In case of severe convulsions, intravenous administration of diazepam or short-acting barbiturates is recommended.
Ajicold herbal syrup
A combination drug used for colds and coughs is Adzhikold herbal syrup. The effectiveness of the medicine is due to its composition. The syrup has mucolytic properties, that is, it helps to thin and remove phlegm, reduces swelling and hyperemia of the throat mucosa, facilitates breathing, and reduces bronchospasms.

- The main contraindication for use is intolerance to the components of the drug. Adzhikold is contraindicated in severe renal and hepatic dysfunction, during pregnancy and lactation, in severe forms of diabetes mellitus, acute pancreatitis, hyperthyroidism and for children under 6 years of age.
- Overdose may occur due to the action of paracetamol. It manifests itself as pronounced pallor of the skin, nausea, vomiting, increased activity of liver transaminases. Symptomatic therapy, gastric lavage and administration of activated carbon are indicated for treatment.
Adjicold hotmix
Another popular form of cold medicines is powders for making solutions with different flavors. Adzhikold Hotmix has an orange flavor and is available in packages of 5, 10 and 50 sachets of 5 g each. The granulated powder contains the same active ingredients as other forms of the drug, i.e.: paracetamol, caffeine, phenylephrine hydrochloride, chlorpheniramine maleate.
- It is used for symptomatic treatment of colds and infectious and inflammatory diseases, which are accompanied by headaches and muscle pain, fever, nasal congestion and acute rhinitis. The medicine is taken 1 sachet 4 times a day. The contents of the bag must be poured into a glass and filled with 200-250 ml of hot water.
- Side effects: increased fatigue, decreased speed of psychomotor reactions, dry mouth, temporary increase in blood pressure and increased heart rate, epigastric pain, urinary retention.
- Contraindications: hypersensitivity to paracetamol or any other components of the drug, patients under 15 years of age, hyperoxaluria. Do not use with tricyclic depressants or monoamine oxidase inhibitors.
- The drug should be prescribed with special caution in the presence of the following systemic manifestations: hypertension, bradycardia, pregnancy and lactation, hyperthyroidism, prostatic hypertrophy, recent myocardial infarction or unstable angina.
- Overdose: loss of appetite, pale skin, liver necrosis, nausea and vomiting. A more pronounced toxic effect occurs with a single use of 10-15 g of the drug. In this case, emergency medical care is indicated with gastric lavage and monitoring of vital signs.
Pharmacodynamics
Ajikold is a combination drug, as it contains several components. Pharmacodynamics is determined by the following substances:
- Paracetamol is an antipyretic and analgesic agent. It reduces the excitability of the hypothalamic thermoregulatory center and participates in the inhibition of prostaglandin synthesis.
- Phenylephrine hydrochloride is a vasoconstrictor that works to relieve nasal congestion and reduce discharge from the sinuses.
- Caffeine is a stimulant that affects the central nervous system, reduces the feeling of drowsiness and fatigue. Increases mental and physical performance.
- Chlorpheniramine maleate is an anti-edematous and anti-allergic agent. Its action is aimed at reducing vascular permeability. It relieves swelling and redness of the nasal mucosa. It eliminates the symptoms of allergic rhinitis and facilitates nasal breathing.
Pharmacokinetics
The anti-cold remedy begins to act 30 minutes after application, providing an antipyretic and analgesic effect. Pharmacokinetics is expressed by the following components:
- Paracetamol is rapidly absorbed from the gastrointestinal tract and binds to plasma proteins. The maximum plasma concentration is observed after 1-4 hours. Metabolizes in the liver, forming paracetamol glucuronide sulfate. The metabolic process is associated with general clearance and the microsomal enzyme system. Penetrates the placental barrier. Glucuronidation products have nephrotoxic and hepatotoxic properties. The substance is excreted as conjugation products, about 5% is excreted unchanged.
- Phenylephrine hydrochloride is poorly absorbed from the gastrointestinal tract, metabolized during the first pass through the liver by monoamine oxidase in the intestinal walls. Undergoing biotransformation, it forms a sulfated derivative - phenylephrine 3-O-sulfate. Bioavailability is low, undergoes systemic absorption.
- Chlorpheniramine maleate is completely and quickly absorbed from the gastrointestinal tract and distributed throughout the body tissues. About 70% of the substance binds to plasma proteins. The half-life is 8 hours. Metabolizes in the liver. Gradually absorbed from the gastrointestinal tract, the maximum concentration in the blood plasma occurs after 3-6 hours. Excreted in the urine, part unchanged and part of the substance in the form of dimethylated metabolites. The rate of excretion depends entirely on the pH of the urine.
- Caffeine is rapidly absorbed, the maximum concentration in the blood plasma is reached 5-90 minutes after oral administration. It is rapidly destroyed in cells, penetrates the placental and blood-brain barrier. About 35% binds to blood plasma proteins. It is rapidly metabolized in the liver, forming metabolites: 1-methyluric acid, 1-methylxanthine and 7-methylxanthine. All metabolites undergo intensive biotransformation, the elimination period is about 5 hours. It is excreted by the kidneys.
Dosing and administration
The anti-cold remedy has several forms of release, on which the method of application and dosage depend. Adzhikold is prescribed according to the following schemes:
- Tablets – adults 2 pcs. 1-4 times a day, for children 1 pc. 1-4 times a day. The course of treatment is 5-7 days.
- Ointment - intended for external use and inhalation for rhinitis. A small amount of the medicine should be spread over the neck, back, chest and rubbed well. After rubbing, wrap up and stay warm.
- Syrup – for children aged 3-14 years, ½ - 1 teaspoon 3 times a day, for patients over 14 years old and adults, 1-2 spoons 3 times a day. Duration of treatment is 14-21 days.
- Powder for solution preparation – the contents of one sachet must be poured with hot water and stirred until completely dissolved. The medicine is recommended to be used one sachet up to 4 times a day. The course of treatment should not exceed 5 days.

Overdose
The medicine must be used in the dosages prescribed by the doctor. Failure to follow the treatment recommendations may cause adverse symptoms. Overdose in most cases is associated with the action of paracetamol. Attacks of nausea and vomiting, pronounced pallor of the skin, increased activity of liver transaminases and increased heart rate appear. To eliminate these reactions, it is necessary to wash the stomach, take activated charcoal and, if necessary, conduct further symptomatic therapy.
Interactions with other drugs
For fast and effective treatment of colds, a comprehensive approach is recommended. Interactions of Adzhikold with other drugs should be monitored by the attending physician. This is due to the high risk of developing adverse reactions from many organs and systems.
- The drug is not recommended to be taken with other medications that contain paracetamol.
- The drug enhances the effect of MAO inhibitors, ethanol and sedatives.
- When used with antidepressants, antipsychotics, antiparkinsonian and phenothiazine derivatives, the risk of developing constipation, dry mouth and fluid retention in the body increases.
- When interacting with glucocorticosteroids, the risk of developing glaucoma increases.
- Paracetamol slows down the action of diuretics.
- Chlorpheniramine in combination with MAO inhibitors may cause hypertensive crisis and hyperpyrexia.
- Tricyclic antidepressants increase their symptomatic effect.
- Halothane increases the risk of developing ventricular arrhythmia.
