Binafin has antimycotic activity.

The active ingredient of the drug, terbinafine, demonstrates a wide range of effects against dermatophytes, which cause diseases of the epidermis, hair and nails, as well as against Candida fungi.

The fungicidal effect against dermatophytes and mold fungi develops even at low LS values. The type of effect exerted against yeast-like fungi (fungistatic or fungicidal) is determined by the type of fungus present.

Indications Binafina

The cream is used for candidiasis (caused by the Candida fungus), dermatomycosis and versicolor lichen.

Tablets are prescribed in case of mycoses affecting the scalp, onychomycosis, and candidiasis (in situations where the prevalence or intensity of the process requires the use of tablets). This dosage form is ineffective for versicolor lichen.

Release form

The element is released in tablets of 0.125 and 0.25 g, as well as in the form of a 1% cream inside tubes of 10, 15 or 30 g.

Pharmacodynamics

Terbinafine works by inhibiting the activity of the enzyme squalene epoxidase within the fungal wall, causing squalene to accumulate within the cell, causing it to die.

When taken orally, the fungicidal properties of the drug are formed inside the epidermis, hair and nails.

Pharmacokinetics

With a single use of a 0.25 g dose, the required blood levels of the drug are noted after 2 hours.

It has a high protein binding rate (99%). It penetrates into the dermis, concentrating inside the stratum corneum, as well as nail plates. By the 2nd day of taking, the indicators inside the stratum corneum increase tenfold, and after 12 days - 70 times. It penetrates into the sebaceous glands, forming high indicators inside the hair and their follicles. Stable values of LS inside the tissues are recorded after 10 days.

The half-life is 24-150 days, which is why the drug remains in the nails and epidermis for a long time after its withdrawal.

Metabolic processes are carried out with the help of hemoprotein P450. During biotransformation, metabolic elements are formed that do not have an antimycotic effect. Excretion occurs with urine. There is no data on the accumulation of the medicinal substance.

After local treatment, only 5% of the portion is absorbed, which makes the systemic effect of the cream extremely low.

Dosing and administration

Using cream.

The medication is applied 1-2 times a day, in a thin layer. The cream should be rubbed in a little, while covering the areas adjacent to the affected area.

For dermatomycosis affecting the feet, trunk or shins, therapy is carried out for 7 days.

In case of versicolor lichen or epidermal candidiasis, treatment continues for 14 days.

In case of infections accompanied by diaper rash (between the buttocks, in the groin area, between the fingers or under the mammary glands), after treatment with the drug, these areas are covered with a gauze napkin.

If therapy is not carried out for a sufficient period of time or is irregular, there is a risk of recurrence of infection.

Use of medication tablets.

An adult should consume 0.25 g of the substance once a day.

In case of dermatomycosis in the foot area, therapy continues for 0.5-1.5 months, and in case of infection of the shins or torso – 3-4 weeks.

In case of mycosis affecting the scalp, treatment lasts 1 month.

In case of epidermal candidiasis, the treatment course lasts up to 1 month.

For onychomycosis affecting the hands, the cycle is 1-1.5 months, and for onychomycosis of the feet – 3 months.

If the rate of nail growth is reduced, therapy may be longer – until a healthy nail grows.

In case of renal dysfunction, the patient is prescribed half the usual dosage of Binafin. If liver dysfunction develops during the course of treatment, the therapy is stopped.

Use Binafina during pregnancy

Binafin should not be used during pregnancy.

If there is a need to use the drug during lactation, breastfeeding should be discontinued.

Contraindications

Contraindicated for use in case of severe personal sensitivity to the drug.

Caution is required when used in people with liver disease.

Side effects Binafina

Among the most frequently developing side effects are: nausea, diarrhea, a feeling of fullness in the stomach, dyspepsia, loss of appetite, taste disorders and pain in the abdominal area. In addition, urticaria, myalgia, arthralgia and rashes are observed.

Rarely observed: liver failure or cholestasis, TEN or SSc, as well as hematological disorders (thrombocyto- or neutropenia and agranulocytosis).

After local application of the cream, itching, redness or burning may occur, but they only rarely require discontinuation of therapy. If urticaria develops, the therapy is discontinued.

Overdose

When poisoned by tablets, dizziness, pain in the epigastric region, nausea and headaches occur.

Gastric lavage and enterosorbent administration are performed. In addition, symptomatic treatment measures are carried out.

Interactions with other drugs

Terbinafine weakly inhibits or potentiates the clearance of many drugs whose metabolic processes are carried out with the help of hemoprotein P450. Among such drugs are tolbutamide, cyclosporine, triazolam with terfenadine, and oral contraception.

In addition, the drug inhibits CYP2D6 metabolic processes, which must be taken into account when using drugs that are metabolized with the participation of this enzyme: MAOIs, antidepressants, SSRIs and β-blockers.

The rate of clearance of terbinafine increases with the administration of agents that potentiate the metabolism of hemoprotein P450 (rifampicin), and also slows down with the use of substances that inhibit it (such as cimetidine).

With such combinations, it is necessary to adjust the portion sizes of Binafin.

Storage conditions

Binafin must be stored at temperatures not exceeding 25°C.

Shelf life

Binafin can be used within a 36-month period from the date of manufacture of the medicine.

Application for children

Tablets can be prescribed in pediatrics only to patients over 2 years of age. The cream can be used from the age of 12.

The analogs of the drug are Atifin, Termicon, Lamisil with Terbisil, as well as Terbinafine and Exifin.
