Perforating defects of the alveolar processes of the upper jaws occur most often when the upper small and large molars are removed, immediately or after a few days appearing as follows:
- when the head is tilted down and to the healthy side, blood is extracted from the nasal cavity (immediately after the extraction of the tooth), water or food (after some time after this operation);
- the patient can not "cheat" cheeks, play wind instruments, blow his nose, intelligibly pronounce the sounds of p, b, c;
- when trying to exhale through the nose with clamped nostrils, bubbles are visible in the perforation area; if it appeared against the background of acute purulent sinusitis, pus is abundantly released from the alveolus, and if the polyp is protrude against the background of a chronic polyposis.
Treatment of perforation defects of the alveolar processes of the upper jaws
Treatment depends on the background of which disease there was a perforation of the oral cavity with the maxillary sinus. If the mucous membrane of the sinus is not inflamed, you can immediately eliminate the perforation of the flap on the leg, facing either on the threshold of the mouth, or (if the threshold is shallow or deformed by scars) to the sky.
With inflammation of the mucous membrane of the maxillary sinus, appropriate conservative treatment of sinusitis is carried out prior to obtaining a "clean" wash liquid (during irrigation of the sinus through the perforation); then produce a typical trepanation of the maxillary sinus through access via Wassmund-H. I. Zaslavsky, revision of the alveoli (remove root debris, sequestration, granulation of the alveoli) and eliminate the defect with the help of plastics.
