Human skin is strewn with sweat glands - tubular structural elements that release excess liquid from the body (sweat) on its surface. The excretion of sweat is a protective reaction against overheating and elimination of harmful substances. This is a natural process, normal diaphoresis is the average daily loss of fluid from about 250 to 600 ml.
Increased sweating or hyperhidrosis - this is the allocation of much more fluid and the associated discomfort. At least - noticeable dark wet circles on clothes in the armpits, sometimes - on the back and chest, repulsive smell and other inconveniences. Excessive sweating in men, according to the frequency of treatment, is less common than that of the opposite sex, but it is abundant and brings no less discomfort.
Basically, the sweat glands are located on the skin of the forehead, palms, feet, in the armpits and in the groin, other parts of the body are equipped with them to a much lesser extent, and the thin skin of the lips and certain parts of the genitals in general is deprived of them.
Sweat consists mainly of water. Only one to two percent of this physiological substance is in salts, acids, other chemical compounds.
Eccrine sweat glands, located on the skin of the palms, feet, face, chest and back, respond to thermoregulation and elimination of metabolic products. They also respond to fever, stress physical stress, in addition, they contain acidic components that provide skin surfaces with bactericidal properties. For example, on the skin of the palms and feet, where there are no sebaceous glands, sweat provides also their natural lubrication, so to speak, performs a protective function, preserving the elasticity of the skin in these places and its working qualities (ability to touch, tenacity).
Apocrine sweat glands are located in the areas of haemophilia - underarms, genitals, perineum, on the head. Their functions are not related to protection, but provide humoral behavioral functions (attraction of individuals of the opposite sex in the most favorable period for reproduction). The most active activity of these glands becomes after reaching the fertile age and gradually fades as the body ages. Pheromones, fatty acids, and cholesterol are secreted by the sweat, it has an alkaline reaction and is more suitable for the reproduction of bacteria. Its smell is individual and unique in every person, although human sense of smell, unlike animals, no longer catches such subtleties. In this there is no need, we lost it in the process of evolution, using for recognition other senses. The smell of sweat, to be more precise, the products of the vital activity of the bacterial flora, which multiplies intensely on the wet parts of the body, is not pleasant to us. Therefore, excessive sweating causes, in addition to physical inconvenience, also moral, and regular hyperhidrosis, which acquires painful forms, may even cause social disadaptation of the person.

Epidemiology
With hyperhidrosis, according to various sources, one to three percent of the world's population is familiar. This is quite a lot. Moreover, medical statistics take into account only cases of official recourse for help. There are suspicions that most people are trying to cope with this problem on their own.
More than half of those who applied to doctors are women, so it is believed that they are more likely to suffer from hyperhidrosis. This ratio is explained by the fact that the fair sex is more emotional and their hormonal activity during life is higher. However, this does not mean that there are actually more of them. They are simply more concerned about this problem, in particular, its aesthetic side.
But men sweat more profusely. Studies show that with the same level of exercise, a man sweats much more than a woman.
The problem of increased sweating is most common in puberty, since it is then that axillary and inguinal glands are activated, and hairyness appears in these areas. When the hormonal background stabilizes, this problem remains in a much smaller number of patients. Their number at the fertile age is stable, and after overcoming the half-century age limit, the number of complaints about sweating is reduced, which is explained by a decrease in the activity of any glands in the body, including sweat.
Causes of the night and day sweating in men
Increase sweating both in a completely healthy person and in the patient, such household risk factors as - hot weather, physical activity, experiences, diet (hot meals and drinks, spicy seasonings), drinking (hangover or withdrawal symptoms), inappropriate clothing (synthetic, too tight), overweight, not too careful compliance with basic rules of hygiene. Often, these factors are combined with each other and with any disease, then the person sweats even more intensively. Normally, under the influence of these factors, depending on the intensity of their action, sweating increases from 800ml to 2-3 liters, however, it can reach 5-10l.
Increased sweating is characteristic of adolescence, when the hormonal balance changes.
Intensive sweating can be a side effect of taking medications, for example, aspirin, acyclovir, ciprofloxacin, insulin, anxiolytic and many others. The instructions usually indicate the possibility of such an effect. After completion of treatment, normal sweating is restored.
Pathological hyperhidrosis can be primary (idiopathic, essential). In some people, a tendency to increased sweating under any conditions and in the absence of diseases is observed among very close relatives, being genetically conditioned. In the pathogenesis of essential hyperhidrosis, this anatomical feature of the skin is considered as a much larger number of sweat glands on the body or certain parts of it and / or an increased excitability of the sympathetic nervous system, the consequence of which is excessive sweat secretion due to the slightest disturbances. Presumably the following factors, which are functional features, and not pathology, influence the level of conduction of sympathetic nerves:
- individual hypersensitivity to epinephrine and norepinephrine;
- an increased number of nerve impulses emanating from the subcortical structures of the brain and the ganglion of the autonomic nervous system, which is also regarded as a feature of the central nervous system;
- level of hormones (thyroid, sex), corresponding to the upper limit of the norm;
- a high level of the neurotransmitter serotonin, which provides the conductivity of nerve impulses in the fibers of the sympathetic departments.
Secondary hyperhidrosis develops against a background of a variety of chronic diseases. In this case, curing or achieving a long-term remission allows, as a rule, to get rid of excessive sweating.
The pathogenesis of enhanced sweat secretion triggers various causes, they are still in the study stage and the exact mechanism of stimulation of the sympathetic autonomic nervous system that is responsible for the regulation of sweat secretion has not yet been established.
However, some points can already be explained. For example, in hot weather, in a warm room, in warmer than required clothes, a physiological cooling process takes place - the surface thermal receptors of our skin transmit the impulses of overheating to the centers of thermoregulation. As a reaction to this information, a reverse pulse arrives to reduce the surface temperature of the body, which causes an increase in the release of the liquid for surface cooling. Accordingly, with physical stress, the energy of the skeletal musculature is released, which increases the surface temperature of the body. The same signals make it necessary to increase the allocation of sweat to protect against overheating.
When taking spicy and hot food in the mechanism of increasing sweating, the impulses transferred between the centers that regulate salivation and, accordingly, the process of sweating are involved.
The neurotoxic effect of ethanol, narcotic and medicinal substances is manifested in disorders in the brain systems responsible for excitation (glutamatergic) and inhibition (GABA -ergic), as a result of complex interactions of neurotransmitters, the thermoregulatory center in the hypothalamus is activated. In addition, the body tries to get rid of the toxic effects of alcohol metabolism products, including all mechanisms for their elimination, including through sweat glands.
With the abrupt withdrawal of psychoactive substances in dependent persons, the level of catecholamines providing translation of nerve impulses increases, which causes increased sweating.
The change in the concentration of catecholamines is observed in the pathogenesis of heart and respiratory diseases, pains of various etiologies, and also accompanies intensive physical labor.
The pathogenesis of psychogenic hyperhidrosis is caused by activation of the stress neurotransmitter norepinephrine and increased synthesis of adrenaline (stress hormone). In an excited state, all the centers come, including - and one that regulates the process of sweating. Even a slight excitement increases sweating, and there is nothing to say about serious stressful situations.
The cause of hyperhidrosis in men can be organic disorders in the areas of the head (hypothalamus, oblong brain) and / or the spinal cord involved in the process of thermoregulation and cooling through the excretion of sweat - congenital, inflammatory, traumatic.
Excessive sweating is possible with infectious diseases, especially when they occur in unexpressed erased forms, this may be one of the few signs of ill health. For example, with tuberculosis, syphilis, tonsillitis, urethritis, prostatitis and other diseases.
The pathogenesis of acute viral and bacterial infections is accompanied by intensive synthesis of pyrogens, having a direct effect on the processes of thermoregulation, causing fever and sweating.
Hyperhidrosis should be the cause of examination in an endocrinologist. It accompanies diabetes mellitus, thyroid dysfunction, other hormonal changes.

Endocrine generalized hyperhidrosis develops in neoplasms of nerve fibers - pheochromocytoma, carcinoid syndrome, against which there is an overproduction of hormones that stimulate the sympathetic nervous system and release of sweat.
Presence of primary and secondary tumors of lymphoid tissue, brain and spinal cord, adrenals, collagenoses, diseases of the cardiovascular, central and autonomic nervous system, respiratory organs affects the work of the thermoregulation center.
When the renal function is impaired, the elimination of fluid through the urinary system is reduced, which is compensated by increased sweating.
Stopping breathing in a dream is always accompanied by increased sweating, and such a respiratory syndrome occurs in men almost three times more often than in women.
The cause of sudden hyperhidrosis, especially at night, may be dysfunction of the testicles and as a consequence - a drop in testosterone levels. This can happen to a man at any age. And if young men have pathology, the physiological aging of the body is a natural process. Male menopause is not as pronounced and pronounced as female, but this event necessarily occurs, and a decrease in testosterone levels can cause night sweats in men over 50 years old. Sweating in a man as a sign of illness
The first signs of excessive sweating usually appear in adolescence, although in children with congenital anomalies of the hypothalamic-pituitary axis, a strong sweating is observed from birth. In this case, the child constantly keeps body temperature above or below normal, unstable blood pressure, there is a tremor of the extremities. The child is very restless and sweats even when dressed according to the temperature conditions.
Idiopathic (hereditary) hyperhidrosis usually also manifests itself in early childhood. But in adolescence, due to physiological changes, excessive sweating can increase. But after 40 years, the symptoms of primary hyperhidrosis begins to decrease. Its signs are expressed in a very intense sweat secretion in one specific place or several places, but locally. The course usually paroxysmal, sometimes the sweating is constant. Patients often can not determine for themselves what provokes sweating attacks, since they are not associated with disturbances, stress, overheating.
Sweating of the entire body surface (generalized) usually indicates the presence of serious health problems: endocrine and neurological diseases, some infections.
The smell of sweat can suggest the direction of the problem:
- Ammonia - indicates a problem with the kidneys; can be a sign of tuberculosis and endocrine pathologies;

The generalized endocrine sweating is characterized by the symmetry of sweat secretion sites and very strong sweating, after which it is necessary to completely change clothes. With a local sweating of almost any genesis, sweat stains are also located symmetrically.
The asymmetrical arrangement of stains on the clothes from the sweat that has appeared is evidence of damage to sympathetic nerve fibers.
In patients with diabetes mellitus, only the upper half of the body suffers from excessive sweating, the lower parts - the pelvic region and limbs, on the contrary, suffer from dryness. With diabetic hyperhidrosis, patients complain of symptoms such as thirst, a feeling of dryness in the mouth, rapid fatigue, frequent and profuse emptying of the bladder, poor wound healing.
Thyrotoxicosis, in addition to bouts of sweating, manifests an unstable mood, subfebrile condition, increased heart rate, an increase in the gap between systolic and diastolic pressure, weight loss in combination with increased appetite, intolerance to heat, insomnia, tremor of limbs, exophthalmos.
With pheochromocytoma, generalized sweating attacks usually occur after hypertensive crises, characteristic of the disease. At the end of an attack a person literally emanates later in combination with an abundant emptying of the bladder.
For carcinoid syndrome, characteristic symptoms along with hyperhidrosis are hyperemia of the upper body, abdominal pain, diarrhea, wheezing and shortness of breath due to respiratory tract spasms, failure of the valvular heart apparatus on the right.
Head sweating in men can have a normal physiological character and arise as a result of the action of exogenous factors that cause sweating.
If it arises periodically and is not associated with the above factors, then this condition can be a symptom of a disease. Most often this is manifested by hypertension, neuropsychic disorders, head skin diseases, in particular, fungal infection, allergy, increased intracranial pressure. Intensively sweats the head in people who are overweight, endocrine pathologies and cancer (night sweats). Also, head sweating in men may be a manifestation of primary hyperhidrosis, but such manifestations are seen already in childhood.
Excessive sweating on the forehead and scalp is often combined with sweaty palms and reddening of the skin of the face, the so-called blushing syndrome.
Local increased secretion of sweat on the face skin is characteristic of Parkinson's disease.
Night sweating of the head and neck, also called a cranial syndrome, is generally more characteristic of males and more often becomes a symptom of pathology, although it can be caused by heavy food and alcohol overnight use. A regularly wet pillowcase on the pillow and wet tangled hair in the morning, not caused by the stuffiness in the bedroom, should be the reason for a visit to the doctor, even if during the day you do not have anything to worry about anymore.
Pathological sweating in the groin in men can be observed with idiopathic hyperhidrosis, and may be a symptom of any somatic pathologies, most likely endocrine, infectious (especially urogenital lesions), oncology is not excluded. So the consequences of the treatment - drug and surgical - can appear. Increased sweating can trigger a violation of innervation in the perineal region due to a hernia or trauma to the lumbar spine. Sweating in the groin often affects people with excess weight, fans of close or synthetic underwear, tight jeans and trousers, neglecting the rules of intimate hygiene. This localization of sweating is rare. Excessive secretion of sweat in the perineal region can cause diaper rash, disruption of the integrity of the skin and attachment of a secondary infection - bacterial or fungal, the symptoms of which will be itching and sweating in the groin in men, accompanied by an unpleasant odor of sweat. At the same time, fungal skin damage of this localization also causes excessive sweating.
Increased sweating of the armpits in men (spots more than 10 cm), not caused by a rapid manifestation of emotions, physical stress, African heat and heredity, is most likely a symptom of hormonal changes, since on the skin in this zone, as in the inguinal, mainly apocrine glands, bearing information about sex and sexuality. Weakness and sweating in men can arise due to an androgen deficiency, the causes of which in adult young people are the acquired nature - trauma, orchitis, varicocele, endocrine disorders. There is such a deficit reduction in sexual activity, muscle mass, the appearance of fat deposits, mood swings. A decrease in testosterone levels may be a symptom of a malignant neoplasm of the prostate or testicles. Symptoms are increasing gradually to establish a lack of testosterone in the early stages of the need for examination and biochemical analysis.
Attacks of hyperhidrosis in men can be caused by an overabundance of testosterone. Sweat intensively stands out in the armpits, on the head, skin of the back, palms and feet. The higher the level of the main male hormone, the more intense the body sweats.
Sharp sweating in men can be with kidney diseases: nephritis, uremia, eclampsia, nephrolithiasis; cardiovascular pathologies in the acute stage: coronary heart disease, rheumatism, thrombophlebitis, hypertension; cerebrovascular disease affecting the center of thermoregulation; acute poisoning, in particular, fungi, insecticides. Night and morning hyperhidrosis is characteristic for pathologies of respiratory organs, in particular, tuberculosis and bronchitis, and also for androgen deficiency. Morning sweating, accompanied by pain in the heart and weakness or headache may be a harbinger of myocardial infarction or stroke.
Sweating of the buttocks in men occurs for the same reasons as sweating in other places. Simply more predisposed to such a localization of perspiration are people who suffer from fatness. The provoking factor is tight synthetic linen and clothing, neglect of hygiene measures, hernia and spinal injuries. In addition, fungal and bacterial lesions of this localization increase discomfort in this place and cause bouts of sweating. This condition is accompanied by itching and burning, hyperemia, a very unpleasant odor. Persistent sweating in the buttock area leads to violations of the integrity of the skin and increases the likelihood of secondary infection.
Tides sweating in men arise for various reasons, most often the operating factors are several. If such situations occur on a one-time basis and their clear cause-and-effect relationship with external temperature or load factors is tracked, then there is nothing to worry about.
Persistent sweating in a man should be a subject of examination, because, in addition to idiopathic hyperhidrosis, it can be a symptom of a serious illness, the treatment of which must be started as early as possible in order to achieve a positive result.
Complications and consequences
Sweating, if it is not a symptom of a serious illness, does not pose a danger to the health and life of the patient. However, the quality of his life, this feature affects negatively, at least, brings psychological discomfort.
Men who suffer from sweating, try to avoid handshakes and other forms of physical contact, do not dance at parties and once again hesitate to take off their jacket, knowing about the wet spots on the shirt.
Sometimes sweating of hands prevents to carry out habitual daily actions - subjects slip out.
Hygienic measures in these people take a lot of time - you have to take a shower several times a day, change clothes. And the opportunity for this is not always available.
As sociological studies show, about half of people with hyperhidrosis do not feel happy and are in a state close to depression. This is serious! More than 80% of people with excessive sweating feel uncomfortable when dealing with strangers, and the fourth part - and with close people. Approximately one-third of those surveyed have such a feature of their body that brings inconveniences, which they feel are significant.
The most frequent physical complication of hyperhidrosis is the attachment of a secondary infection - ever wet parts of the body, especially those that are constantly closed with clothing, with diaper rash, create a very favorable environment for the reproduction of fungus and bacteria.
Sites of the body, on which sweat is excessively secreted, are prone to fungal infections, papillomas and warts prolapse, the formation of abscesses. Such processes are accompanied by an unpleasant odor (bromhidrosis).
Increased sweating also contributes to more frequent exacerbations of chronic skin diseases, for example, atopic dermatitis.
Diagnostics of the night and day sweating in men
The doctor necessarily conducts a survey and examination of the patient. During the interview, it is usually specified how long the patient has suffered from excessive sweating and what preceded her appearance, the patient constantly sweats or periodically, how often it becomes necessary to change clothes, shower. In addition, the doctor is interested in what events usually provoke attacks of hyperhidrosis, at what time of day these seizures occur more often, whether close relatives of the patient are prone to sweating, and also whether the patient has chronic diseases.
On examination, visual signs are visible - stains on clothing, maceration of the skin, rash, hyperemia of the face and upper body. Survey and questioning can only establish the presence of excessive sweating, and to identify the causes of laboratory and instrumental research.
Standard tests that are assigned to determine the causes of excessive sweating include: general blood and urine tests, blood biochemistry, determination of blood sugar, thyroid hormone levels. Investigate the blood for the presence of the patient syphilis, HIV infection, hepatitis B and C. If necessary, other analyzes and consultations of related specialists may be prescribed.
Mandatory instrumental diagnostics include fluorography or lung radiography. If necessary, ultrasound examination of the peritoneal and pelvic organs, the thyroid gland, electrocardiography and other studies may be prescribed depending on the expected pathology.
In addition, there are methods to assess the qualitative characteristics of sweat and the intensity of its release. True, they are rarely used, more for research purposes, since for the choice of a method of practical elimination of excessive sweating, their results are not of decisive importance.
The most commonly used iodine-starch method (Minor test). Allows you to estimate areas that are prone to excessive sweat secretion, used to compare the situation before and after treatment. The test is not at all complicated: the skin in the area of excessive sweating is smeared with a cotton swab dipped in iodine solution, allowed to dry and sprinkled with starch. When sweat starts to emit, it reacts with the substances applied to the skin with the formation of dark purple spots. The spots are surrounded by a marker and photographed, this procedure is usually performed before injections of botox or laser beam treatment.
Gravimetry - the degree of excessive sweating is determined by the removal of prints within a minute from the active sweating site to a special sheet of paper with hygroscopic properties. The conclusion is made on the weight of the paper before and after applying to the body, allows you to determine the weight of the sweat that was released in a minute.
