Pathologies of the reproductive system that cause frequent urination include prostatitis, prostate adenoma, prostate stones, and malignant tumors. Frequent urination is one of the manifestations of these diseases (most often prostate adenoma), but they can also serve as a source of ascending infection, especially with ascending urethritis, orchitis, and epidermitis. Normally, the prostate is 2.5-3.5 by 2.5-3.0 cm in size, its contours are clear, the interlobar groove is pronounced, the lobes are symmetrical (although the right one may be slightly larger), the consistency of the glands is elastic, uniform, palpation is painless, the rectal lining above it is mobile and painless. The prostate juice normally contains in the field of vision: leukocytes up to 6-8, erythrocytes - 2-4, lecithin grains - 20-40, Trousseau-Leleman bodies - 6-8, there may be a small amount of mucus and desquamated epithelium.

Prostate adenoma

Prostate adenoma develops in more than half of men, in the presence of prostatitis, frequent urination appears after 35 years, but the main age is 50-60 years. Mostly men who lead a sedentary lifestyle get sick. It is based on a gradual increase in the size of the gland, sometimes 3-4 times, it is of a doughy consistency, symmetrical, painless. In the clinical course, several stages of development are distinguished:

Stage 1 (preclinical): symptoms such as periodic and slightly frequent urination, discomfort in the perineum and rectum, lower abdomen. Early signs include: impotence, premature ejaculation and hemospermia (with the latter - oncovigilance).

Stage 2 (dysuria and dystonia) is characterized by frequent urination, which is present first at night, usually closer to the morning, and later during the day. Moreover, there is a frequent symptom of "imperative urge", sharply expressed, accompanied by urinary incontinence, but unlike prostatitis and cystitis, it is not accompanied by pain. The urine stream weakens, more at the beginning, a thin stream falls vertically, "on the boots", even when straining. There is no residual urine.

Stage 3 (period of residual urine). The clinical picture is the same as in the second stage, but there is a double process (at the first stage there is no satisfaction, and after 20-30 minutes there is a repeated urge with the release of residual urine). Confirmed by ultrasound of the prostate and bladder with determination of the amount of residual urine (sometimes it can reach 1.5-2 liters). At this stage, the overlying urinary organs are already affected.

Stage 4 (paradoxical ischuria - "urine retention with incontinence"). The sphincters of the bladder cannot withstand a large amount of residual urine, and it begins to separate in drops, frequent urination develops against the background of urinary incontinence with impaired urination. The upper sections of the urinary system suffer significantly, up to the development of uremia.

Prostate stones

They are often formed against the background of prostate adenoma and chronic prostatitis, but in practice they are quite rare. There is no typical clinical picture, it proceeds as prostatitis or prostate adenoma: pain in the perineum, painful and frequent urination, sometimes hematuria and hemospermia are detected. They are detected by ultrasound of the prostate and survey radiography of the lower part of the pelvis.

Prostatitis is quite common and is the main cause of ascending infection with damage to the bladder, ureters and kidneys, in addition, during an exacerbation, due to swelling, they themselves can cause frequent urination. In all cases of chronic prostatitis, it is necessary to examine the patient for a specific infection.
