Changing the joints of the foot inevitably leads to a violation of its normal biomechanics; the occurrence of local overload zones; violation of the supporting, pressor and balancing functions. In these cases, the change in the relationship in the stop-support system can help by using various orthopedic corrective devices, the insoles are usually used.

Defeat of the foot joints is one of the most common orthopedic problems of rheumatological practice. In 90% of patients with rheumatoid arthritis there are lesions of the feet. In osteoarthritis, the lesion of the first metatarsophalangeal joint, according to the frequency of spread, ranks third after the pathology of the knee and hip joints.

In addition, the foot base of the entire musculoskeletal system plays a crucial role in the distribution of dynamic loads in all other links. Determination of the distribution of dynamic loads between the lateral and medial condyle of the tibia from the position of the foot served as a prerequisite for the use of insoles with an elevated lateral margin in patients with medial gonarthrosis.

The difference in the length of the limbs more than one centimeter can be one of the causes of the pain syndrome in the lower back. In these cases, an important pathogenetic treatment measure is the compensation by the orthosis of the length of the shortened limb.

What are the insoles for the insoles?

Insoles insoles improve the function of the foot due to the uniform redistribution of static and dynamic loads, eliminate the zones of local overloads (especially in the region of the heads of metatarsal bones), set it to a neutral position, reduce excessive pronation in the subtalar joint during the re-rolling, restoration of the spring function. Compensation of the difference in length of the lower limbs, redistribution of loads between the condyles of the tibia with medial gonarthrosis. Supinic insoles help in the prevention of recurrence of deformities after reconstructive operations on the foot and ankle in patients with rheumatological diseases.

Indications and contraindications

Deformation of the forefoot (hallux valgus, hallux rigidus, hammer-shaped deformation of fingers, subluxation of metatarsophalangeal joints); metatarsalgia; flat or hollow table; plantar fasciitis; heel spur; medial gonarthrosis; state after reconstructive operations on the foot and ankle joint.

There are no contraindications.

How to choose the insoles of the arch supports?

For the correct choice of tactics of orthotics, a thorough assessment of the condition and function of the foot is necessary, using X-ray, sub-metric, planographic and, if possible, subgraphic methods.

As in other groups of orthoses, there are two approaches: orthosis using standard factory blanks or individual manufacturing of insoles insole. The use of standard semi-finished products and finished products is possible if the dimensions of the foot correspond to the parameters of the factory models. However, pronounced deformities of the foot - a contraindication for the use of standard orthoses (hypercorrection can aggravate the pathological condition). For the production of a gypsum model, the method of producing a negative impression of the foot is used both as a non-loading and loading (preferably) method for individual production.

Effect

Reducing pain and improving walking parameters. Using insoles of insteps in patients with gonarthrosis reduces the need for NSAIDs. The preventive role of foot orthoses has not been studied.

Factors affecting efficiency. Selection in strict accordance with individual parameters of the foot and features of functional disorders.

Complications

In case of improper selection of the insoles of the insteps (discrepancy with the relief of the foot), painful sensations during walking can increase, there are rubs and calluses. In this case, its use should be stopped immediately and ask for help from a specialist.

Alternative methods

Use of special orthopedic shoes. If the insoles are not effective, surgical treatment is used.
