Currently, the debut of dysmenorrhea in 75% of cases coincides with the menarche, and only in every fourth patient the symptoms of dysmenorrhea occur 1-4 years after the menarche.
The main symptom of dysmenorrhea is pain syndrome. Monthly expectation of pain affects the overall well-being, emotional and mental activity. Survey of patients often contributes to the establishment of family cases of dysmenorrhea - the presence of the disease in the mother or the next of kin.
With a predominance of sympathetic vegetative tone, the girl, in addition to painful feelings, complains about:
- pain in the heart and palpitations;
- mood changes (internal tension and anxiety, uncertainty, obsessive fears, pessimism, down to the development of depression);
- an intense headache in the type of migraine;
- disruption of the intestine (intestinal colic due to spasm of arterioles, constipation);
- sleep disturbance, up to insomnia;
- general weakness and dizziness;
- fever with chills or internal tremor;
- increased sweating and the appearance of red spots on the neck in the form of a vasculitis about necklaces;
- nausea;
- increased frequency of urination;
- feeling of heat.
They note the pallor of the skin and acrocyanosis, the dilatation of the pupils. With the predominance of a parasympathetic type of response to menstrual pain, another clinical picture unfolds. Patients complain about:
- bloating and diarrhea;
- a noticeable increase in body weight on the eve of menstruation;
- swelling of the face and extremities;
- decreased efficiency;
- drowsiness;
- the occurrence of itching or allergic reactions;
- lowering body temperature and blood pressure;
- vomiting and increased salivation at the time of the onset of pain;
- increased chilliness at the time of attack of pain; about attacks of suffocation, accompanying pain;
- convulsions and fainting at the time of a bout of pain;
- the appearance of passive-defensive behavioral reactions.
