Causes and pathogenesis of penile gangrene and scrotum. It is believed that the gangrene of the penis and scrotum is caused by Staphylococcus aureus, streptococcus, less often - Proteus. In the pathogenesis of the disease, an important role is played by sensitization to pathogens and their decay products, the development of allergies, the involvement of skin vessels, the development of ischemia and necrosis.
Symptoms of gangrene of the penis and scrotum. The disease develops, often in men, unexpectedly, against a background of complete health, without any reasons. There are swelling of the penis and scrotum, fever (38-39 ° C), malaise. A few days later, surface necrosis develops, which seizes the entire front part of the scrotum and penis, causing tissue destruction. Eggs are never involved in the process due to the extensibility of the skin of the scrotum and the fibrous membranes that protect them. In most patients, the intensity of pain is expressed in a mild or moderate degree.
With the development of the disease in women, the pathological process is often localized on the small labia or in the clitoral region, passing into the perineum, less often to the pubic region and thighs.
Leukocytosis is noted, and ROE is enhanced.
Differential diagnosis. Gangrene of the penis and scrotum should be distinguished from cancer, complicated form of solid chancre, chancriniform pyoderma.
Treatment of gangrene of the penis and scrotum. It is recommended to determine the sensitivity of microflora to antibiotics, to prescribe broad-spectrum antibiotics in shock doses.
Applied fortifying and vitamin preparations, external - aniline dyes, ointments and creams containing steroids and antibiotics.
