Surgical interventions and inflammatory processes in the uterus can cause serious damage to a woman's body. This often leads to the formation of uterine adhesions. They are small formations of connective tissue. Their presence in the fallopian tubes entails the impossibility of conception.

According to the ICD 10 code, adhesions are classified as inflammatory diseases of the female pelvic organs and are coded under numbers N70-N77.

Causes of adhesions of the fallopian tubes

Medicine knows several main factors that can lead to the development of this situation. The following reasons can provoke the development of irritation and lead to the formation of adhesions of the fallopian tubes.

  • Mechanical effects. These include surgical interventions. They severely injure the uterine mucosa, leading to the development of adhesions.
  • Gynecological diseases. Previously suffered serious diseases, including salpingitis, can close the external openings of the tubes and thus lead to their adhesions. Chlamydia and endometriosis are especially dangerous.

Most often, adhesions appear due to previous abortions, cauterizations, and abdominal surgeries. Tears during childbirth, hysteroscopy, cesarean section, and laparoscopy can lead to the development of the process. An intrauterine device can injure the uterus. Pathogenesis

The adhesive process most often manifests itself in different ways. Pelvic pains of a pulling and aching nature are noted. Without understanding the true cause of this condition, women resort to self-medication. The pathogenesis of the phenomenon is quite interesting and depends entirely on the location of the adhesion development.

Thus, constant constipation can lead to a disruption of the intestinal functionality and thus cause the formation of loops in the form of adhesions. The negative process also has a negative effect on pregnancy. As a result of the formation of adhesions, the fallopian tubes are deformed, which does not allow the egg to enter the uterine cavity.

The adhesion process is based on mechanical damage. It may be associated with a previous abortion or surgical intervention. The damaged parts of the uterine mucosa are not restored, the tissue is not able to regenerate and adhesions form at the site of damage.

Symptoms of adhesions of the fallopian tubes

It is not always easy to notice the presence of a problem. Many patients note the appearance of aching and pulling pains. Due attention is not paid to these symptoms. First signs

Uterine adhesions do not manifest themselves in any way. Their presence in a woman's body does not affect her well-being at all. The menstrual cycle is not disrupted, there is no particular pain. You can suspect something is wrong when you cannot get pregnant. Most often, the lack of conception is the first sign of obstruction.

However, adhesions can be suspected. This is possible if symptoms of an inflammatory process appear. It is characterized by the presence of a pronounced pain syndrome and a significant increase in body temperature.

As mentioned above, the main sign of the development of the adhesion process is the absence of pregnancy in the presence of regular unprotected sexual activity. A woman can suspect the development of a pathological process on her own. There are no problems with ovulation, basal temperature is normal, ultrasound showed no abnormalities, but pregnancy has not occurred.

Pain due to adhesions of the fallopian tubes

There is still no precise data regarding pain syndrome in the presence of adhesions. In most cases, pain is associated with the postoperative rehabilitation period or the presence of an inflammatory process. Whether severe pain can occur with adhesions in the fallopian tubes or not, no specialist can answer unequivocally.

Thanks to the conducted research, one thing has become known: the frequency of pain syndrome in women with adhesions is exactly the same as in the absence of this process. Its severity directly depends on the extent of the scar.

Operations that successfully remove adhesions can relieve pain. Most women describe the pain syndrome as not strong and short-lived. It can intensify with movement, during and after sexual intercourse. The pain can be pronounced when sitting for a long time or as a result of hypothermia.

Uterine-ovarian adhesions

This process is characterized by particular complexity. The fact is that the uterine-ovarian adhesions completely block the "gap" between the uterine cavity and the ovaries. This leads to the impossibility of conception. Pregnancy can occur, but if there is no possibility of penetration of the egg into the uterine cavity, an ectopic pregnancy develops.

If the pathological process is not noticed in time, there is a risk of death. An ectopic pregnancy is characterized by severe bleeding, which is difficult to stop. If it is detected in time, the fertilized egg is removed. In some cases, part or all of the fallopian tube is removed. This surgical intervention means that the woman will never be able to become a mother.

Adhesive disease is the most formidable phenomenon that can cause serious health problems. There were cases when the process was so favorable that it did not entail the development of complications. If the adhesive process made itself known, everything depends on its extent.

Diagnosis of adhesions in the fallopian tubes

It is not so easy to detect diseases. In this case, it is impossible to make a diagnosis based on the patient's complaints, because there are simply none. Diagnosis of adhesions in the fallopian tubes involves the use of some instrumental methods.

  • Hysterosalpingography. This is a radiological method based on the introduction of a special contrast agent into the uterine cavity. Its progress is monitored using an X-ray machine.
  • Hydrosonography. The method is based on the introduction of a sterile solution into the uterine cavity. It is studied using ultrasound.
  • Laparoscopy. The fallopian tubes can be examined by making several incisions in the abdominal wall. Carbon dioxide and a camera are introduced through them. This allows the condition of the fallopian tubes and their patency to be assessed.
  • Fertiloscopy. The method is similar to laparoscopy, however, the incisions are made directly in the vagina.

These methods, even in combination, are not capable of giving a 100% result. Therefore, they are supplemented with additional methods. These include: ultrasound, examination of the cervix and examination of the sperm of the woman's sexual partner.

Tests

During diagnostics, specialists collect all data on the woman's life. In the absence of fallopian tubes, it is necessary to provide extracts on previously performed operations. An examination by a therapist, infectious disease specialist, endocrinologist and psychiatrist is also important. As for tests, you will have to take quite a lot of them.

General blood test. It allows tracking the quantitative content of all important components. These include: leukocytes, hemoglobin, erythrocytes, platelets. The coagulation time and ESR are determined. Blood biochemistry is carried out, aimed at tracking the amount of total protein, urea, sugar and creatinine. They resort to the help of a coagulogram, the blood group and its Rh factor are determined. As an additional source of information, a general urine test is taken.

Tests for RV, hepatitis, HIV and Australian AG are mandatory. You will have to give blood for hormones: estradiol, testosterone, prolactin and progesterone.

As additional studies, a general smear and bacterial culture are performed. The man must be tested for HIV, RV, Australian AG, Hepatitis C, B. At the same time, a spermogram is also studied.

Instrumental diagnostics

The first step is to determine the presence/absence of regular ovulation in a woman. This is done by ultrasound, and the patient must also measure her basal temperature over several cycles. As an instrumental diagnostic, many methods are used to provide a complete picture of what is happening.

  • Ultrasound. A regular transvaginal examination is unable to determine tubal obstruction. A special ultrasound gynecological examination will help to sort this out. The only drawback of this method is the inaccuracy of the results. However, this method is widely used. Before the procedure, the doctor introduces a special sterile solution into the uterine cavity. This will straighten the walls of the uterus and make them more visible on the ultrasound. After this, the specialist determines where the fluid is flowing. If the fallopian tubes are characterized by their obstruction, then the uterus will begin to stretch under the pressure of the introduced solution.
  • HSG – hysterosalpingography, x-ray of the uterus and tubes. This method is more informative than the previous one. But in recent years it has not been used so often. It is especially informative in diagnosing tuberculosis of the female genital organs, and not uterine adhesions. The essence of the procedure is the introduction of a contrast agent into the uterine cavity and the execution of several x-rays. In case of obstruction, the solution will collect in one place and this will be visible on the image.
  • Diagnostic laparoscopy. This method is the most popular and informative. It allows diagnosing not only the obstruction of the fallopian tubes, but also identifying the main causes of infertility. The advantage of the study is the high accuracy of the results obtained. To determine the obstruction, a special solution is injected into the cervix, which penetrates into the abdominal cavity.
  • Fertiloscopy and transvaginal hydrolaparoscopy. This method involves examining the female genitals using a video camera. Most often, this procedure is performed together with chromohydroturbation, hysteroscopy and salpingoscopy.

The above methods allow to determine the exact cause of the obstruction development. But for greater information, instrumental diagnostics is combined with laboratory tests.

Ultrasound

Ultrasound examination allows to determine the presence of pathological processes in the female genitals. The examination is based on the principle of echolocation. The device sends an ultrasound signal and receives it in the reflected form from various tissue environments. Ultrasound is informative, but it is not enough to determine adhesions.

It is carried out by means of an abdominal sensor, that is, through the abdomen and transvaginally. Despite its weak informativeness, the method is the safest. It can be carried out even on pregnant girls. However, a regular ultrasound examination does not provide enough information to determine the presence of adhesions. In this case, they resort to the help of UZGSS. It is based on the introduction of a special sterile substance into the uterine cavity. The specialist observes the movement of fluid and can make a diagnosis based on this.

Nowadays, ultrasound is not used very often, but it is not only effective in cases of obstruction. In general, it is an effective and safe examination.

Differential diagnostics

The patient's blood and urine are taken to detect signs of an inflammatory process. This can be indicated by altered ESR indicators, the level of leukocytes, and the appearance of C-reactive protein. Differential diagnostics are based on methods aimed at studying the blood and determining the levels of components contained in it.

If there are altered indicators, the doctor may assume the presence of an inflammatory process, as a result of which the obstruction developed. This will allow determining the further tactics of the diagnostic measures.

  • Bacteriological smear. It is taken to confirm/refute the presence of infection. Bacteria that have penetrated the woman's genitals could provoke an inflammatory process with a change in the structure of the mucous membrane. As a result, the development of adhesions is possible. The procedure for taking a smear is painless, provided that the infection is located at the level of the vagina. If it is located much further, the material for examination is taken from the fallopian tube area. Endoscopic methods are used for this process.
  • Hormonal analysis. In this case, the patient's blood is tested. Obstruction of the fallopian tubes may be related to hormone levels. They must be taken on certain days of the menstrual cycle.

Who to contact?

Treatment of uterine adhesions

Before starting treatment, the specialist must make sure that the patient has only obstruction. A standard comprehensive examination will determine the exact cause of the process and select the optimal scheme for their elimination. Treatment of uterine adhesions is aimed at the complete removal of the pathology. It can be carried out both conservatively and surgically.

Conservative treatment involves the use of anti-inflammatory therapy. It consists of the use of antibiotics, injections and physiotherapy. These methods will allow achieving positive dynamics, but only if no more than six months have passed since the formation of adhesions. If the adhesion process is pronounced, it will be necessary to resort to surgical intervention.

Surgical treatment is indicated at the age of up to 35 years, given that the woman has regular ovulation and the obstruction is partial. Even such a solution to the problem does not guarantee 100% success.

If a woman manages to become pregnant after the treatment, she should immediately contact a doctor. A specialist will determine the location of the fertilized egg to avoid the development of an ectopic pregnancy.

Medicines

A number of special medications are used to eliminate the adhesion process. They must be taken together to achieve the optimal therapeutic effect. In order to eliminate adhesions, they resort to the help of resorbable drugs. These include: Trypsin and Chymotrypsin. Antihistamines can be prescribed along with them: Diphenhydramine and Suprastin. Anticoagulants such as Heparin are also widely used. To relieve the inflammatory process, they use: Paracetamol, Butadion and Ibuprofen. To eliminate severe pain, they will help: Analgin, Tempalgin and Diclofenac. To cope with infections and speed up the healing process, antibiotics: Tetracycline, Trimezol and Biseptol.

  • Trypsin. The drug is administered intramuscularly. The dose is prescribed by a doctor, depending on the severity of the adhesion process. It cannot be used for bleeding wounds. It can lead to the development of burning, allergic reactions.
  • Chymotrypsin. The drug is administered intramuscularly, according to the instructions of the attending physician. It cannot be used in case of malignant neoplasms, bleeding wounds and individual intolerance. May cause itching and allergic reactions.
  • Diphenhydramine. The drug is taken 30-50 mg 1-3 times a day. The duration of therapy depends on the severity of the adhesion process. The drug cannot be used in case of hypersensitivity and bronchial asthma. It can lead to the development of fatigue, have a sedative effect and provoke tachycardia.
  • Suprastin. The drug is used during meals, 0.025 grams 2-3 times a day. In severe cases, it is administered intramuscularly. It is not recommended for people whose work requires increased concentration. It can lead to the development of general weakness and drowsiness.
  • Heparin. Doses and method of administration are prescribed individually by the attending physician. The drug should not be used for bleeding of any localization and hypersensitivity. It can provoke bleeding and allergic reactions.
  • Paracetamol. The drug is actively used to eliminate the inflammatory process. Its daily dose depends on its severity. The amount of the drug used should not exceed 3 tablets per day. It is not recommended to use it in case of hypersensitivity, as well as during pregnancy and breastfeeding. It can cause drowsiness, nausea, allergic reactions.
  • Butadion. The drug is used 0.1-0.15 grams up to 4 times a day. The duration of the course of treatment is 5 weeks. The drug should not be taken in case of gastric ulcer and liver and kidney dysfunction. It can provoke nausea and pain in the stomach.
  • Ibuprofen. The average dosage is 400 mg 3 times a day. The dosage is adjusted depending on the severity of the pain syndrome. The drug should not be taken in case of erosion of the gastrointestinal tract, ulcerative colitis and hypersensitivity. It can lead to the development of nausea, heartburn, constipation and even anorexia.
  • Analgin. The drug helps to cope with pain, for this purpose it is used in the amount of 2-3 tablets per day. The dosage can be changed depending on the severity of the pain syndrome. The drug cannot be used in case of severe hypersensitivity. It can lead to the development of disorders of the gastrointestinal tract.
  • Tempalgin. The drug is characterized by its effectiveness. Its effect is much higher than that of Analgin. It can be used in the amount of 2-3 tablets per day. In no case should the medicine be consumed with alcoholic beverages. Its use is prohibited in case of intolerance, pregnancy and in childhood. It can affect the functionality of the liver and kidneys, lead to the development of allergic reactions.
  • Diclofenac. The drug can be used both in tablet and injection form. In case of obstruction of the fallopian tubes, it is used orally, depending on the severity of the pain syndrome. The daily dose does not exceed 2-3 tablets. Only the attending physician can adjust it. The drug cannot be used in case of impaired liver and kidney function, pregnancy and hypersensitivity. It can cause nausea, vomiting, abdominal pain and allergic reactions.
  • Tetracycline. Before using the drug, sensitivity to microflora is carried out. The permissible dosage is 0.25 grams every 6 hours. The duration of therapy is individual. It is not recommended to take the drug for fungal diseases and leukopenia. May provoke allergic reactions.
  • Trimezol, Biseptol. The optimal dosage is 4 tablets per day. The effect of the drugs is the same. They should not be taken in case of severe liver and kidney dysfunction, as well as hypersensitivity. They can lead to the development of allergic reactions.

Suppositories for adhesions in the fallopian tubes

The use of suppositories will speed up the healing process. The use of suppositories for adhesions in the fallopian tubes is aimed at resorption and removal of the inflammatory process. For this purpose, such drugs as Longidaza, Meloxicam and Lidase are used.

  • Longidaza. Suppositories are intended for rectal and vaginal administration. Vaginally, they are used at night, one suppository at a time. The duration of treatment does not exceed 10 days. Rectally, suppositories are administered once every two days, the total duration of therapy is 20 days. They cannot be used in case of irritation, hypersensitivity, cracks, and renal dysfunction. They can cause allergic reactions.
  • Meloxicam. The total daily dose should not exceed 15 mg (one suppository). The duration of therapy is 10 days. Suppositories should not be used in case of ulcers, severe renal failure and hypersensitivity. They can cause drowsiness, dizziness, confusion, leukopenia.
  • Lidase. Suppositories are administered once a day, one at a time. The duration of treatment is 7-10 days. They cannot be used in case of hypersensitivity, bleeding. Suppositories can lead to the development of allergic reactions.

Injections for adhesions in the fallopian tubes

Injections help to cope with the pathological process. Their action is aimed at relieving inflammation, eliminating pain syndrome and resolving adhesions. Injections for adhesions in the fallopian tubes are prescribed by the attending physician, in combination with other treatment methods. The following solutions are actively used: Longidaza, Lidase and Plasmol.

  • Longidaza. The solution is administered intramuscularly, in a volume of 3000 IU. The duration of treatment is 5-15 days. Everything depends on the severity of the disease. The interval between injections can be 3-10 days. It is not recommended to use injections in case of malignant neoplasms, pregnancy, in childhood and hypersensitivity. Allergic reactions may develop, which disappear on their own after 2-3 days.
  • Lidase. The solution is administered subcutaneously and intramuscularly. For injection, the contents of one capsule are dissolved in 1 ml of 0.5% novocaine. The course of treatment depends on the severity of the adhesion process. On average, it is 10-20 days. Injections are not recommended for malignant neoplasms. The drug may cause allergic reactions.
  • Plasmol. The solution is administered subcutaneously. One injection per day, 1 ml, is sufficient. The duration of therapy is 10 days. The solution cannot be used in case of tuberculosis, endocarditis, nephritis and hypersensitivity. Allergic reactions may develop.

Douching with propolis for adhesions of the fallopian tubes

Propolis has become widespread in the field of traditional and alternative medicine. It is famous for its antibacterial and antiseptic properties. It is used to affect many organs and systems of the body. Douching with propolis is also widely used; it helps to cope with adhesions of the fallopian tubes.

To alleviate the condition, it is necessary to perform a procedure using a 3% alcohol tincture of propolis. The duration of treatment is 10 days. The use of douching, and then applications will allow you to achieve a positive result.

To increase the effectiveness, propolis is used together with calendula. It is necessary to take tinctures of these components in equal quantities. A tablespoon of the mixture is dissolved in 500 ml of warm water and douching is done. The resulting product can also be used as a lotion.

Douching with an alcohol-water emulsion of propolis will help to eliminate the lotions and remove the infection from the vagina. The course of treatment is 2-3 weeks.

To eliminate the inflammatory process, douching with a 3% propolis solution is used. The procedure is carried out once a day for 10 days.

Folk remedies

The healing properties of folk remedies cannot be underestimated. Today, there are many effective recipes that are widely used even in the treatment of serious diseases. Folk remedies can also cope with adhesions. The main thing is to prepare and use the potions correctly.

  • Infusion of cinquefoil decoction. The main ingredient is taken in the amount of a tablespoon. The cinquefoil must be poured with 300 ml of vodka and infused well. Take a tablespoon of the product daily, diluted in 50 ml of water.
  • Sage infusion. A teaspoon of the herb is steamed in a glass of boiling water. Everything is infused for 30 minutes and taken one third of a glass 3 times a day.
  • Milk thistle infusion. A teaspoon of seeds or stems of the main ingredient is poured with 250 ml of boiling water. The infusion should be taken warm. A new portion should be brewed each time.
  • Infusion of juniper berries. Take 15 grams of berries and let them steep for 4 hours in a glass of warm water. The resulting remedy should be consumed one tablespoon 3 times a day.
  • Infusion of mugwort root. It should be prepared in the evening. To do this, pour 3 tablespoons of crushed mugwort root into a thermos. Pour 3 cups of boiling water over everything. In the morning, strain the infusion and drink a glass 3 times a day.

You shouldn't neglect folk methods of treatment. But it's also wrong to take them as a basis. Folk treatment can be used in combination with other traditional methods.

The procedure is carried out individually and depends on the severity of the disease. Usually, 10-15 procedures are enough. To obtain the optimal result, it is recommended to undergo 2-3 courses, with an interval of 2 weeks.
