Causes of purulent-infiltrative omentitis
Presence of a purulent process in the abdominal cavity or cavity of the small pelvis. In acute inflammatory diseases of the abdominal cavity, the free edge of the large omentum, as a rule, is part of the conglomerate, thus limiting the inflammation focus from the free abdominal cavity. Inflammatory changes in the omentum may be from minor (free edge is soldered to the parietal peritoneum and capsule of tubo-ovarian formation) to expressed. In some cases, during a prolonged course of a purulent process, the gland can be changed all the way to the transverse colon (in the form of a shell), contain abscesses of various sizes in its structure and reach considerable dimensions, and its thickness may be 5-6 cm.
In such cases, the entire inflammatory conglomerate is easily palpable through the anterior abdominal wall, occupying the small pelvis, as well as hypo- and mesohastral abdominal parts.
Symptoms of purulent-infiltrative omentitis
In the clinical picture, along with signs of a purulent-infiltrative process in the abdominal cavity (in the acute stage - pain, temperature, other intoxication, as well as partial intestinal obstruction, in the remission stage - weakness, propensity to constipation and symptoms of prolonged chronic intoxication) reactive pancreatitis (nausea, vomiting, flatulence, moderate increase in blood and urine diastase, and transaminases), which requires appropriate correction.
Diagnosis of purulent-infiltrative omentitis
An unchanged and even altered epiploon is rarely separately identified on the echogram, more often in the abdominal cavity a conglomerate of organs (infiltrate) is defined, which includes an omentum.
Features of the operational aid
In the presence of purulent-infiltrative omentitis, the omentum resection is performed within the "healthy" tissue, the gland sticks are carefully ligated "with turnover" with preliminary piercing with a thin vikril or catgut.
Prevention of purulent-infiltrative omentitis
To prevent all forms of complications of purulent inflammation of the internal sexual organs, an active tactic of management of patients with purulent inflammatory diseases of the internal genital organs is required, and, first of all, timely operative treatment.
