Removing a wisdom tooth is one of the most difficult dental procedures, since it is most often caused by improper positioning, carious damage, severe tooth decay and, as a consequence, the occurrence of a source of inflammation in the oral cavity.

"Wisdom teeth" - this name is associated with the period of their eruption: they usually grow in a person at a fairly mature age - from 18 to 24 years old, and sometimes later. According to the ordinal arrangement in the dental row (starting from the center), such teeth are sometimes called "eights" or "third molars" (a molar is a large molar). Usually, a person has 4 wisdom teeth. According to the results of numerous studies, they do not perform certain functions in the chewing process, although they have the same structure as other teeth. The need to remove wisdom teeth arises in cases where they cannot erupt or grow incorrectly (for example, horizontally), thereby damaging the teeth located next to them. Hard-to-reach location complicates the process of cleaning wisdom teeth, which provokes the development of caries. Due to the gradual destruction of the wisdom tooth, the proliferation of bacteria increases, which often leads to inflammatory processes and the development of infections.
Indications for wisdom tooth removal
Removal of wisdom teeth is recommended in cases that indicate complications caused by disturbances in tooth growth. When such problems arise, patients note pain in the area of tooth eruption, difficulty opening the mouth, redness and swelling of the gums, fever, the appearance of an unpleasant odor from the mouth, discomfort when chewing, painful swallowing.
Indications for wisdom tooth removal are directly related to symptoms that signal an inflammatory process or the occurrence of an infection:
- development of pericoronitis - a process of difficult tooth eruption, as a result of which inflammation of the gum covering it (“hood”) and periodontal tissues occurs;
- abnormal growth of teeth (for example, at an angle or horizontally), resulting in damage or destruction of nearby teeth;
- destruction of a wisdom tooth due to carious lesions;
- the trauma of the tooth eruption process, which is primarily associated with damage to the gums, injury to the mucous membrane of the inner surface of the cheeks, the formation of scars, the development of an inflammatory process, etc.;
- formation of a follicular cyst at the root of a wisdom tooth;
- development of an abscess or phlegmon caused by incorrect placement of a wisdom tooth;
- osteomyelitis of the jaws in the area of wisdom tooth eruption;
- development of trigeminal neuralgia due to difficult eruption of wisdom teeth, as well as other problems.
At the slightest discomfort caused by painful eruption of a wisdom tooth, it is recommended to immediately contact a dentist who will conduct an examination and, based on an X-ray, objectively assess the situation in order to make an adequate decision.
One of the most common problems is the difficult eruption of the wisdom tooth, which leads to inflammation of the gums that partially cover the tooth, the so-called "hood". Its constant trauma from hard food, as well as the formation of plaque, create very favorable conditions for the increased reproduction of pathogenic bacteria. Thus, a source of infection forms around the wisdom tooth, and the periodontal tissues become inflamed.
Failure to seek medical attention in a timely manner or complete lack of treatment for a damaged wisdom tooth can lead to serious complications. The most common of these are osteomyelitis of the jaw, abscess or phlegmon. That is why it is so important to ensure that medical care is provided to the patient as soon as possible.
Wisdom tooth removal procedure
Removing a wisdom tooth requires a qualified approach from a competent specialist, so this operation should be performed by an experienced, trained dentist. Before the procedure, the doctor must conduct an X-ray examination of the diseased tooth to determine its location, as well as the structural features.
The procedure for removing a wisdom tooth, if the patient visits a dental clinic in a timely manner, is virtually painless, under local anesthesia, and subsequently does not have any unpleasant complications. Typically, such an operation consists of the following stages:
- administration of local anesthesia;
- an incision in the gum to provide access to the diseased tooth;
- wisdom tooth removal using modern dental instruments;
- suturing the gums.
In most cases, dentists use non-absorbable suture threads for suturing, which are removed almost painlessly a few days after the wound at the site of the extracted tooth has healed. If the wisdom tooth extraction procedure was performed correctly, the wound from the extracted tooth will heal quickly and without complications. At the same time, the patient must carefully follow the dentist's recommendations regarding oral hygiene.
Anesthesia for wisdom tooth extraction
Wisdom tooth extraction is most often performed under local anesthesia. Various methods and means are used as anesthesia.
The process of local anesthesia is as follows: the injected substance acts on the nerve, blocking the nerve impulse that transmits a signal about the irritation of the nerve endings (i.e. pain sensations) to the brain. Thus, a person becomes immune to pain in the place where the active substance acts. After some time, this substance is eliminated from the body, and a complete restoration of nerve impulses occurs.
Anesthesia for wisdom tooth extraction can be used in various forms.
- Infiltration anesthesia is the most common type of local anesthesia used for the treatment and removal of molars or root canals, as well as dental operations on the dental pulp. The anesthetic is injected into the mucous membrane, gums, or intraosseously using a syringe.
- Application anesthesia is a superficial anesthesia. It is used in the form of sprays and gels mainly for the removal of loose (milk) teeth in children, as well as for the purpose of removing tartar and deposits, disinfecting the oral mucosa before the introduction of a deeper anesthetic.
- Conduction anesthesia is a deeper type of pain relief and is used in large-scale surgical interventions: dental operations on gums, removal of large molars, etc. The active substance is injected into the branches of the trigeminal nerve.
- Intraligamentary anesthesia is used by dentists to numb a single tooth. In this case, the anesthetic is injected directly into the ligament around the tooth.
Some of the best anesthetics widely used in modern dentistry are substances based on "Articaine". Their names are "Septanest", "Ultracaine", "Ubistezin", etc. The duration of action of such painkillers is up to 6 hours. For asthmatics, patients with allergic diseases, as well as pregnant and lactating women, the drug "Mepivastezin" is indicated.
Wisdom tooth removal surgery
Removing a wisdom tooth can be simple or complex, depending on the degree of damage to the tooth and the type of problem that has arisen. Simple wisdom tooth removal involves using pliers and elevators, and the procedure itself does not involve complex manipulations such as gum incisions and drilling out a certain area of the bone.
In this case, the wisdom tooth removal operation is carried out step by step:
- determining the presence or absence of allergic reactions in the patient to any medications in order to select the optimal method of anesthesia;
- anamnesis collection - a detailed survey of the patient about the presence of serious diseases (in order to prevent the development of complications during the operation or postoperative period);
- administration of anesthetic;
- waiting for the pain-relieving effect to take effect after anesthesia (approximately 3-5 minutes);
- preparation of the dentist for the operation (selection of medical instruments based on the location of the wisdom tooth, its condition, the presence of infection, inflammatory process, etc.);
- performing an operation using special dental equipment;
- suturing of the tooth socket to speed up wound healing.
A simple wisdom tooth extraction procedure generally takes up to 10 minutes (excluding the time it takes for the anesthetic to take effect). The patient should return to the dentist a few days after the procedure.
In complicated cases, a dental drill is used to perform a wisdom tooth extraction surgery (usually a retained tooth that is unable to fully erupt, or a tooth with a horizontal growth direction), gum incisions are made, and the wound is necessarily sutured. The preparation for the procedure itself is similar to that carried out during a simple tooth extraction, with the only difference being that the waiting time for the anesthetic to take effect increases to 10 minutes. The entire range of manipulations associated with wisdom tooth extraction is carried out exclusively in medical conditions that meet hygienic standards and sterility. The procedure for complex wisdom tooth extraction usually includes a number of actions:
- making an incision in soft tissues;
- drilling or sawing out the bone area located above the wisdom tooth;
- tooth extraction using dental instruments;
- wound suturing.
A couple of days after the operation, the patient is re-examined. The dentist removes the stitches only after he is sure that the wound has healed and there are no signs of inflammation or infection. The postoperative period is usually accompanied by a 5-day course of antibiotic therapy.
Ultrasonic removal of wisdom teeth
Removal of wisdom teeth can be performed using various medical technologies. Nowadays, ultrasound surgery (the so-called "piezosurgery") has found its wide application - one of the most modern methods, which is used in performing manipulations related to bone tissue, as well as performing surgical operations of the maxillofacial area of varying degrees of complexity.
Ultrasonic removal of wisdom teeth using special equipment has a number of advantages and guarantees:
- the ability to perform operations in the most inaccessible places;
- minimal level of trauma to tissues and the oral cavity;
- absence of trauma to the oral mucosa and bone (swelling, severe pain, heating);
- significant reduction in postoperative complications;
- high precision of the manipulations performed, which leads to a predictable result;
- rapid wound healing after surgery.
In complex cases, such as the removal of an impacted (unerupted) wisdom tooth, an ultrasonic system becomes the best option for performing the operation, since the use of a drill can lead to dangerous consequences in the form of injury to the mandibular nerve or maxillary sinuses. When access to a wisdom tooth is limited due to its distant location in the oral cavity, it is quite difficult to operate with rotating dental instruments. Therefore, removing a wisdom tooth using an ultrasonic technique is a rational way to perform this type of operation.
Wisdom tooth removal during pregnancy
Removal of wisdom teeth during pregnancy is allowed only in cases where the expectant mother has problems associated with the eruption or abnormal growth of the tooth. In particular, this is severe pain, swelling of the mucous membrane, fever, inflammation in the oral cavity and other problems.
Removal of wisdom teeth during pregnancy in the first trimester is not recommended due to possible risks to the fetus. The operation is performed only in extreme cases when symptoms of severe inflammation are observed due to impaired tooth growth.
In the second trimester, the risk level for the fetus is significantly reduced, so if it is necessary to extract a wisdom tooth, surgery is indicated. However, in this case, it is necessary to remember the possibility of toxic effects of anesthetics on the body of the expectant mother and child. If possible, it is still advisable to postpone the removal of the wisdom tooth until the postpartum period.
In the third trimester of pregnancy, wisdom tooth removal is possible, but again only in extreme cases, due to the risk of premature birth. During breastfeeding, there are no contraindications to wisdom tooth removal, but even in this case, the dentist should use anesthetics with the lowest toxicity level.
It is important to understand that any surgical intervention, including tooth extraction, is a real stress for the human body, especially for pregnant women. Therefore, before making a final decision on the operation, an experienced dentist carefully considers alternative means and methods. If the situation is not critical, tooth extraction is postponed until the postpartum period. Fortunately, today there are special anesthetics developed for pregnant women. They are absolutely safe because they cannot penetrate the placental barrier to the fetus.
Wisdom tooth removal under anesthesia
Wisdom tooth extraction can be performed under general anesthesia in cases where the tooth is completely or partially destroyed, as well as if it has a complex root structure. General anesthesia allows the doctor to freely extract all tooth fragments without worrying about the patient feeling pain or accidentally injuring himself on medical instruments due to a sudden movement.
Removal of wisdom teeth under anesthesia can only be performed in professional dental clinics. During such an operation, a dental surgeon, an anesthesiologist, and in some cases a competent resuscitator usually take part.
Among the main advantages of tooth extraction under general anesthesia are the following:
- complete absence of pain and discomfort during the operation;
- preventing the development of panic fear in the patient;
- the possibility of performing complex operations to remove wisdom teeth;
- eliminating the possibility of accidental injuries.
When removing a wisdom tooth of any complexity, general anesthesia makes it possible to perform this procedure quickly and at the highest professional level. General anesthesia will also be the best option in case of jaw prosthetics, extraction of damaged teeth and their fragments, branched roots, curettage or scraping. After the operation, the patient may experience a feeling of discomfort, which is usually associated with tissue healing, but this process is short-lived. Of course, before deciding to perform an operation to remove a wisdom tooth under general anesthesia, the doctor must assess the patient's physical condition: the presence of allergic reactions to any medications, as well as cardiovascular diseases or other serious conditions for which general anesthesia may be contraindicated.
Upper wisdom tooth removal
Removing a wisdom tooth is a very complicated operation, especially when the tooth has significant damage or destruction, as well as a curved root. Often, when upper and lower wisdom teeth erupt, problems arise because this process occurs slowly and not completely, as a result of which the tooth remains under the mucous membrane, which is injured by hard food and causes pain to a person. If a wisdom tooth does not erupt completely, a "crater" is formed in it, in the recess of which food accumulates. This leads to the development of caries, as well as the appearance of an unpleasant odor from the oral cavity.
The removal of the upper wisdom tooth is carried out due to its carious destruction or abnormal development. Since wisdom teeth are considered atavisms, since they do not perform an essential function, it is advisable to remove them rather than treat them. It should be noted that the removal of the upper "eight" is faster and easier than the lower one. First of all, this is due to its accessibility.
The operation to remove the upper wisdom tooth is performed under local anesthesia. Usually, the dentist uses S-shaped forceps to successfully perform the procedure. However, in complex cases, the operation is performed by a dental surgeon in stages:
- the gum is cut;
- the tooth is sawn into smaller fragments;
- the tooth parts are carefully removed;
- Sutures are placed on the cut gum.
After the operation, the patient may experience swelling and pain for 3-5 days, which are usually treated with painkillers.
Lower wisdom tooth removal
Removing a wisdom tooth that has problems with growth, as well as obvious destruction, is the only correct solution. In this case, tooth treatment is practically not considered. Particular difficulties in removing a lower wisdom tooth are created by its location, as well as the fact that the bone of the lower jaw is much denser than the upper one. A special radiograph taken before the removal procedure will help to avoid various complications during the operation. With the help of an radiograph, the topography of the wisdom tooth, the number and direction of its roots, the localization of destructive foci near the damaged tooth, as well as other anatomical features are determined. Without an radiograph, an operation to remove is allowed only if the neck is exposed or the tooth is very loose.
The removal of the lower wisdom tooth is usually performed under regional anesthesia, since local anesthesia is ineffective in this case. Usually, the dental surgeon begins such an operation with the help of a trained assistant. First, anesthesia is administered, and then surgical manipulations are performed to extract the lower "eight". If the tooth is hidden under the gum ("hood"), the doctor makes an incision and opens the gum. After this, the dental crown is released, and the roots are removed using a straight or angled elevator. Crown and horizontal forceps are also used as instruments. Then the dental surgeon "scrapes out" the contents of the socket remaining after the removal of the lower wisdom tooth. Wisdom tooth hood removal
The removal of a wisdom tooth is caused by serious problems with the incorrect growth of the "eight". Quite often, the eruption of a wisdom tooth is accompanied by inflammation of the so-called "hood" (the mucous membrane of the gum, partially covering the dental crown). The inflammatory process is associated with pericoronitis - the active development of an infection between the surface of the erupting wisdom tooth and the hood hanging over it. In this case, the patient may complain to the doctor about swelling, puffiness and pain in the gum, enlarged lymph nodes, fever, swelling of the cheek, difficulty swallowing.
Removal of the wisdom tooth hood is considered a conservative treatment for pericoronitis. The primary task of the dental surgeon is to excise the hood membrane in order to eliminate the zone of active reproduction of microbes. Such surgical intervention is performed under local anesthesia and, in fact, is low-traumatic. It includes the following stages:
- administration of anesthetic;
- excision of the hood using surgical instruments;
- thorough washing of the postoperative wound with effective antiseptics;
- drug therapy;
- a follow-up appointment with the patient for a control examination of the postoperative wound.
After surgical procedures to remove the dental hood, the patient is prescribed special rinses with an antiseptic solution (potassium permanganate, furacilin, etc.), as well as antibiotics (in case of intense inflammation).
Unfortunately, it often happens that hood excision does not give the desired results, and the inflammatory process does not decrease. In this case, wisdom tooth extraction is recommended. To resolve this issue thoroughly, the dentist uses an X-ray to determine with maximum accuracy the probability of correct wisdom tooth growth. Sometimes it is better to immediately use the tooth extraction method than to prescribe ineffective treatment to the patient.
Removal of impacted wisdom tooth
The removal of a wisdom tooth may be caused by the impossibility of its eruption. In this case, the tooth remains inside the jaw bone or is partially covered by the mucous membrane. If such a picture is observed, then we are talking about the so-called impacted tooth. It happens that such a tooth, located under the gum or jaw bone, does not cause any problems or discomfort to a person. However, there are cases when an impacted tooth causes the development of various complications, in particular, inflammation in the maxillofacial apparatus. Then this anomaly requires immediate intervention of an experienced medical specialist - a dentist or orthodontist.
Removing an impacted wisdom tooth is the only correct solution if the patient is bothered by alarming symptoms indicating the development of inflammation: fever, discomfort, pain in the ear on the side of the impacted tooth. The removal procedure itself is quite complicated and can take up to 3 hours, since the location of the tooth inside the bone prevents its extraction in the usual way. The operation is performed under local anesthesia and consists of the following stages:
- the mucous membrane of the gum is cut;
- the bone tissue is drilled using a bur;
- the tooth is divided into separate fragments;
- all parts of the tooth are removed;
- an antiseptic is placed into the hole (wound);
- stitches are applied (in case of an extensive wound);
It should be noted that the recovery process after such an operation is very painful. Patients experience severe pain when opening their mouths, and they experience swelling in the area where the bone was drilled. Of course, such consequences are quite normal. In case of intense pain, the doctor prescribes anesthetics to the patient. In general, the rehabilitation period usually takes up to 5 days.
Wisdom tooth root removal
Removal of a wisdom tooth can be complicated by a curved root system, which interferes with the operation. When the dental crown is in the field of view, it is easier for the dental surgeon to determine the location of the roots and perform the removal. But if the wisdom tooth is completely destroyed, then it is almost impossible to assess its condition without an X-ray. In this case, only an X-ray will allow you to determine the exact location of the tooth, study the shape of its roots, find out the features of the structure of the socket, its thickness and density.
Removing the root of a wisdom tooth is, without exaggeration, one of the most unpleasant manipulations in dental practice. The level of complexity of such a procedure depends on both the size of the tooth root and its branching, and on the abnormal changes in the tissues adjacent to it. In any case, such an operation should only be performed by a highly qualified specialist.
Of course, before the procedure for removing the root of the wisdom tooth, the patient is given an anesthetic. High accuracy in determining the dose of the drug allows maintaining the anesthetic effect throughout the operation. An important factor in the successful removal procedure is the choice of surgical instruments. With the help of modern elevators and forceps, which work on the principle of a "lever", significantly increasing the load, you can quickly remove not only teeth, but also deep roots. To prevent complications in the postoperative period, the patient must follow all doctor's instructions to prevent infection or inflammation.
Removal of wisdom teeth rudiments
Removal of a wisdom tooth sometimes occurs when the tooth has not yet erupted, but there are already problems with its growth, which can have a significant negative impact on the normal development of the bite.
Removal of wisdom teeth rudiments is indicated for a person when various orthodontic problems arise due to disturbances in the development of the tooth. The most optimal age for this procedure is considered to be from 13 to 16 years. However, such an operation is also allowed for older people if their wisdom teeth have not yet erupted. Most often, the rudiments of the "eights" located on the lower jaw are removed, since the crowding of teeth caused by them is expressed more strongly than on the upper jaw.
The following factors are indications for surgery to remove the rudiments of the “eights”:
- pathologies in the eruption and normal development of wisdom teeth;
- various complications due to delayed eruption of the “eights”: formation of bone pockets, development of cysts, as well as destruction of hard tissues, etc.;
- preventive measures aimed at preventing inflammation and various complications that accompany difficult eruption of the "eights";
- preparing a patient for facial surgery;
- prevention of recurrent jaw pathologies.
Removal of wisdom teeth rudiments is a rather complex dental operation that requires special conditions, so it is carried out strictly in a hospital to avoid postoperative complications.
Removal of a decayed wisdom tooth
Removal of a wisdom tooth is recommended in cases where its eruption causes a number of complications. Sometimes "third molars" (as wisdom teeth are called in medical terminology) are born "into the world" already destroyed. In such a situation, tooth removal is inevitable, since the infection can quickly penetrate into the dental canal and cause a serious complication called "periodontitis". Symptoms of the development of this disease are severe pain in the tooth, radiating to the ear and spreading throughout the jaw. In such a case, removal of a destroyed wisdom tooth must be carried out as soon as possible so that the infection does not cause conditions that are dangerous to a person's life.
Sometimes the inflammatory process occurring in the tooth can be chronic, i.e. the wisdom tooth may look destroyed, but not hurt. However, exacerbations are possible at any time: against the background of decreased immunity or the development of a cold, a destroyed wisdom tooth will cause a lot of problems for a person. In addition, it is a constant source of the spread of chronic infection throughout the body. Therefore, it is recommended to remove a wisdom tooth destroyed by caries in any case, and this should be done as soon as possible.
Removal of an unerupted wisdom tooth
Removing a wisdom tooth makes sense if the tooth has started to bother a person, causing severe pain, discomfort when chewing, and also delivering many other unpleasant sensations. Very often, a problem with the eruption of the "eight" occurs due to the fact that there is not enough space in the dental arch for the normal growth of the wisdom tooth. In this case, it is recommended to remove the unerupted wisdom tooth as early as possible to avoid the development of various complications, in particular, serious infectious and inflammatory processes in the maxillofacial area.
Removing an unerupted wisdom tooth is a very important procedure. The complexity of the operation is due to the immersion of the tooth into hard bone tissue. The deeper the unerupted tooth is located, the more difficult it is to remove. Basically, the process of removing such a tooth occurs as follows.
- Before the operation, the patient receives adequate anesthesia.
- The dental surgeon makes an incision in the mucous membrane in the area where the tooth is located.

- Using special medical instruments (elevators), the doctor separates the wisdom tooth from the surrounding tissues and dislocates it from the socket.

In dental practice, there are often cases when a wisdom tooth grows abnormally, cannot fully erupt, or its position in the oral cavity is incorrect. In this case, we are talking about the so-called "dystopic" (abnormally growing) wisdom tooth. Such complications are usually associated with the fact that wisdom teeth erupt last, and often there is simply not enough space for them in the arch.
Removal of a displaced wisdom tooth is associated with difficulty in its eruption due to already formed bone tissue. At the first symptoms of inflammation as a result of the eruption of the "eight", it is necessary to consult a dentist, since in this case the removal must be carried out immediately.
The extraction procedure begins with anesthesia. Then the dental surgeon cuts the mucous membrane to gain access to the tooth. Sometimes it is necessary to saw the tooth into separate parts in order to extract all the fragments efficiently. After the tooth is extracted, the postoperative wound is treated with antiseptics and sutures are applied.
After the removal of a displaced wisdom tooth, the patient is not recommended to eat or drink for several hours. It is necessary to carefully monitor the wound healing process, especially when a blood clot forms in the socket, which performs protective functions and is very important for wound healing.
Recommendations after wisdom tooth extraction
Removing a wisdom tooth requires following doctor's recommendations aimed at speedy healing of the wound and recovery of the body after the stress experienced.
Below are the basic recommendations after wisdom tooth removal:
- A cotton-gauze swab placed on the socket of an extracted tooth after surgery to stop bleeding must be kept there for at least 20 minutes.
- After the operation, it is not recommended to eat for 3 hours; smoking and drinking alcohol are also prohibited.
- In case of severe pain, it is recommended to take a painkiller, after consulting a doctor.
- To reduce bleeding and prevent swelling, you can apply cold to the cheek on the side of the removed tooth.
- To avoid wound infection and the development of an inflammatory process, on the second day after tooth extraction surgery, you should begin rinsing your mouth with an antiseptic solution (as prescribed by your doctor).
- The patient should reduce physical activity that may provoke bleeding from the socket.
- The diet should include soft foods to avoid re-injury to the wound.
- It is necessary to avoid touching the postoperative wound with the tongue, since any movement can damage the blood clot that forms in the socket to ensure rapid healing of the wound.
