Belogent is a GCS for local use and also an aminoglycoside aminocyclitol.
Indications Belogent
The medicine is indicated for the following disorders:
- atopic, seborrheic, contact dermatitis, as well as dermatitis developing due to allergic reactions;
- eczema or psoriasis;
- dermatoses with complications in the form of secondary infection;
- disseminated neurodermatitis;
- mosquito dermatosis;
- impetigo;
- lichen planus;
- anogenital itching.
Release form
It is produced in the form of an ointment or cream for external use. The volume of the tube with the medicine is 15 or 30 g. One package contains 1 tube.
Pharmacodynamics
Betamethasone dipropionate is a fluoride derivative of hydrocortisone (synthetic form). It quickly penetrates the skin, has powerful local antiallergic, antibacterial, anti-inflammatory, and antipruritic properties. It helps to get rid of the cause of inflammation, and also prevents the release of histamine and the development of local manifestations of an allergic reaction. Due to the local vasoconstrictor effect, exudation reactions are weakened.
Gentamicin is an aminoglycoside antibiotic with certain bactericidal properties. It is an antibiotic with a wide range of medicinal action. It has an antibacterial effect on gram-negative microorganisms (Escherichia coli, as well as Proteus), as well as individual gram-positive microbes, including staphylococci, which are resistant to penicillin.
It is not effective against fungi, anaerobic bacteria, and viruses.
Pharmacokinetics
Betamethasone dipropionate does not undergo biotransformation in the skin. Absorption into the body from the skin is insignificant - no more than 1%. Metabolism occurs in the liver, and excretion is mainly with urine, and also, to a lesser extent, with bile. Absorption of the active component may increase when used on delicate skin, on folds, as well as on skin whose epidermis has been damaged or on which there is inflammation. In addition, absorption is enhanced due to frequent use of the drug, as well as as a result of using the drug on large areas of the skin. More pronounced absorption of the substance is observed in young patients.
Gentamicin sulfate is not absorbed on intact skin, but if there are burns, lesions or ulcers on it, systemic absorption of the drug is possible after local use. After passing the metabolism process, the substance is excreted from the body unchanged along with urine.
Dosing and administration
The ointment is applied in a thin layer to the inflamed areas, gently rubbing the medicinal substance into the skin. The ointment may be applied to areas where the epidermis is thickened more than 2 times a day. The treatment course lasts no more than 1 month. Within a year after its completion, a repeat course may be prescribed (prescribed only by a doctor). It is used to treat subacute conditions, chronic dermatosis, and lichenification.
The cream is used to treat acute forms of pathology, as well as in the presence of oozing on the skin.

Use Belogent during pregnancy
During pregnancy, the drug can be used only in exceptional situations, and the appropriateness of its use is determined by the attending physician. It should be taken into account that in this case, Belogent can be used for a short period of time, and only small areas of the skin can be treated with it.
It is prohibited to treat the skin near the mammary glands during lactation.
Contraindications
Among the main contraindications of the drug:
- skin infections of viral origin;
- the presence of wounds in the area that needs to be treated with the drug;
- skin reactions that occur after vaccination;
- the presence of common acne on the skin;
- children under 1 year of age;
- high sensitivity to gentamicin, as well as betamethasone and auxiliary elements of the drug;
- presence of rosacea;
- trophic ulcers;
- cutaneous tuberculosis;
- syphilitic manifestations on the skin.
Side effects Belogent
As a result of using Belogent, the following side effects may develop:
- the appearance of stretch marks on the skin;
- development of hypopigmentation or urticaria;
- atrophic changes in the skin;
- local skin hyperemia;
- the appearance of itching or burning;
- increased hair growth on the body;
- acne-like rash;
- development of telangiectasia.
Overdose
With prolonged or excessive (in high doses) use on large areas of the skin, as well as when using an airtight dressing, overdose manifestations may occur. In this case, there is an increase in systemic side effects that are characteristic of GCS drugs (such as glycosuria, hyperglycemia, and hypercorticism syndrome) or gentamicin (nephrotoxic and ototoxic effects, which can be especially dangerous if the patient has renal failure).
In the case of a single overdose of gentamicin, no signs of overdose were observed. However, in the case of prolonged use in excessive doses, the growth of antibiotic-resistant bacteria may increase significantly.
To eliminate the disorders, treatment aimed at eliminating the pathological symptoms is necessary. Acute hypercorticism is usually reversible. If necessary, electrolyte balance is corrected. If the toxic effect is chronic, the use of GCS should be gradually discontinued. If excessive growth of drug-resistant bacteria begins, Belogent should be discontinued and appropriate treatment should be prescribed to the patient.
Interactions with other drugs
Due to external use of GCS, drug interactions did not occur. But it should be noted that during therapy it is not recommended to vaccinate against smallpox, and in addition to this, to perform immunization of other types (especially if long-term treatment of large areas of the skin is carried out), because the body may produce an inadequate immunological reaction, which manifests itself in the form of production of appropriate antibodies.
The drug can increase the effectiveness of immunosuppressants, but the effect of immunostimulants, on the contrary, is weakened.
The drug is not recommended for combined use with other skin medications, as there is a risk of weakening its effect.
Gentamicin may interact with heparin, β-lactams (eg, cephalosporins), sulfadiazine, and amphotericin B.
