These terms "infected hydronephrosis and pionefrosis" indicate different stages of secondary kidney infection in the background of hydronephrosis.
Pionephrosis is a special form of acute and chronic pyelonephritis, associated with obstruction of the ureter with a stone or stricture. The most frequent place of obstruction is the tuberculosis-ureteric anastomosis.
Symptoms of the infected hydronephrosis and pionefrosis
The condition of the patient, as a rule, is extremely severe, reveals the following symptoms of pionephrosis: high body temperature, chills, pain in the lumbar region. In the anamnesis - urolithiasis, recurrent urinary tract infections and urological interventions. With infected hydronephrosis, the cup-and-pelvis system is also expanded, but its contents are uniform.

Diagnostics of the infected hydronephrosis and pionefrosis
Characteristic features of ultrasound of the kidneys: a large, dilated cup-and-pelvis system, filled with heterogeneous contents (purulent urine, fragments of tissues), and pronounced manifestations of the infectious process in the papillae and the medulla of the kidney. With pionefroze purulent melting of the renal parenchyma leads to a significant decrease in kidney function and is life-threatening.

Treatment of the infected hydronephrosis and pionefrosis
Treatment of pionephrosis consists of parenteral administration of antibacterial drugs and drainage of the kidney. Later, after sanitation of the urinary tract, removal of obstruction is shown.
Pionephrosis is a serious disease requiring urgent surgical treatment (nephrectomy) and parenteral administration of antibiotics.
