysis will be ineffective.

Interactions with other drugs

The combination of the drug with loop diuretics causes blockade of tubular excretion of cefazolin (therefore, this combination is not recommended for use).

The active ingredient of Cefamezin can lead to the development of a disulfiram-like effect when combined with ethyl alcohol.

Probenecid inhibits the excretion of cefazolin.
