Conservative treatment is used in rare cases, for example, for the treatment of polyps of the nose with formations of small size. It is designed to neutralize the influence of factors contributing to hyperplasia of the mucous membrane, to eliminate infectious and / or allergic processes in the nasopharynx, to correct the immunity.
Drug therapy is used in cases when surgical intervention is impossible due to decompensated cardiovascular pathologies: hypertension, coronary heart disease, severe heart failure or bronchial asthma; disorders of blood clotting processes.
In such cases, therapy with corticosteroids, most often, Prednisolone in high doses and for a long period of time. The drug inhibits cellular proliferation, the growth of the polyp slows down, and it can regress. Nevertheless, systemic therapy is fraught with serious complications from the immune system, inhibition of the adrenal function, the development of peptic ulcer and other side effects of glucocorticosteroids. To avoid undesirable effects, a local introduction of corticosteroid hormones directly into the polyp is practiced. Such medical polypotomy leads to the destruction of the polyp, the withering away of the tissues of the mucous build-up and the removal of residues by natural means with detachable mucus. The patient "flies" the polyp. As a rule, two injections are done: first - one, two weeks - the second. For each patient, the approach to dosing and the number of injections, as well as the choice of the drug, is individual. Improvement occurs within one or two months.
After elimination of polyps, appropriate treatment is selected for the prevention of relapse.
In gynecological practice, attempts are being made to medically treat endometrial polyps or the cervical canal. Basically, medication treatment is considered inexpedient and inefficient, but there is evidence that sometimes it is possible to reduce the size of the build-up or even its regression. Especially often, this treatment is used at a young age of the patient, small sizes of polyps, negative attitude of the patient to surgical intervention.
Various drugs are used depending on the identified etiologic factors that have caused polyposis sprouting.
Patients under 35 years of age are prescribed hormonal contraceptive pills, for example, Regulon. This is a combined preparation for oral contraception, which contains-ethinyl estradiol and desogestrel. Its action is based on inhibition in the pituitary system of the synthesis of follicle-stimulating and luteinizing hormones. The lack of these hormones makes it impossible to ovulate, increases the density of mucus in the cervix and slows down the cell proliferation. The duration of treatment is usually not less than six months.
Patients over 35 years of age are assigned progesterone analogues, for example, Dufaston, which selectively acts on the progestin receptors of the intrauterine epithelium, neutralizes cellular proliferation that occurs with excessive estrogen exposure. Does not affect the metabolic processes and actions inherent in other progestogen preparations. Virtually no side effects. The doctor appoints the treatment plan individually.
In the premenopausal period, agents that reduce the negative effect of estrogens on the uterine mucosa are used. Synthetic analogues of gonadotropin-releasing hormone (Zoladex) are prescribed, which reduce the level of estradiol.
In the presence of infection and the inflammation caused by it, a course of antibacterial, antiparasitic or antiviral therapy is prescribed.
However, in most cases, the presence of fibrotic polyp is a direct indication of its removal. After the histological examination, the tactics of further treatment are determined with the aim of preventing relapses.
If the patient was diagnosed with fibrotic polyps and there were no signs of hormonal disorders - the menstrual cycle was not violated, then hormone therapy is not prescribed after removal of the polyp. An anti-inflammatory therapy may be prescribed if there is evidence to this.
If the glandular fibrous polyp is diagnosed, then such patients are shown hormonal therapy depending on age - by means for oral contraception or progestogens.
Polyps of the stomach, having a hyperplastic nature and small sizes up to 10 mm, can not be removed. However, it is necessary to observe, passing the endoscopic examination of the cavity of the stomach every year. In the presence of gastritis, the detection of Helicobacter pylori infection, the gastroenterologist prescribes a course of appropriate therapy, diet. It is necessary to abandon bad habits - drinking, smoking. Drug treatment will be aimed at eliminating the pathology that led to polyposis. With proper treatment and elimination of the cause, a regression of polyposic growth can occur. With increased acidity, drugs that reduce the secretion of gastric juice and protect the stomach epithelium from the aggressive action of hydrochloric acid are used: Omeprazole, De-Nol, Gestal, Almagel and others. With reduced acidity, preparations are prescribed to stimulate its production, Plantaglucidum, Romazulon, Pepsidine, preparations of gastric juice. Elimination of Helicobacter pylori is carried out by standard methods of eradication of these bacteria. Polyps, having a size of more than 10 mm, are subject to removal.
The only real method of treating polyps of the intestine, urethra, and bladder is their removal. Drug treatment on detection is not practiced and can be prescribed in the presence of background diseases for the prevention of relapse.
Alternative treatment of fibrotic polyps
Recipes of alternative medicine suggest getting rid of polyps in different organs. There are universal prescriptions concerning neoplasms in general. Of course, it is very doubtful that with the help of treatment at home it is possible to achieve elimination of fibrotic polyps, especially large ones. However, small, not requiring immediate surgery, can be regressed. After removal of growths, the use of alternative medicine will be a good preventive measure to prevent recurrence of the disease. However, if you are prescribed hormonal or anti-inflammatory therapy with medicines, then before using znacharian methods, it is worth consulting with a doctor whether they can be combined.
From the polyps in the nose are applied washing with sea water. It is prepared by dissolving in tepid boiled water (1/4 liter) a teaspoon of sea salt. When using ordinary rock salt, two drops of iodine solution and an incomplete teaspoon of baking soda are added to the solution. Stir well and filter so that no salt crystals remain dissolved. To wash the nose, you can use a small rubber pear or a disposable syringe 10-20ml. Leave the mucus from the nose after the procedure carefully, so as not to damage the vessels of the mucosa and not cause bleeding.
In polyps in the nasal passages, it is recommended to lubricate them with honey using a cotton swab. Constantly, until they disappear.
Another simple means - you can inhale the smoke from a warmed piece of propolis.
In polyps in endocervix, it is recommended to put intravaginally suppositories, from the soap slices cut into pieces.
With polyps of any localization it is recommended to drink infusion of pine needles from young spruce or pine shoots. They are collected in the autumn after the first frost. It is believed that the use of coniferous infusion has an anti-inflammatory effect, prevents malignancy and growth of polyps, and also - a large amount of vitamin C in coniferous needles strengthens the immune system and increases the body's resistance.
To prepare the beetroot, the needles are peeled off from twigs, washed under running water, ground and covered in a thermos with six tablespoons of raw material prepared in this way. The infusion is prepared in the evening. Pour ½ liter of boiling water and leave until the morning. In the morning, strain and before each meal take a table spoon.
Polyps of any localization will come out after using the following means: seven yolks from hard-boiled fresh (no more than three days) chicken eggs are ground with six tablespoons of gruel from the purified pumpkin seeds (they should be dried at room temperature, they can not be fried or oven-dried) and fill 0.5 liter of unrefined sunflower oil. Stirred. A mixture of half an hour must be protested in a water bath. After that, they let it cool and clean in the refrigerator. In the morning on an empty stomach take a teaspoon of the mixture for five days, then take a break for five days, and again spend a five-day course. It is necessary in this way to eat the whole portion.
The main plant used in the treatment of herbs in polyps is celandine. It is used alone and in combination with other medicinal plants. There are many recipes, however, modern studies have shown that celandine works only with direct contact with the polyp. For example, with polyps of the intestine it is necessary to do enemas with celandine, nasal - to wash the nose, drink - with polyps of the stomach or gall bladder, make swabs and syringings for polyps of the cervical canal and endometrium. Proportion of cooking infusion for such procedures: for 200ml of boiling water, two tablespoons of chopped herb.
Another popular plant used to eliminate gynecological problems is the uterine borovaya. Infusion is prepared as follows: 200ml of boiling water pour a tablespoon of dried raw fines. Insist until completely cooled. Drink twice a day for 100ml before meals. Every day you need to prepare a new portion of the present.
In herbal collections from polyposis, different plants are used - chamomile, calendula, sage, thyme, yarrow, cones of hops. All of them contribute to the reduction of inflammatory processes, strengthen immunity, reduce feelings of discomfort. However, it is worth remembering that sometimes plant remedies do not combine with medications and have contraindications, so before using it, you need to study this issue well.
Homeopathy
In the treatment of fibrotic polyps of different localizations and origin, various drugs are used. One of the more well-known antitumour agents is Thuja, the tree of life. It is used for different localizations and types of tumors, including those with a tendency to polypobrazovaniyu. Lycopodium (Plune clavate) is given to patients with polyps in organs located mainly on the head - in the nose, ears, eyelids. Pulsatilla (a meadow chamber) is a female drug, it is prescribed for polyps in the uterus and urethra, sometimes in the nose.
Often, mineral substances are used: Alumina, Arsenic album, Aurum, animal components - Sepia (contents of a bag of black cuttlefish), Apis (honey bee), Lachesis (the poison of a South American snake).
To treat a fibrotic polyp with homeopathic remedies, it is necessary to consult a homeopathic doctor. The arsenal of drugs used to eliminate such pathology is very large, therefore, an expert in this field can only choose an effective drug that takes into account the features of manifestations, localizations, concomitant diseases and traits of the patient's character.
Official medicine uses homeopathic medicines manufactured by the pharmaceutical industry. They can be appointed to eliminate the causes that caused polyposis growth (stopping inflammation, normalizing the hormonal background), and also in the postoperative period to prevent relapses. For polyps in the uterus, oral drops of Mulimen, Klimakt-Heel or Klimaktoplan tablets may be prescribed, the spraying of Euphorbium compositum cenotropfen C, the gastrointestinal tract polyps Nuks-vomica Homaccord can be included in the treatment of nasal polyps. In the treatment regimen, stimulators of metabolic processes and tissue respiration, Ubihinon and Coenzyme compositum, can be included. Homeopathic medicines and their combinations should be prescribed by a doctor who is familiar with the specifics of their action. In this case, they can make a worthy alternative to hormone therapy.
Removal of the fibrous polyp
The main and most effective method of combating polypous growths is their removal. Currently, preference is given to low-traumatic methods of polyp removal, although in large formations classical surgical procedures are used. In addition, much depends not only on the size of the polyp, but also on its location, as well as the degree of equipment of the medical institution.
Nasal polyps are currently removed by the following methods: using a polyp loop, an endoscope, a laser knife. Classical polypotomy with the help of a metal loop is practiced now. This is the most traumatic method, the recovery period after removal of the loop is the longest and, in addition, it can be used to get rid of only the polyps that are located in the nasal passages.
Using a laser, the removal of polyps occurs virtually bloodless and quickly, in an outpatient setting. The laser beam can also be processed only by single polyps located in the nasal passages.
To remove externally accessible polyps, cryodestruction (removal with liquid nitrogen) or thermal destruction, when quartz fiber is introduced into the formation, by means of which the formation is heated to 70 ℃, is also applied. Both these procedures lead to rejection of the growths in about three days. If they do not go out alone, the doctor removes the remnants of the tweezers.
The most progressive method at the present time is the removal of polyps with the help of endoscopic equipment. This method can be used for any arrangement of polyps and any number of them. Endoscopic operations are carried out with the help of tools or shaver (a sighting device that provides high accuracy), while only diseased tissues are removed, and healthy ones are not affected.
External polyps of the urethra in women are removed by the classical method with the help of a scalpel, cryodestruction, electrocoagulation is also used. The most modern method of removal is the layer-by-layer excision of the formation of a radiosurgical knife with the aid of Surgiton apparatus. With a large polyp size, the method of wedge resection of the urethra using a scalpel or a radio knife is used.
To remove polyps of the urethra in men and polyps of the bladder in persons of both sexes, the method of choice is endoscopic.
Removal of polyps of the stomach up to 3 cm is performed with the help of endoscopic equipment. Laparoscopy is done (endoscopes are injected through small punctures) or as in diagnosis - through the mouth and esophagus.
When the size of the polyp is more than 3 cm and / or suspected of cell degeneration, an open cavity operation is performed. In the presence of malignant process, necrosis or pinching the legs of the polyp perform gastrectomy.
With the help of endoscopic equipment, operations are carried out to remove polyps by a dielectric loop (electrocoagulation) or laser. The first method significantly reduces the risk of bleeding, the second is rarely done - with build-ups less than 10mm, layer-by-layer cauterization of the polyp with a laser is accompanied by smoke and poor visibility, and there is no material for histological examination.
Polyps located in the intestine are removed by such methods:
- endoscopic with the help of electrocoagulation endoplay (so remove small benign formations of the middle parts of the intestine);
- endoscopy using biopsy forceps is used to remove large growths (they are removed by pieces);
- laparoscopy - removal of special tools through small incisions;
- open cavity operation, sometimes with resection of part of the intestine.
The best method for removal of endometrial polyps today is hysteroscopy. Scraping the uterine cavity and removing the polyp takes place under the control of the optical system and makes it possible to make sure that all formations are eliminated not only in the uterine cavity, but also in its neck.
According to the indications and in the absence of appropriate equipment, scraping of the cavity and cervix of the uterus is performed blindly (curettage).
The most effective and low-traumatic method of eliminating polyps is burning them with a laser beam.
Cataracts on the cervix are also removed using a radio-wave knife and cryodestruction.
