Adonis-bromine

Adonis-brom is a combination medicine made from plant materials. Its properties include cardiotonic and sedative effects.
Indications Adonis-bromine
The medicine is indicated for moderate to mild chronic circulatory failure and is used as a sedative.
Release form
It is available in tablet form. One package contains a blister with 10 tablets, or a jar with 25 tablets.
Pharmacodynamics
The properties of the medicine are determined by the components that are part of it - an extract from Adonis of polar cymarin glycosides (about 1-10% of the total number of glycosides) with adonitoxin (about 3-20% of the total number of glycosides), and in addition bromide ions.
The latter allow stabilizing the function of GABA receptors (in the open form), after their connection with aminalone, thereby facilitating the passage of bromide and, in addition, chloride ions into the cells, and reducing neural activity.
Cymarin binds to the neurosteroid site responsible for recognition in the GABA receptor, thereby increasing its sensitivity to the properties of aminalone. Inside the myocardial and neuronal membranes, cymarin acts as a blocker of Na+/K+-ATPase activity, thereby preventing the removal of Na(+) from cells. An increase in the sodium level inside cells destabilizes the process of calcium removal from them, and also accelerates the contractile function of the myocardium.
Adonis-brom has weak cardiotonic properties, mainly due to the presence of a systolic component: it increases the force of heart contractions (positive inotropic effect), as well as myocardial excitability (positive bathmotropic effect).
Diastolic properties are expressed more weakly - the effect is characterized by a slowing of the heart rate (negative chronotropic effect), as well as the conduction of nerve impulses through the myocardium (negative dromotropic effect).
As a result of using the drug, the heart contractions are slightly increased, and the time period of systole is reduced, simultaneously with the lengthening of the diastole period. There is also a decrease in heart rate.
Adonis-brom accumulates and, as a result, enhances the slowing processes in the cerebral cortex. In addition, it stabilizes the balance between the functions of inhibition and excitation within the central nervous system.

Use Adonis-bromine during pregnancy
Adonis glycosides are able to pass through the barrier formed by the placenta, which is why the drug is not recommended for use during pregnancy.
Adonis-brom can penetrate into breast milk, so it is necessary to stop breastfeeding for the duration of the treatment course. Breastfeeding can be resumed at least a month after completion of therapy.
Contraindications
Among the contraindications:
- severe form of bradycardia;
- individual intolerance to the components of the drug;
- acute stage of gastritis or enterocolitis;
- ulcer;
- MAC syndrome;
- endocarditis;
- AV block;
- the presence of angina pectoris (in this case, the drug is allowed to be used only if heart failure was diagnosed in parallel);
- ventricular tachycardia;
- carotid sinus syndrome;
- glycoside poisoning;
- myocardial infarction (permitted to use only in cases of severe heart failure accompanied by an increase in the myocardium);
- Wolff-Parkinson-White syndrome;
- hypercalcemia or hypokalemia;
- isolated form of mitral valve stenosis;
- childhood;
- thoracic aortic aneurysm;
- state of shock;
- hypertrophic stage of cardiomyopathy.
Side effects Adonis-bromine
As a result of using the drug, side effects such as vomiting with nausea are possible (they occur due to the effect of cardiac glycosides on the vomiting center with chemosensitive receptor zones, and in addition to this, the reflex reaction of the gastric mucosa to the irritating properties of the drug). Also, the gag reflex can occur due to the excitation of receptors in the heart.
Overdose
If the dosage is exceeded, CNS dysfunction (anxiety, headaches, vision problems, insomnia and symptoms of depression) or diarrhea, as well as loss of appetite, may develop.
As a result of an overdose from taking cardiac glycosides, a sharp development of bradycardia is possible, as well as bi- or trigeminy or polytopic extrasystole, as well as a slowdown in conduction between the ventricles and atria.
Taking toxic doses can provoke ventricular fibrillation and cardiac arrest. Since the drug can accumulate, its toxic effect can manifest itself in some form even in the case of prolonged use of the drug in standard doses.
In case of poisoning due to overdose of cardiac glycosides, it is necessary to stop taking the medicine for some time. If necessary, the patient is prescribed antiarrhythmic drugs, as well as potassium preparations.
Interactions with other drugs
As a result of combining with preparations of lily of the valley, foxglove, and strophanthus, the toxic properties of other cardiac glycosides may be enhanced.
Combination with antiarrhythmic drugs of type IA (these are procainamide, quinidine, and also disopyramide), as well as laxatives, GCS and calcium preparations increases their toxic properties and medicinal effect on the body.
Increases the depressant effect exerted on the central nervous system by benzodiazepine derivatives, as well as sedative-hypnotic drugs, anticonvulsants, neuroleptics and alcoholic beverages.
When combined with halothane anesthesia, the level of bromide ions in the body increases, and the toxic properties and medicinal effects of Adonis-bromine also increase.
As a result of eating salty foods during treatment with Adonis-brom, the rate of excretion of bromides in the urine increases, and in addition, the effect of the drug is weakened.
