Indications Aescin
Indications for the use of tablets are infections caused by pathogenic microbes that are sensitive to the drug:
- chronic form of venous insufficiency;
- inflammation of the veins on the legs;
- hemorrhoids;
- treatment, as well as the prevention of hematomas and swelling arising after surgery or trauma.
The gel is used topically, as part of the symptomatic treatment, and in addition, when:
- trauma of closed type and sprain;
- local inflammation (with edemas or without them);
- pain in the back with signs of squeezing the roots of the nerve endings of the spinal cord (with lumbago, osteochondrosis or sciatica).
The medicine can be used as an auxiliary for weakening other manifestations: gravity along with pain in the legs, itching of the skin, edema of the legs, and along with this, cramps of the tonic type in the calf muscles that appear as a result of a disturbance of the circulatory process in the legs.
Pharmacodynamics
The drug helps to regulate the permeability of the walls of the capillary vessels, and also enhances the strength of the capillary endothelium.
Eszin is the main saponin, which is extracted from the fruits of the esculum. It has anti-inflammatory and anti-edematous properties, it helps to normalize the tone in the membranes of blood vessels, but it is impossible to reliably fix the effect of the drug for local use. Reduces vascular permeability.
The properties of the drug are due to the suppression of the activity of the enzyme hyaluronidase, which affects the permeability of the vascular walls. The improvement in their tone is due to the stimulating effect that α-escin has on the secretory function of the hormones of the adrenal cortex, on the formation in the vascular walls of PGF2α, and on the release of norepinephrine from nerve endings. In addition, the effect of the decomposition products of escin (it is similar to the effect of hormones of the adrenal cortex). All these effects allow to increase the tone of muscle cells in the vascular walls.
Salicylate diethylamine has anti-inflammatory and analgesic properties.
Heparin, which is contained in the gel Aescin, is a natural polyanionic glycan that depresses all phases of blood uptake. After topical application, it has an anti-inflammatory and antithrombotic effect on the skin.
Pharmacokinetics
Eszin after internal reception is quickly absorbed from the digestive tract (approximately 11%). The peak of its concentration also reaches rapidly, and then its redistribution occurs in the tissues. With plasma proteins, the active ingredient binds in significant amounts. Excretion is carried out together with urine, as well as with bile.
With local use, the drug is also well absorbed. After topical application in the skin with subcutaneous tissues, a high concentration of α-escin is determined. But in the blood plasma and other tissues this indicator is rather low.
Dosing and administration
The dosage of the drug in adult tablets is 1 unit three times per day. If necessary, the dosage is allowed to be increased to 2 tablets three times per day. To prevent the development of bruising and swelling after surgery, you should start using the medicine at least 16 hours before the procedure. The maintenance dosage consists of 2 tablets twice a day.
Tablets should be eaten after eating, squeezed with water. The duration of the treatment course is appointed by the doctor - in accordance with the effectiveness of the drug, as well as the severity of the disease. The average duration of the course is usually 2-3 months.
The gel should be applied 3-5 times a day, in a thin layer, rubbing it slightly into the skin at the site of the lesion. To accelerate the absorption of the active ingredient, you can perform a small massage before applying the medication. The duration of the course of treatment is prescribed by the doctor and depends on the course and severity of the disease, but it should be borne in mind that it is better not to use the gel for a long time (more than 3 weeks). Also, do not apply it to large areas of the skin.
Use Aescin during pregnancy
There is no clinically confirmed information regarding the use of the drug during pregnancy, and also during breastfeeding.
Studies conducted on animals did not demonstrate the effect of α-escin on the course of pregnancy, as well as the development and growth of the fetus. But still the medicine is not allowed to use on the 1st trimester. In the future, Aescin can be used only with the permission of the treating doctor, in cases where the benefits for women exceed the possibility of developing negative consequences for the fetus.
During lactation, it is prohibited to treat the areas in the sternum with gel.
Contraindications
Among the existing contraindications:
- individual sensitivity to the active component of Aescin or other auxiliary components;
- the patient has renal failure;
- age younger than 18 years.
The gel is forbidden to treat mucous or skin areas with open wounds, and in addition to apply with necrosis of the skin, as well as after irradiation procedures.
Side effects Aescin
Due to the use of the drug (in tablets), such adverse reactions can develop:
- organs of the immune system: hypersensitivity reactions, including pruritus, hives, and a feeling of heat;
- organs of the gastrointestinal tract: pain in the epigastrium, a disorder in the digestive tract, vomiting along with nausea, and diarrhea;
- organs of the cardiovascular system: the development of tachycardia.
In the case of taking drugs at high dosages, thromboses, nephrotoxic reactions, and a decrease in blood pressure can be observed.
When using the gel, local reactions are observed - heat, itching or development of urticaria.
Such symptoms mostly disappear on their own.
Since the gel contains methylparaben (E218), as well as propyl paraben (E216), it is possible to develop allergic reactions in a delayed form.
Interactions with other drugs
The drug can increase the effectiveness of antithrombotic drugs. And cephalosporins can increase the strength of Aescin's effects.
Do not combine Aescin and aminoglycosides, as this combination enhances the nephrotoxic properties of the latter.
Also, you can not combine the drug in the form of a gel with NVPs, warfarin and salicylates.
Storage conditions
Keep the drug should be in the usual for medicines conditions - closed from sunlight and moisture, a place inaccessible to young children. The temperature is a maximum of 25 ° C.
